Trust an AI-assisted decision when you can show what the system is for, what could go wrong, how it was tested, who is accountable, and how a qualified person can challenge or reverse its output. A confident-looking recommendation is not evidence of safety.
The higher the stakes—health, safety, employment, credit, housing, essential services, privacy or fundamental rights—the stronger those controls must be. Use the checklist below before deployment, during operation and whenever a person asks how a decision was made.

What “trust” should mean in an AI-assisted decision
Trust is not the same as accuracy on a vendor demonstration. It is a defensible process that combines evidence, human agency and a remedy when the system is wrong. An AI may recommend an action, rank applicants, flag possible fraud or generate a risk score; a person or organization remains responsible for the consequences.
Start by separating three questions:
- Is the decision fit for purpose? The system addresses a clearly defined task and its errors matter in a way you understand.
- Is the process controlled? Data, model changes, security, privacy, human review and monitoring are documented.
- Can an affected person obtain a meaningful remedy? They receive an understandable explanation, a route to challenge the outcome and a recorded final decision.
“Human in the loop” is not automatically meaningful. If a reviewer merely clicks approve, lacks context or cannot override the model, the organization has human presence but little human oversight.

Use a risk-based checklist before you approve any system
1. Define the decision, purpose and stakes
Write down exactly what the AI recommends or decides, who is affected, who will act on the output and what a wrong result could cause. Include foreseeable misuse, not just the intended workflow. A model designed to prioritize support tickets, for example, might be repurposed to deny service; that different use requires a new assessment.
Classify the consequences. Health and safety, livelihood, access to essential services, privacy and fundamental rights are high-consequence areas. Consider whether harm is reversible: a mistaken low-priority label may delay help, while an incorrect denial of housing or employment can be difficult to repair. Higher stakes and less reversible harm require stricter evidence, more capable reviewers and stronger stop controls.
2. Assign a named, empowered owner
There must be a person or accountable team that can approve the system, pause it, authorize changes, investigate incidents and answer affected people. Put that responsibility in writing rather than assigning it vaguely to “the AI team” or a supplier.

The National Institute of Standards and Technology’s AI Risk Management Framework (AI RMF), released on January 26, 2023, is voluntary and intended to help organizations incorporate trustworthiness into the design, development, use and evaluation of AI systems. Its voluntary status does not remove your need to meet binding laws or sector rules; it gives you a structure for assigning and evidencing responsibility.

3. Map the complete lifecycle
Use NIST’s four functions as a backbone. The companion Playbook provides suggested actions, but NIST explicitly says it is neither a checklist nor a set of steps that must be followed in its entirety. Tailor the work to the decision’s risk.
| Function | Questions to answer |
|---|---|
| Govern | Who owns the system? What approval gates, change controls, incident procedures and stop mechanisms apply? |
| Map | What is the intended use, who is affected, what harms and misuse cases are foreseeable, and how autonomous is the system? |
| Measure | How were validity, reliability, safety, security, privacy, fairness, explainability and robustness evaluated? |
| Manage | Which risks are accepted, mitigated, transferred or avoided? What monitoring, escalation, rollback and shutdown actions follow from the measurements? |
Update the map when the data, model, user population, workflow or decision context changes. A system that was acceptable for one department or region may not be acceptable in another.
4. Examine data and model quality
Ask the provider and your own data team:
- What data trained, tuned and tested the model, and was its provenance documented?
- Which groups and conditions are represented, under-represented or absent?
- How were validity and reliability measured for the actual task rather than a convenient proxy?
- What are the error types and rates for relevant groups and operating conditions?
- Which privacy, access-control, retention and security safeguards protect the data and outputs?
- What are the known limitations, out-of-distribution conditions and cases in which the model should abstain?
Request evaluation results that match deployment. Aggregate performance can conceal materially different error patterns between groups. A vendor’s benchmark may also use a population, language or time period unlike yours. Preserve the limitations alongside the model version so users do not mistake an old test for a current guarantee.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problems5. Test before launch and continuously afterward
Require documented testing, evaluation, validation and verification appropriate to the use case. Before launch, define thresholds for unacceptable error, fairness problems, security failures and privacy incidents. Decide who can approve an exception and what evidence is required.

After launch, monitor for drift, new failure modes, subgroup performance, complaints, overrides and incidents. Set explicit rollback or shutdown triggers. Examples include a sustained error threshold, a newly observed harmful pattern, a security compromise or a data-source change that invalidates prior testing. A monitoring dashboard is useful only when someone has authority and a procedure to act on it.
6. Make human review genuinely effective
A reviewer needs appropriate knowledge, experience, authority and independence. They must see enough context to interpret the output, understand uncertainty and recognize when the case is outside the model’s competence. Give them practical controls to pause, override, reject or escalate a recommendation, with time and workload that make those actions realistic.
The UK Information Commissioner’s Office warns that automation bias and poor interpretability can make nominal review ineffective. Reduce that risk by showing relevant evidence rather than only a score, requiring a reason for accepting high-impact recommendations, sampling decisions for second review and training staff to treat the output as a fallible aid.
Independence matters when the reviewer’s performance target is tied to agreeing with the system. For high-consequence cases, consider a reviewer who is not the person who commissioned the model or who benefits from a particular outcome.

