Yes—but only when the entire collection, drying, shipping, extraction and measurement workflow has been validated for capillary or dried blood. A lancet and collection card do not analyze blood by themselves. They create a specimen that a laboratory later extracts and measures with methods such as capillary electrophoresis, immunoassay, chromatography or mass spectrometry. Results are reliable only for the specific analyte, device and laboratory procedure that have been validated together.
What “one drop of blood” testing actually involves
A finger-prick sample is capillary blood, not venous plasma. The drop is collected onto a matrix and converted into a transportable microsample. Depending on the system, that matrix may be filter paper, a fixed-volume capillary device or a volumetric absorptive microsampling (VAMS) tip. After the sample dries, it is shipped or processed. In the laboratory, staff elute or extract the dried material and run an analytical method.

The collection device therefore feeds a laboratory workflow; it is not a miniature chemistry analyzer. The final application dictates the collection and storage approach. Materials, anticoagulants, preservatives or drying conditions that are acceptable for one assay can make another specimen unusable.
The five-stage workflow
- Capillary collection: A sterile lancet punctures the fingertip and a free-flowing drop is produced.
- Controlled application: The drop is placed on filter paper, a fixed-volume capillary or a VAMS tip.
- Drying: The specimen dries under the conditions specified by the laboratory or kit.
- Transport and extraction: The dried sample is packaged, mailed or transferred; the laboratory removes the analytes from the matrix.
- Measurement and interpretation: A validated instrument method reports concentrations that must be compared with reference intervals established for that specimen type.
The FDA-recognized CLSI NBS01 standard describes collection sites, equipment and technique for acceptable dried-blood-spot specimens used in newborn screening. A consumer collection card should not be assumed to meet that standard unless the laboratory says it does.

How accurate can a finger-prick result be?
Accuracy is method-specific rather than a property of “finger-prick testing” in general. Capillary and venous blood differ in composition, and hematocrit, spot homogeneity, drying, extraction recovery and matrix effects can all shift a result. A workflow that agrees closely with venous samples for one group of analytes may perform differently for another.
Evidence from validated workflows
| Evidence | What was measured | Reported performance | How to read it |
|---|---|---|---|
| Dvořák and Kubáň (2024) | Creatinine from hemaPEN dried spots using automated capillary electrophoresis | 2.74 µL of capillary blood per spot; calibration 25–500 µM; limit of detection 5.0 µM; more than 12 dried spots per hour; repeatability for peak areas ≤7.1% | These figures apply to the study’s device, extraction and electrophoresis conditions. |
| Crawford et al. (2020) | Five analytes in matched finger-stick and venous samples | Mean biases below 3% after intrinsic normalization and end-to-end verification | This supports that particular validated workflow, not every analyte or collection device. |
| PFAS microsampling comparison (2025) | 49 PFAS compounds under home-sampling and postage conditions | Mitra met the study’s recovery and precision criteria for 45 of 49; hemaPEN for 37 of 49; conventional dried-blood spots for 16 of 49 | Study-specific recovery results, not universal product claims. |
The 2026 International Association of Therapeutic Drug Monitoring and Clinical Toxicology guideline is more cautious about converting capillary-blood measurements into plasma concentrations. It calls for comprehensive clinical validation, case-by-case evaluation and an independent set of paired capillary and venous samples. A laboratory that cannot show this evidence should not present a finger-prick number as interchangeable with a venous-plasma result.
What a dried blood spot can measure
A dried spot can support quantitative testing when the analyte survives drying, the matrix is compatible with extraction and the laboratory has validated calibration and reference data for that specimen type. Examples in published work include creatinine and per- and polyfluoroalkyl substances (PFAS). Other tests may be feasible, but feasibility is not proof of clinical equivalence.

Questions to ask about any analyte
- Has the laboratory validated this exact analyte in capillary or dried blood rather than only in venous plasma or serum?
- Is the reported result a whole-blood concentration, a plasma-equivalent estimate or a qualitative finding?
- Were hematocrit effects, drying time, extraction recovery and shipping temperature studied?
- Are reference intervals and clinical decision limits established for this specimen type?
- What happens if the spot is too small, layered, contaminated or outside the allowed drying window?
Without clear answers, a numerical result may look precise while lacking a clinically interpretable reference framework.
Collection methods compared
| Method | Strength | Main limitation | Best comparison criteria |
|---|---|---|---|
| Conventional dried-blood-spot card | Low cost, simple storage and mailing | Spot size, hematocrit and uneven distribution can bias quantitation | Analyte validation, extraction procedure, hematocrit sensitivity and shipping stability |
| Mitra VAMS | Absorbs a fixed volume for remote collection | The US product is research-use-only and requires intended-use validation | Tip volume, assay compatibility, regulatory geography and laboratory workflow |
| hemaPEN | Four fixed-volume capillary samples from one source | Specialized device with territory-dependent status | Volume precision, replicate sampling, extraction and availability |
| HemaXis DB10 | Microfluidic volume control and protected dried spots | Certified-laboratory analysis and local validation remain necessary | Regulatory status, stability and laboratory compatibility |
| HemaSpot | Simplifies collection, storage and shipment | Not equivalent to every quantitative fixed-volume method | Sample capacity, analyte menu, IVD geography and shipping rules |
These are collection tools, not standalone home chemistry analyzers. A device can improve volume control or shipping convenience while still requiring a laboratory to validate the intended assay. Regulatory status is jurisdiction-specific; for example, the manufacturer states that Mitra use in the United States is research-only, and the HemaXis DB10 product statement identifies it as a CE/IVD and FDA Class 1 medical device. Those labels do not, by themselves, validate a particular clinical test.
How to collect and mail a usable sample
Follow the receiving laboratory’s instructions over generic advice. The laboratory’s assay determines the required volume, device, drying period and packaging.

