Don’t automatically fix the CVSS 10 first. Use CVSS to understand technical severity, EPSS to estimate near-term exploitation activity, KEV to identify vulnerabilities CISA records as exploited in the wild, and your application context to determine whether the vulnerable dependency is present, reachable and consequential. A KEV listing is an urgent signal even when EPSS is low; none of these signals alone tells you the full risk to your application.
What do CVSS, EPSS and KEV tell you?
They answer different questions, so they should inform one decision rather than be treated as interchangeable scores.

| Signal | What it answers | How to use it |
|---|---|---|
| CVSS severity and vector | How severe are the vulnerability’s technical characteristics under the scored assumptions? | Inspect the vector and metric groups. A Base score is not a measure of whether your application can reach the vulnerable code. |
| EPSS probability | How likely is exploitation activity for this CVE to be observed in the next 30 days? | Use the probability as an absolute estimate, not a severity rating or a prediction that your organization will be attacked. |
| EPSS percentile | How does this CVE’s probability rank against other currently scored vulnerabilities? | Use it for relative ordering, not as a probability. Check the score itself as well. |
| KEV status | Has CISA recorded the vulnerability as exploited in the wild? | Treat a listing as an urgent prioritization signal; a low EPSS score does not cancel confirmed exploitation. |
| Dependency presence and reachability | Is the affected package and version in the shipped system, and can the vulnerable behavior be invoked? | Validate against the dependency graph, deployed artifact and application behavior. |
| Consequence, controls and fix feasibility | What could exploitation mean for this service, and how practical is remediation? | Consider asset importance, compensating controls, fixed releases, compatibility and mitigation options. |
CVSS v4.0 has Base, Threat, Environmental and Supplemental metric groups. Base describes intrinsic vulnerability characteristics; Threat captures changing threat information; Environmental reflects the consumer’s environment; Supplemental metrics add context without changing the final score. Read the CVSS v4.0 specification and its user guide when interpreting a score. Even a contextualized CVSS score does not establish that a dependency’s vulnerable path is reachable in your application.

EPSS is a forward-looking estimate, not a severity score: its value runs from 0 to 1, and FIRST updates scores daily. A probability of 0.05, for example, means an estimated 5% likelihood of observing exploitation activity in the next 30 days—not a 5% chance your own system will be attacked. The percentile is a relative ranking, not another way to express that probability. See FIRST’s EPSS FAQ and EPSS usage guidance.
#1 Best Overall
KEV is different again: it records vulnerabilities CISA says have been exploited in the wild. CISA recommends using the KEV catalog as an input to vulnerability prioritization. FIRST distinguishes this evidence of exploitation from EPSS’s estimate of future exploitation activity and advises following KEV if the two appear to conflict.
How should you prioritize dependency vulnerabilities?
Use this triage sequence for each finding. It is a practical synthesis of FIRST, CISA and GitHub guidance, not an official universal scoring algorithm.
-
1. Verify the finding and the affected version
Confirm the CVE, package name and affected version against the lockfile and dependency graph. Determine whether the dependency is direct or transitive, then check the package or vendor advisory for fixed versions and any stated mitigations. Don’t plan an upgrade until you know which version is actually affected and what release addresses it.

2. Confirm what is shipped and reachable
Check whether the affected package and version are present in the built or deployed artifact—not merely listed in a development manifest. Investigate whether the vulnerable functionality can be reached in this application, and whether authentication, network boundaries or other compensating controls change exposure. Reachability is application-specific validation; CVSS, EPSS and KEV do not supply it.
-
3. Check KEV and elevate a match
Look up the CVE in CISA’s current catalog. If it is listed, treat confirmed exploitation as an urgent input to scheduling, even if EPSS is currently low. Check the live catalog during triage because membership can change.
-
4. Check current EPSS probability and percentile
Record the score and lookup date. Use the probability to compare estimated near-term exploitation likelihood; use percentile only to understand relative position among scored CVEs. Since EPSS scores are updated daily, refresh them during ongoing triage rather than treating a saved value as permanent.
-
5. Read the CVSS vector, not just the headline number
Review the score’s version and vector to understand the technical impact and exploit conditions represented by the metrics. A high Base score can describe a severe vulnerability without showing whether your particular dependency is present or reachable. Where Environmental or Threat metrics are available, interpret them in their stated context rather than assuming the Base score is a local risk assessment.

6. Weigh consequence, exposure and release feasibility
Consider what compromise could mean for the service, data and connected systems; how exposed the vulnerable code is; and what controls limit impact. Compare the available fixed release or mitigation with compatibility, testing, rollback and deployment constraints. Set prioritization thresholds according to your team’s capacity and service consequences—there is no universal EPSS cutoff or official CVSS–EPSS–KEV formula.
-
7. Remediate, mitigate or explicitly defer
Choose the safest practical fix or mitigation. If deferring, record the reason, owner and conditions that would trigger another review. After the change ships, verify the deployed dependency version and rescan or close the alert; changing a manifest alone does not prove the vulnerable version is gone from production.
Which vulnerability should you fix first?
When one finding is in KEV and the other only has a higher EPSS score
Start with the KEV-listed issue as the more urgent exploitation signal, including when its current EPSS probability is lower. Then validate whether the affected version is deployed and reachable, assess the service consequence, and identify a safe remediation or mitigation. If it is not present in a shipped artifact, document that finding and continue verifying rather than assuming the catalog entry applies to your service.
When a CVSS 10 competes with a high EPSS score
Neither number settles the comparison. CVSS describes technical severity under its scoring assumptions; EPSS estimates the likelihood of observing exploitation activity in the next 30 days. Check KEV, then compare actual presence and reachability, likely consequence, controls and fix feasibility for both findings. A severe but unreachable dependency may be less immediately exposed than a reachable vulnerability with strong exploitation signals, though the former still merits an evidence-based disposition.

Keep the signals separate in the triage record. A low EPSS value is not proof of safety, and a high percentile is not necessarily a high absolute probability. A KEV listing is evidence of exploitation rather than a forecast. Capture when EPSS was checked, refresh it while a finding remains open, and recheck KEV and vendor remediation guidance before acting.

How to make the process repeatable
For every finding, retain the CVE and affected package/version, whether it is direct or transitive, evidence that it is or is not in the shipped artifact, reachability assessment, KEV status, EPSS probability and lookup date, CVSS version/vector, service consequence, controls, chosen fix or mitigation, and any deferral rationale. This makes a priority decision reviewable and helps a new score or deployment change trigger the right reassessment.
For teams using GitHub, Dependabot alert guidance includes dependency relationship and organization-specific context, and GitHub announced EPSS scores in Dependabot alerts. See GitHub’s alert-prioritization guidance and its EPSS announcement. These features can support triage, but they do not replace checking what is deployed, reachable and consequential in your environment.
