A lineage is a group of closely related viruses descended from a common ancestor. Strain is often used informally, but the CDC definitions used for SARS-CoV-2 do not establish it as a sharply separate category opposite “variant.”
In practice, the safest interpretation depends on context: a mutation is a genetic change, a variant describes a genome carrying changes, a lineage describes ancestry, and WHO labels such as variant under monitoring (VUM), variant of interest (VOI) and variant of concern (VOC) describe public-health assessment—not simply genetic difference.

The terminology at a glance
| Term | What it describes | How to interpret it |
|---|---|---|
| Mutation | One change in the virus’s genetic code | A genetic event; it may have no measurable effect on the virus |
| Variant | A viral genome containing one or more mutations | A virus with a particular genetic makeup, not automatically a more dangerous virus |
| Lineage | A group of closely related viruses sharing a common ancestor | An evolutionary branch, often named through the Pango system |
| Sublineage | A lineage descended from a parent lineage | A nested branch within a larger lineage |
| Strain | An informal term whose SARS-CoV-2 boundary is not fixed in the cited CDC definitions | Use only with a clear qualification or attribution |
| VUM, VOI and VOC | WHO public-health tracking categories | Assessment labels, not synonyms for every genetically different virus |
The CDC describes SARS-CoV-2 in several ways depending on the question being asked. A laboratory may report a sequence, an evolutionary analyst may assign a lineage, and a public-health agency may place a lineage or group of lineages in a monitoring category. Those labels can coexist because they answer different questions.
What a mutation means
A mutation is a single change in the virus’s genetic code. Viruses copy themselves repeatedly, and copying can introduce changes. A mutation can be detected in a sequence without changing how efficiently the virus spreads, how severe illness becomes, or how well a test, treatment or vaccine works.

The word therefore describes what changed in the genetic code, not what the change does. Some mutations are neutral in the circumstances studied. Others can alter a protein, affect how the virus enters cells or change a site recognized by antibodies. Establishing an effect requires laboratory, epidemiological or clinical evidence; the presence of a mutation alone is not proof of a meaningful biological change.
For that reason, “a new mutation was found” should not be rewritten as “a dangerous new strain emerged.” The first statement reports a genetic observation. The second makes a risk claim that needs separate evidence.
What a variant means
The CDC’s SARS-CoV-2 Variant Classifications and Definitions, updated September 1, 2023, states: “A variant is a viral genome (genetic code) that may contain one or more mutations.” This definition focuses on the sequence itself. It does not require that the virus be more transmissible, cause more severe disease or evade immunity.

Scientists use “variant” when they need to distinguish one genetic form of SARS-CoV-2 from another. A variant may carry one mutation or many. The term can be used before researchers know whether the differences affect transmission, disease, diagnostics, vaccines or treatments.
News coverage sometimes uses “variant” as shorthand for a virus that has become a public-health concern. That is a context-dependent shortcut, not the basic genetic definition. A report should identify any additional designation—such as a WHO VOI or VOC—rather than treating “variant” itself as a danger rating.
What a lineage and sublineage mean
Lineage is an ancestry term
A lineage is a group of closely related viruses with a common ancestor. It describes an evolutionary branch: sequences in the group share enough inherited genetic history to be treated as related. The Pango lineage system is commonly used to express this ancestry with hierarchical names.

Lineage does not by itself indicate severity or population risk. Two lineages can differ genetically while having similar public-health behavior, and a lineage that receives attention in one country may not have the same effect elsewhere.

