1. Ask Codex to investigate before editing
A bug report is easier to investigate when it includes the software version and environment, steps to reproduce, and the expected and actual behavior. Add a minimal reproducible example when possible. Those details also help distinguish a confirmed cause from a plausible guess. OpenAI’s Codex Bugs category guidance calls out the version, environment, reproduction steps, and expected versus actual behavior.

First tell me the most likely failure path and what evidence in the code supports it. If you cannot reproduce the bug or identify the cause, say what is missing and stop before proposing a patch.The stop condition helps keep an uncertain diagnosis from turning straight into a speculative patch. It sets a blocked condition; it does not establish that Codex can reproduce the issue in every environment.

2. Set a boundary for the patch
State both the behavior to fix and the scope Codex should stay within. Naming files or a component gives you a concrete boundary to compare with the eventual diff. If the fix appears to require a wider change, ask for the reason before work moves beyond that boundary.

Do not change unrelated behavior. Before editing outside that boundary, report the reason.Explicit constraints make the request and the resulting changes easier to compare, but they do not guarantee that the agent will obey the boundary. 3. Request a regression test and relevant edge cases
A fix should have a test that would fail on the reported bug and pass with the correction. Ask Codex to follow the repository’s existing test pattern, then name edge cases and failure paths that matter for this behavior. Avoid a generic request for “more tests” when you can identify the cases that could expose the same underlying problem.

4. Specify the checks and request exact results
Name the test, lint, or build commands that provide meaningful verification for this bug. Ask for the exact commands and outcomes, including any check that could not run. A command that was not run is not evidence of a passing check.
Run [specific test command] and [any relevant lint/build command]. Report the exact commands and whether each passed, failed, or could not run. 5. Ask for a reviewable handoff
Finish by asking for a concise route from the diagnosis to the evidence: the root cause, changed files, behavior change, tests and checks run, and remaining uncertainty. Have Codex point to relevant code and test evidence, and distinguish what it could verify from what it could not.

How to adapt the prompts to a real bug
Fill in details that are specific enough to distinguish this failure from nearby behavior. Set a narrow boundary where the likely fix belongs, but allow Codex to explain why a necessary change would cross it. Choose tests and commands that actually exercise the affected code. If the cause, reproduction, or check remains uncertain, keep that uncertainty visible instead of treating a confident summary as verification.
