A supervised study at one Boston medical center suggests a conversational AI chatbot may help gather a patient’s history before a scheduled primary-care visit. It does not show that a chatbot can independently diagnose or treat patients, replace a clinician, or safely handle urgent-care cases without oversight.
What the study tested
The study evaluated AMIE (Articulate Medical Intelligence Explorer), a research conversational diagnostic AI developed by Google, as a pre-visit history-taking tool. Adults scheduled for an appointment at Beth Israel Deaconess Medical Center interacted with AMIE by text before meeting their primary-care physician. The system collected a clinical history and suggested possible diagnoses; a summary and transcript could be made available to the physician.

A physician supervisor monitored the interactions and could interrupt if a safety concern arose. The registry lists Beth Israel Deaconess Medical Center as the sponsor and Google LLC as a collaborator. Google Research describes the work as an initial real-world clinical feasibility step, not a test of autonomous care. Study registry; Google Research’s study account.

Google Research’s publication record says 100 adult patients completed AMIE text interactions. It reports that the correct diagnosis established through chart review appeared somewhere in AMIE’s differential diagnosis in 90% of cases, and that top-three diagnostic accuracy was 75%. These are different measures: the first counts a correct diagnosis anywhere in the list, while the second concerns whether it appeared among the three leading possibilities. Neither figure alone shows that the system would choose the right diagnosis first or produce the best care plan.

Customer support team responds within minutes if you have questions about contents.
In blinded evaluation, the record says AMIE’s differential diagnoses and management plans had overall quality and safety similar to those of primary-care physicians. Physicians scored higher on the practicality and cost-effectiveness of management plans. The results therefore point to potential value in collecting information and helping clinicians consider possibilities, while underscoring that a useful plan must also work in practice. These findings come from a single-center, supervised feasibility evaluation; they do not establish equivalent performance in routine or unsupervised care. Google Research publication record.
What the evidence does—and does not—establish
- It tested a specific workflow: adult patients used a text chatbot before a scheduled appointment at one academic medical center.
- Clinicians remained involved: a physician supervised the interaction, and the chatbot’s history, summary, and suggestions were intended to support a clinician’s visit.
- It was a feasibility study: the reported results do not prove improved health outcomes, reduced workload, or safety in general use.
- Its reach is limited: findings from this group and setting cannot automatically be extended to emergency departments, other patient populations, other health systems, or other chatbots.
- Diagnostic accuracy is not the whole job: having the final diagnosis somewhere in a list is not the same as identifying it as the most likely answer, and a diagnosis does not guarantee a practical, affordable management plan.
A New York Times report reproduced by KhanList adds that the trial lasted eight months, physicians found the chatbot helpful in about 75% of patient cases, and it may have influenced how clinicians handled more than half of cases. The same report describes one date error among hundreds of conversations and says supervisors provided additional context five times without stopping a session. These are secondary news-reported figures, not outcome definitions established by the publication record cited above, so they should be read with that qualification. How this relates to later AI-care research
Google later announced a separate nationwide randomized study with Included Health. That announcement describes a different project; it does not make the Beth Israel Deaconess feasibility study randomized or nationwide. Google Research announcement.
So, could an AI chatbot aid urgent care?
Possibly—as a supervised way to collect a patient’s story and prepare information for a clinician to review. This study offers an early feasibility signal for that narrow role. It is not evidence that patients should rely on a chatbot instead of seeking care, or that AI can independently make urgent-care decisions. Whether the workflow improves care or reduces clinicians’ work remains a separate question from whether it can be used in a real clinic.