Sublineages are nested descendants
A sublineage is a direct descendant considered in relation to its parent lineage. The word helps explain a family-tree relationship: a parent branch gives rise to a more specific descendant branch. A sublineage name therefore tells you about ancestry, not automatically about a new level of danger.
Because SARS-CoV-2 continues to diversify, surveillance systems may track parent lineages and several sublineages at once. A sublineage can become important if evidence shows expansion, a new wave, or a potential effect on public-health measures, but those conclusions come from assessment rather than the name alone.
Why “strain” is an unreliable opposite of “variant”
In everyday reporting, “strain” is often used to mean a recognizable form of a virus. Some writers use it for a lineage, others for a variant with notable biological properties, and still others use it simply as a general synonym for variant. The CDC definitions cited here explicitly define mutation, variant and lineage but do not establish one universal SARS-CoV-2 threshold that turns a variant into a separate “strain.”
That means there is no dependable rule such as “a few mutations make a variant, while a larger number make a strain.” Genetic distance, biological behavior and naming practice do not line up under one globally accepted cutoff. Calling something a strain without explaining the intended meaning can therefore create false precision.

Genetic names and public-health labels answer different questions
Pango: evolutionary ancestry
Pango names lineages in a hierarchical structure. Descendant names reflect their relationship to parent lineages, allowing analysts to describe increasingly specific branches as the virus evolves. A Pango label is therefore primarily an ancestry and sequence-classification identifier.
Nextclade and Nextstrain clades: genetic relatedness
Nextclade classifies sequences by genetic relatedness and groups related viruses into clades. WHO tracking tables can display a Pango lineage alongside a Nextstrain clade. These labels are useful for comparing sequence relationships, but they are not public-health risk grades.
WHO labels: surveillance and action
WHO can assign a public-health status to a lineage or group of lineages after assessing evidence about expansion or replacement, increased circulation, new waves and whether public-health actions might need adjustment. Its categories include VUM, VOI and VOC. A category can change as evidence accumulates.

Conversely, do not treat a VOC label as a replacement for the underlying lineage name: the category says how WHO assessed potential relevance, while the lineage identifies ancestry.
What VUM, VOI and VOC indicate
Variant under monitoring (VUM)
A VUM is tracked because available evidence warrants observation. Monitoring does not mean that the virus is confirmed to cause more severe disease or to evade immunity. It signals that surveillance and evidence review are appropriate.
Variant of interest (VOI)
A VOI has characteristics or early evidence that merit a more structured assessment. The designation is still an evaluation category, not a claim that every person infected will experience a particular outcome.