- Confirm the test and specimen type. Ask whether the laboratory accepts capillary whole blood, a dried blood spot or VAMS, and whether the result is clinically validated for that specimen.
- Prepare the site. Wash and dry your hands, use the recommended finger and warm the hand if instructed. Do not apply lotions or disinfectant residue that could contaminate the sample.
- Use the supplied lancet. Puncture the side of the fingertip. If the instructions call for it, wipe away the first drop; avoid forceful squeezing, which can dilute blood with tissue fluid.
- Fill the device correctly. Let a free-flowing drop contact the marked area, capillary or absorptive tip. Do not touch the collection surface to skin, and do not layer multiple partially dried drops unless the protocol explicitly allows it.
- Check volume and saturation. Make sure every required circle or fixed-volume channel is filled. An underfilled or smeared specimen may be rejected or reported with a qualification.
- Dry completely. Place the sample in the orientation and environment specified by the laboratory. Keep it away from heat, direct sunlight, moisture and dirty surfaces; do not seal a damp card in plastic.
- Package and document. Use the supplied bag, desiccant and paperwork. Record collection date and time when requested, because stability limits can depend on elapsed time.
- Ship promptly. Follow the laboratory’s mailing schedule and temperature requirements. Postal conditions are part of the validated method, not an afterthought.
When a collection should be repeated
Contact the laboratory before sending a replacement if the blood did not soak through the marked area, the spot is visibly contaminated, the device was touched, the sample stayed wet, or the package was exposed to an unapproved temperature. Sending a questionable specimen without disclosure can produce a delayed rejection or a misleading number.
Why capillary and venous results can disagree
Biological differences
Capillary blood is a mixture influenced by arterial and venous circulation and local tissue exchange. Concentrations can therefore differ from a venous draw even before collection and drying are considered. The direction and size of the difference depend on the analyte and clinical state.
Hematocrit and spot distribution
Red-cell concentration affects how blood spreads through paper and how much plasma reaches an extraction punch. Two people with the same plasma concentration can yield different dried-spot signals if their hematocrits differ and the method does not correct for that effect.

Drying, extraction and matrix effects
Incomplete drying, humidity, temperature, adsorption to the collection material and incomplete elution can lower recovery. Conversely, contaminants or uneven distribution can raise a measured signal. A validated method characterizes these effects rather than assuming they cancel out.

If convenience and low cost matter most
A conventional card may be practical when the laboratory has validated the analyte on that card and provides clear instructions. It is less suitable when exact volume is critical or when hematocrit sensitivity has not been characterized.

1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsIf fixed volume and replicate testing matter
VAMS and multi-channel capillary devices can reduce volume variation and preserve replicate aliquots. Confirm the intended-use status in your country and verify that the receiving laboratory has an extraction method for that device.

If regulatory documentation is decisive
Ask for the device’s classification in your jurisdiction and the laboratory’s assay-specific validation summary. A CE/IVD or FDA device classification describes the device’s regulatory position; it does not automatically authorize every test performed with it.
If you need a result for medical decisions
Use a laboratory that can explain its paired capillary-versus-venous verification, reference intervals and reporting limitations. For a critical diagnosis, medication adjustment or unexpected result, a clinician may require confirmation with a conventional venous sample.
Interpreting a result safely
Read the specimen type, units, reference interval and any qualification on the report. Do not substitute a dried-blood whole-blood value for a plasma value simply because the numbers use similar units. Ask whether the reported interval was established for the same collection device and assay.
A result outside the expected range is not automatically an error, and a result inside the range is not proof that every health concern is excluded. Symptoms, timing, medications, hydration and clinical history still matter. Seek professional care for urgent symptoms rather than waiting for a mailed microsample result.
Bottom line: when one drop is enough
One finger-prick drop can support accurate blood-chemistry measurement when the analyte, collection device, drying and shipping conditions, extraction procedure and instrument method have all been validated together. Published studies show excellent performance for specific workflows—including creatinine with a 2.74-µL hemaPEN spot and five-analyte comparisons with mean bias below 3%—but those figures cannot be generalized to every test. Treat the collection device as the front end of a laboratory system, verify the assay’s capillary validation and use venous confirmation whenever the result will drive an important clinical decision.
