CRISPR is now a clinical treatment, not only a laboratory technique. Casgevy (exagamglogene autotemcel) is a one-time, autologous stem-cell therapy edited with CRISPR/Cas9 for eligible people with sickle cell disease (SCD) and transfusion-dependent beta-thalassemia (TDT). The U.S. Food and Drug Administration (FDA) expanded its authorization to patients aged 2 years and older on July 1, 2026. In the European Union, the European Medicines Agency (EMA) product information describes use in patients aged 12 years and older. Those age limits apply to different jurisdictions and should not be treated as a worldwide rule.
Casgevy does not repair the DNA change that causes sickle hemoglobin. Instead, it edits a regulatory enhancer in the patient’s blood-forming stem cells so they produce more fetal hemoglobin. After the edited cells are returned following conditioning chemotherapy, they engraft in the bone marrow and can reduce the blood disorders’ defining complications. The approach is promising, but it is an intensive treatment with interim evidence, small study populations, long-term monitoring, and no guarantee of a permanent cure for every patient.

What moved CRISPR from the laboratory into clinical care?
Casgevy is the first FDA-approved therapy that uses CRISPR/Cas9 gene editing. It is also a living-cell product: clinicians collect a patient’s own blood-forming stem cells, send those cells through an editing and manufacturing process, and reinfuse them into the same patient. Because the cells are autologous, the treatment is made from the recipient’s cells rather than from a donor.

Casgevy is intended for patients who meet the applicable regulatory label and a specialist treatment center’s clinical requirements; an age threshold alone does not establish eligibility.

Who can receive Casgevy?
Authorization differs by jurisdiction. The following comparison keeps the dates and age limits attached to the regulator that issued them.
| Jurisdiction and status | Age described in the authorization | Diseases covered | What the difference means |
|---|---|---|---|
| United States — FDA expansion announced July 1, 2026 | 2 years and older | Eligible SCD and TDT patients under the U.S. label | The 2026 decision added access for younger children; it does not mean every two-year-old with either disease qualifies. |
| European Union — EMA product information | 12 years and older | SCD and TDT within the EU indication | The EU age description is not interchangeable with the newer U.S. threshold. |
Individual assessment still matters. Doctors must consider the diagnosis, disease severity, previous and current care, the ability to collect enough stem cells, fitness for conditioning chemotherapy, and whether the patient can be monitored through recovery. The public regulatory summaries do not establish one universal checklist for every country or treatment center.

Editing a control switch rather than the sickle mutation
CRISPR/Cas9 is programmed to cut DNA at a selected location. In Casgevy, the target is an enhancer involved in regulating the BCL11A gene in blood stem cells. Editing this enhancer reduces the relevant regulatory activity and allows developing red blood cells to make more fetal hemoglobin.

Fetal hemoglobin is naturally produced early in life and is less likely to form the damaging structures associated with sickle hemoglobin. Increasing it can make red cells function more normally. This is an indirect strategy: Casgevy does not rewrite the patient’s sickle-cell mutation into a normal adult hemoglobin sequence.
Why the patient’s own stem cells are returned
Blood-forming stem cells can repopulate the bone marrow. Casgevy edits cells collected from the patient, and those cells are then infused back after chemotherapy clears space in the marrow. If engraftment succeeds, the edited stem cells continue producing blood cells with increased fetal hemoglobin.
What does treatment involve?
Casgevy is not a routine injection or a same-day outpatient procedure. A typical pathway has several linked stages, although timing and details depend on the treatment center and the patient’s condition.
- Specialist evaluation. A hematology and cell-therapy team confirms that the disease and clinical circumstances fit the applicable indication, reviews previous treatment, and assesses whether the patient can undergo collection and conditioning.
- Stem-cell mobilization and collection. Medicines are used to move blood-forming stem cells into the bloodstream. The cells are collected by apheresis, a process in which blood circulates through a machine that separates the needed cells and returns the rest.
- Manufacturing and CRISPR editing. The collected cells are processed outside the body and edited at the BCL11A enhancer. Manufacturing and release testing must be completed before the cells can be returned.
- Conditioning chemotherapy. Before reinfusion, the patient receives chemotherapy to reduce existing marrow cells and create room for the edited cells. This is a major source of short-term treatment burden and infection and blood-count risks.
- Reinfusion. The edited autologous cells are infused through a vein, much like a stem-cell transplant infusion.
- Engraftment and monitoring. The cells must settle in the bone marrow and begin making blood cells. The clinical team follows blood counts, infections, bleeding, organ effects, and disease-related events during recovery.
- Long-term follow-up. Regulators require ongoing safety monitoring, including registry-based follow-up, because uncommon effects and durability may only become clear years after treatment.
The one-time infusion therefore sits inside a prolonged treatment journey. Patients and families should plan for repeated visits, periods of low blood counts, supportive care, and time away from school or work according to the center’s protocol.