Variant of concern (VOC)
A VOC is assigned when evidence indicates a potentially important effect—such as changes relevant to transmission, disease severity, immune protection, diagnostics or treatments—and the public-health implications justify stronger attention. The classification is based on assessed evidence and can be revised.
These categories are not permanent properties of a virus. They reflect what health authorities know, where the evidence was collected and what actions may be needed at that time.
Why classifications can differ by country
CDC notes that U.S. and WHO classifications may differ because a variant’s impact can vary by location. A lineage that is expanding in one region may be rare in another. Prior immunity, vaccination coverage, demographic patterns, health-system conditions and the availability of genomic sequencing all affect how evidence is interpreted.
When reading a headline, check four details:
- Which organization made the designation? WHO and a national agency may use different frameworks.
- Which geography does it cover? A U.S. classification is not automatically a global classification.
- When was it assessed? A status from an earlier release may no longer describe current circulation.
- What evidence was considered? Genetic growth, laboratory findings and clinical outcomes answer different risk questions.
A dated example from WHO tracking
WHO’s variant-tracking page listed four variants under monitoring as of July 27, 2026: PQ.16.1.1, NB.1.8.1, XFG and BA.3.2. The page pairs several Pango lineage names with Nextstrain clades and records designation or risk-evaluation dates.
| WHO snapshot date | Variants under monitoring listed | What the list does—and does not—show |
|---|---|---|
| July 27, 2026 | PQ.16.1.1; NB.1.8.1; XFG; BA.3.2 | Shows a dated monitoring status; it is not a permanent list and does not provide a prevalence estimate |
This is a historical snapshot of the page’s status on that date. It should not be described as a list of the variants circulating “today” without checking a newer surveillance release. Nor does being listed under monitoring prove that one of these viruses causes more severe illness.
How WHO’s tracking framework changed
WHO said its updated working definitions and tracking system took effect on March 15, 2023. The revision was intended in part to evaluate Omicron sublineages independently, rather than treating every descendant as part of one undifferentiated group.
Best Value
WHO described the designations as reflecting assessed potential for expansion or replacement, new waves with increased circulation and possible need to adjust public-health actions. The framework therefore connects sequence surveillance with decisions about monitoring and response; it is not merely a catalog of genetic differences.
In a March 16, 2023 statement, WHO reported that Omicron viruses accounted for more than 98% of publicly available sequences since February 2022. That figure referred to sequence submissions available through the period of the statement. It was not a current estimate of infections, cases or prevalence, and it should not be reused as a present-day percentage.
How to read a COVID-19 variant report
Use this sequence when a report mixes terms:
- Find the genetic term. Is the report describing a mutation, a genome, a lineage or a sublineage?
- Separate ancestry from effect. A lineage name tells you where the virus fits in the family tree; it does not establish severity or immune escape.
- Look for an official category. If WHO or a national agency uses VUM, VOI or VOC, treat that as a separate assessment layer.
- Check the date and location. Surveillance status can change, and local impact may differ from global status.
- Read the evidence behind the claim. A laboratory result, growth advantage, hospitalization analysis and vaccine-effectiveness study measure different things.
- Do not infer risk from the word “strain.” Ask what the author means or substitute the precise term.
Why the distinction matters
For personal health decisions
Terminology alone cannot tell you how sick an individual will become or whether a particular test or treatment will work. Those questions depend on clinical guidance, current public-health information and the person’s medical circumstances. A genetic label is not a diagnosis and does not replace medical advice.
For interpreting vaccine and treatment news
A study may examine neutralization, vaccine effectiveness, antiviral activity or diagnostic performance. Those findings should be tied to the tested lineage or variant and the study conditions. It is an error to transfer a result from one genetic group to every virus described casually as a “strain.”
For understanding surveillance
Genomic surveillance detects and classifies sequences, while epidemiological surveillance tracks how infections behave in populations. Combining the two helps agencies determine whether a genetic change is associated with expansion, replacement or a possible change in public-health response.
For clear technical writing
Precise wording prevents a genetic observation from being mistaken for a risk judgment. A sentence such as “the laboratory identified the XFG lineage, which WHO listed under monitoring on July 27, 2026” keeps ancestry and public-health status separate. It does not claim that the lineage is more severe or more prevalent than the evidence shows.
Common mistakes and better wording
| Problematic wording | Why it misleads | More precise alternative |
|---|---|---|
| “Every variant is a new strain.” | There is no universal SARS-CoV-2 rule making those terms opposites. | “This variant is a genetically distinct genome; its lineage and public-health status are reported separately.” |
| “The mutation made the virus more dangerous.” | A mutation alone does not establish an effect on disease or transmission. | “The mutation was detected; studies are needed to determine whether it changes viral behavior.” |
| “VOC means the virus is always severe.” | VOC is a WHO assessment category, not a guarantee of an outcome for every infection. | “WHO classified it as a VOC because available evidence indicated important public-health implications.” |
| “The current variants are the four on the July 27 list.” | The list is dated and does not show current prevalence. | “WHO listed these four variants under monitoring as of July 27, 2026.” |
| “The Omicron percentage proves current circulation.” | The 2023 figure concerned submitted sequences during an earlier period. | “WHO reported a historical sequence-submission statistic in March 2023.” |

- If it is a descendant branch within that family, use sublineage.
- If it is an official assessment of surveillance relevance, name the organization and category, such as WHO VUM, VOI or VOC.
- If a source says strain, preserve the term only when quoting or attributing it, and explain the source’s intended meaning.
Bottom line
“Variant” and “strain” are not clean technical opposites for SARS-CoV-2. Variant is the defined genetic term for a viral genome that may contain one or more mutations; lineage and sublineage describe ancestry; and WHO categories describe public-health assessment. Because evidence and designations change by place and date, the most accurate explanation always names the specific genetic entity, the organization making any risk classification, and the date of that assessment.