7. Explain the outcome in terms the person can use
An explanation should identify the decision process and the factors that materially affected the result, state important limitations and describe the next available action. A technical description of model architecture is not a useful explanation to someone whose application was rejected.
ICO guidance frames explainability around four principles: be transparent, be accountable, consider the context and reflect on impacts to individuals and wider society. Apply those principles to the recipient’s situation. Tell the person whether AI was used where notice is required, what information was considered, whether a human reviewed the case and how to request review or object where applicable.
8. Protect affected people with notice and a remedy
Provide a clear contact point, a review route, expected response times and a record of the final human decision. The route should not send a person back to the same automated channel that produced the disputed result. Accept relevant new information, correct inaccurate data and explain whether the original output was changed.

9. Check the law for the jurisdiction and use case
Requirements differ by country, sector and the role your organization plays. Do not treat a voluntary framework as a legal safe harbor. Consult the rules that apply to employment, credit, health, public services, privacy and consumer protection in the places where the system operates.
Article 14 of Regulation (EU) 2024/1689, published on July 12, 2024, requires high-risk AI systems in the European Union to be designed for effective human oversight. Oversight must be proportionate to risk, the system’s autonomy and its context. It must enable people to understand capabilities and limits, detect anomalies and unexpected performance, interpret outputs and avoid automation bias. If you serve people in the EU, determine whether your system falls within the regulation’s high-risk categories and meet the applicable obligations on the relevant timetable.
10. Keep an auditable evidence file
Retain the system purpose, data and model versions, evaluation and test results, monitoring logs, reviewer identity and rationale, incidents, overrides, complaints and approved changes. Record when a human disagreed with the model and what happened next.
Evidence should let an independent reviewer reconstruct the path from input to recommendation to final decision. If you cannot show how an outcome was produced, who challenged it or why it stood, trust has not been established—regardless of the model’s average accuracy.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Questions to ask a vendor before deployment
- What exact decision is the product designed to support, and what uses are prohibited or unsupported?
- Which data sources, populations and time periods support the claimed performance?
- What validation results exist for our language, geography, workflow and affected groups?
- How are uncertainty, abstention and out-of-distribution cases surfaced to reviewers?
- What security, privacy, retention, access and deletion controls apply to our inputs and outputs?
- How are model, data and configuration changes notified, tested and approved?
- What logs, explanations and exportable records will we receive?
- How can our staff override or stop the system, and how quickly can the vendor support rollback?
- What incident-notification, correction and complaint procedures exist?
- Which responsibilities remain ours, and which—if any—does the contract assign to the supplier?
Ask for evidence rather than assurances. A polished demonstration does not answer questions about subgroup errors, drift or a reviewer’s actual authority.
How to compare two AI-assisted decision systems
Compare systems against the same decision and operating conditions. Weight each dimension by the stakes, autonomy and context rather than awarding every category equal importance.
| Dimension | Evidence to compare |
|---|---|
| Validity and reliability | Task-specific evaluation, repeatability and performance on your population and data conditions. |
| Safety and security | Failure modes, abuse resistance, access controls, vulnerability handling and incident history. |
| Privacy | Collection, purpose limitation, retention, deletion and protection of sensitive information. |
| Fairness and bias | Representation, subgroup error analysis, mitigation and monitoring after deployment. |
| Explainability | Useful reasons, uncertainty information, limitations and records available to affected people. |
| Reviewer independence and power | Training, context, time, authority to override and freedom from automation pressure. |
| Monitoring | Drift detection, complaint and override review, thresholds and response ownership. |
| Documentation and response | Version records, change control, incident escalation, rollback and shutdown capability. |
| Reversibility of harm | How quickly and completely an incorrect decision can be corrected for the affected person. |
Warning signs that should stop or delay deployment
- The purpose is vague, expands without approval or relies on a proxy for a high-stakes decision.
- The supplier will not disclose relevant limitations, testing conditions or change history.
- Performance is reported only as an overall average, without relevant subgroup or error analysis.
- Reviewers cannot see supporting context, override the output or escalate unusual cases.
- There is no named owner, incident plan, rollback trigger or shutdown mechanism.
- Affected people cannot obtain a comprehensible explanation or reach an independent review channel.
- Logs do not preserve the model version, input context, reviewer action and final rationale.
A practical decision rule
Approve an AI-assisted decision system only when the purpose and stakes are explicit, accountable ownership is assigned, data and model quality are evidenced, testing and monitoring have action thresholds, human review is skilled and empowered, explanations and remedies are available, legal duties are addressed and records can support an audit. If any of those controls is missing in a high-consequence use, pause deployment or narrow the use until the gap is resolved.