What benefits have been shown in studies?
EMA’s public assessment summarizes interim results from two ongoing studies. Neither study compared Casgevy with another medicine or with placebo, and the participant groups were small. The outcomes are therefore important signals of benefit, not population-wide guarantees.
| Study population | Interim result reported by EMA | How to interpret it |
|---|---|---|
| Transfusion-dependent beta-thalassemia | 39 of 42 participants maintained hemoglobin above 9 g/dL without transfusions for at least 12 consecutive months. | This measures a transfusion-free period in the named study, not permanent transfusion independence for every person with TDT. |
| Sickle cell disease | 28 of 29 participants had no painful crisis for at least 12 consecutive months. | The result comes from an interim, single-arm study with ongoing follow-up. |
| Sickle cell disease | All 29 participants avoided hospitalization for painful crises for at least 12 consecutive months. | Avoiding hospitalization during the stated period does not establish that crises or admissions can never recur. |
The studies’ non-comparative design means they cannot by themselves show how Casgevy performs against every established treatment option. Follow-up is also still developing, so durability beyond the reported periods remains an important question.
What are the risks and burdens?
Immediate and treatment-related effects
EMA lists headache, nausea, and muscle and bone pain among common side effects. Conditioning chemotherapy can lower blood-cell counts, weaken defenses against infection, and require close observation and supportive treatment. Reduced platelets can increase bleeding risk; the recovery period is managed with laboratory monitoring and transfusion or other support when clinically necessary.

Potential genetic risks
Any genome-editing treatment raises concern about unintended DNA changes. EMA describes a theoretical risk that unintended genetic alterations could contribute to cancer. No such cancer cases had been observed at the time of its assessment, but that observation does not eliminate a theoretical or long-term risk. This is why extended follow-up and registry monitoring are part of the treatment program.

Practical burden
Patients must tolerate stem-cell collection, a manufacturing wait, chemotherapy, hospitalization or intensive outpatient monitoring, and a period of marrow recovery. Access is limited to specialized centers with the required cell-processing and transplant capabilities. The regulatory authorization does not imply that treatment is immediately available in every region.
Does CRISPR cure sickle cell disease?
It is more accurate to describe Casgevy as a disease-modifying, potentially curative-intent therapy than as a guaranteed permanent cure. The treatment can produce sustained fetal-hemoglobin production from engrafted cells, and the interim SCD results show substantial freedom from painful crises in the study group. However, the evidence comes from small, ongoing studies without a comparator, and long-term durability and uncommon risks are still being characterized.
A patient who is considering Casgevy should ask what outcome the treatment center expects in that individual’s case, what uncertainties remain, and what follow-up will be required. “Curative” should not be understood to mean that every symptom, organ complication, or future health risk disappears immediately after infusion.
How are U.S. and EU decisions different?
The FDA’s July 1, 2026 announcement expanded U.S. access to children as young as 2. Acting Director Karim Mikhail said the decision allows “pediatric patients as young as 2 years of age” to access an additional treatment option for these debilitating, life-threatening diseases. The EMA product information currently describes an EU indication beginning at age 12.

These decisions illustrate why a patient’s location matters. A family in the United States may encounter a different age threshold from a family in an EU member state, while local reimbursement, treatment-center capacity, and national implementation can add further differences. Regulatory approval is not the same as automatic coverage or universal availability.
What does newer safety guidance mean?
In April 2026, the FDA issued guidance recommending next-generation sequencing methods for nonclinical safety assessment of investigational human genome-editing products. The guidance reflects continuing regulatory attention to unintended edits and the quality of safety characterization as the field develops. It is not a clinical outcome and does not show that Casgevy has a specific off-target problem.
For patients, the practical implication is that genome-editing oversight is evolving. Future products may be evaluated with more detailed methods, and existing products remain subject to post-authorization monitoring and updated evidence.

No single option is right for everyone. A useful discussion with a specialist should compare the following dimensions rather than focus only on the word “CRISPR.”
| Decision factor | Questions to discuss |
|---|---|
| Eligibility | Does the patient’s diagnosis, age, disease history, and overall health fit the local label and center criteria? |
| Pathway and burden | Can the patient undergo mobilization, apheresis, chemotherapy, reinfusion, and a prolonged recovery period? |
| Expected benefit | Which outcome matters most: reducing transfusions, preventing painful crises, avoiding hospitalization, or another disease-specific goal? |
| Evidence maturity | How long have comparable patients been followed, and what remains unknown beyond the reported study period? |
| Long-term monitoring | What visits, blood tests, registries, and safety checks will be required after treatment? |
| Alternatives | What established care, clinical trials, or transplant approaches are appropriate, and how do their risks and evidence differ? |
The available evidence does not support ranking Casgevy above every existing option or recommending it to every person with SCD or TDT. The decision belongs with a qualified clinical team that knows the patient’s history.
Questions to take to a treatment center
- Which regulatory indication and age rule apply where I live?
- How many collection attempts are expected, and what happens if too few stem cells are obtained?
- What conditioning regimen, admission time, infection precautions, and blood-count support should we plan for?
- How long is the manufacturing process, and what contingency exists if the product cannot be released?
- Which benefits are realistic for my disease, and how will they be measured?
- What short-term side effects require urgent contact with the center?
- What long-term registry, cancer surveillance, and laboratory follow-up are required?
- How might treatment affect fertility, schooling, employment, travel, and family support?
What remains uncertain?
Casgevy’s arrival establishes that CRISPR editing can be delivered as an approved human therapy, but it does not settle every question about the technology. The pivotal results remain interim, the studies are small and non-comparative, and longer follow-up is needed to understand durability and rare adverse events. Access will also vary by country and by the number of centers equipped to deliver complex cell therapy.
Regulators continue to refine how unintended edits and other long-term risks are assessed. As labels, follow-up data, and national implementation change, patients should rely on current information from their regulator and treatment center rather than assume that one country’s authorization applies elsewhere.
