If you notice a sudden sign of stroke, call emergency services now. In the United States, call 911; elsewhere, call your local emergency number. Do not wait to see whether symptoms go away. Note when they began and tell the dispatcher and responders.
What are the early signs of a stroke?
Stroke warning signs typically appear suddenly. They can include:

- Numbness or weakness in the face, arm, or leg, especially on one side of the body.
- Confusion, trouble speaking, or difficulty understanding speech.
- Trouble seeing in one or both eyes.
- Trouble walking, dizziness, or loss of balance or coordination.
- A sudden severe headache with no known cause.
The U.S. Centers for Disease Control and Prevention lists these as stroke symptoms and advises calling 911 right away: CDC: Signs and Symptoms of Stroke. You do not need to see every symptom. A single sudden change is enough to treat the situation as an emergency.

Use B.E. F.A.S.T. as a memory aid
- B — Balance: Watch for sudden dizziness, trouble walking, or loss of balance or coordination.
- E — Eyes: Look for sudden vision changes or trouble seeing.
- F — Face: Ask the person to smile, if you can do so without delaying the call. Check for drooping or an uneven smile.
- A — Arms: Ask the person to raise both arms. Notice if one drifts down or cannot be raised.
- S — Speech: Listen for slurring or difficulty speaking. If useful, ask the person to repeat a simple phrase.
- T — Time: Call emergency services immediately.
These are clues, not a diagnostic test. The American Heart Association and American Red Cross recommend immediate EMS activation when stroke is suspected; their 2024 adult first-aid guideline also recommends using a recognition scale such as FAST: 2024 AHA and American Red Cross First Aid Guidelines. A normal-looking face, arms, or speech does not rule out stroke if another sudden warning sign is present.

FAST and B.E. F.A.S.T.

What should I do if someone may be having a stroke?
- Call the emergency number now. In the United States, call 911. If you are elsewhere, call the local emergency response number. Tell the dispatcher you suspect a stroke and describe what you observed.
- Note when symptoms began. If you know when the signs first appeared, record the time and share it with dispatchers and clinicians.
- Follow the dispatcher’s instructions and stay with the person while help is on the way.
- Do not wait for symptoms to pass or try to diagnose the cause at home. Some other conditions can resemble stroke, but working out the cause should not delay the emergency call.
What if the symptoms go away?
Call emergency services anyway. A transient ischemic attack (TIA) can cause the same kinds of symptoms and may last only minutes. The American Stroke Association warns: “Don’t ignore these warning signs, even if they go away.” A suspected TIA also needs urgent medical attention: American Stroke Association: Stroke, TIA and Warning Signs.

Why does the time matter?
A stroke interrupts blood flow to the brain, and brain cells can begin to stop working and die within minutes. The American Heart Association estimated in a May 1, 2026 public release that nearly 2 million brain cells die per minute during an untreated stroke. That is an average estimate, not a patient-specific rate or a countdown.

Treatment options depend on clinical evaluation. According to the American Stroke Association, eligible patients with ischemic stroke may receive alteplase or tenecteplase within 4.5 hours of symptom onset; selected eligible patients with large clots may have a thrombectomy as soon as possible within up to 24 hours. These are treatment windows, not a reason to wait: American Stroke Association: Stroke Treatment. Clinicians determine what treatment is appropriate after evaluating the individual patient.
