An eSIM is not a GPS tracker or a health sensor. It is a secure component that stores a carrier profile and authenticates your device to a mobile network. The carrier can still estimate where the device is from its connections to cell towers and identifiers linked to your subscription. Health measurements come from sensors in a phone, watch, wearable, or medical device and from the apps and cloud services that process them; the eSIM may only provide the cellular data connection.
What an eSIM actually does
An eSIM, or embedded SIM, is an eUICC secure element built into a device. Instead of inserting a removable plastic card, you download a SIM profile into that element. The profile contains credentials and network information used to authenticate the subscription. A carrier can provision, replace, or manage the profile remotely.

GSMA describes the security of an eSIM as equivalent to that of a removable SIM. Its 2024 device overview lists smartphones, smartwatches, wearables, GPS trackers, and healthcare devices among eSIM-capable categories. That list describes where an eSIM can be installed, not what the eSIM can sense. The secure element does not contain a GPS receiver, heart-rate sensor, blood-oxygen sensor, sleep sensor, glucose sensor, microphone, or camera.

The distinction matters because the same eSIM technology can appear in very different products. In a phone, it authenticates cellular service. In a GPS tracker, the tracker’s positioning hardware calculates coordinates and the eSIM sends them to a server. In a cardiac monitor, the medical device measures a signal and the eSIM can transmit the result. The sensing and analysis are performed by the surrounding hardware and software.

Can an eSIM track where you are?
The eSIM itself does not independently determine or transmit a GPS coordinate. A carrier can nevertheless infer the approximate location of an active device from the device’s interactions with the cellular network. When a phone or connected device registers, places a call, sends data, changes serving cells, or otherwise creates network traffic, the carrier can log those events and associate them with the subscription and handset.
What the carrier uses
The Federal Trade Commission identifies several persistent identifiers used in mobile monitoring and analytics:
| Identifier | What it represents | Privacy implication |
|---|---|---|
| IMEI | The device identity associated with the handset or modem | Can link network activity to a particular physical device |
| IMSI | The subscriber identity associated with the SIM profile | Connects activity to the mobile subscription |
| MSISDN | The telephone number associated with the subscription | Provides an account-level way to associate activity with a customer |
| TMSI | A temporary mobile-subscriber identifier | May rotate over periods ranging from hours to weeks, but rotation does not make the device invisible |
Cellular location is an estimate, not a promise of a single universal accuracy level. A carrier’s result depends on the network, the device, the surrounding radio environment, and how often the device communicates. Dense networks can provide more useful positioning than sparse ones, but no single precision or retention period applies to every carrier, country, or plan.
Can an eSIM see your GPS location?
No. GPS coordinates are calculated by a receiver in the phone, watch, tracker, or other device. The eSIM has no access to that calculation merely because it authenticates cellular service. If an app sends GPS coordinates to a cloud service over mobile data, the app, operating system, service provider, and possibly other recipients may receive those coordinates. That is a separate data flow from the eSIM profile.
Other ways a phone can reveal location
Applications can request precise location through the operating system’s location services. Depending on permissions and settings, location can also be inferred from Wi-Fi networks, Bluetooth beacons, or an IP address. These sources may be available to the phone operating system, an app, or an online service even when the carrier only has network-level information. Turning off an app’s location permission therefore does not erase the carrier’s ability to observe cellular network connections, and limiting carrier connectivity does not automatically stop an app that has another permitted location source.

Does an eSIM know your health data?
An eSIM alone cannot monitor heart rate, sleep, blood oxygen, blood pressure, glucose, mood, or any other physiological measurement. Those readings require sensors and software in a phone, smartwatch, fitness tracker, glucose monitor, or medical device. A companion app can store the readings, calculate trends, combine them with profile information, and share them with a cloud account or another service.
Where health measurements originate
- A watch or fitness tracker can collect signals such as pulse or movement using its own sensors.
- A phone can receive data from a paired wearable or accessory and display or analyze it.
- A glucose or blood-pressure device can measure a value and use a companion app to record it.
- A medical device can perform patient-specific analysis and send results to a clinician-facing system.
If one of these products has cellular service, the eSIM may carry the measurement to the app or server. Carrying the data is not the same as measuring it. A carrier may see that data was transmitted, along with network identifiers and traffic metadata, but the eSIM does not interpret a heart-rate waveform or sleep record.
When an app becomes the important party
The app or service that receives sensor output often determines what is retained, analyzed, or shared. The FTC’s mobile-health guidance covers apps that connect to fitness trackers, sleep monitors, blood-pressure monitors, watches, and other health-monitoring devices. Apps that control medical devices or perform patient-specific analysis can fall under Food and Drug Administration oversight. That regulatory question concerns the app or device function, not the presence of an eSIM.
Who can collect which information?
For a real setup, separate the collecting component from the communications component. The following comparison helps identify where to look for controls and policies.
| Component | Typical information it can collect | How location or health enters the record | Controls to review |
|---|---|---|---|
| Mobile carrier | Network events, account details, device and subscriber identifiers, and an inferred network location | Location is inferred from cellular connections; health data is not measured by the eSIM | Carrier privacy policy, account access, retention and sharing terms, and legal-request process |
| Phone operating system | Location services data, device identifiers, and information supplied by apps | Can calculate or broker precise location when permissions allow | Per-app location settings, background access, advertising-ID controls, and system privacy dashboard |
| Watch, tracker, or medical device | Sensor readings such as movement, pulse, sleep, blood pressure, or glucose | Hardware sensors create the measurements; cellular service may transmit them | Device permissions, pairing settings, firmware account, and local-data options |
| Health or fitness app | Measurements, profiles, notes, device information, and sometimes location | Combines data from sensors, the phone, and user input | Consent screens, background collection, connected services, export and deletion tools |
| Cloud service or data broker | Records received from apps and other sources, potentially linked by identifiers | Can combine health, device, location, and account data | Sharing partners, retention, access requests, deletion, and opt-out rights |
Exact recipients and retention are not universal. They vary by named carrier, device, app, plan, country, and jurisdiction. An eSIM plan’s policy is not automatically the policy for the phone operating system, a smartwatch account, a health app, or a broker.

What privacy laws cover eSIM location and health information?
Legal protection depends on who collected the information, how it was obtained, and where the person and company are located. HHS explains that mobile apps can collect network location, geolocation, device IDs, advertising IDs, and information typed or uploaded by users.

HIPAA is conditional
When an app operated by, or for, a regulated health-care provider links identifying information to a person’s health condition, the resulting information can be protected health information under HIPAA. HIPAA generally does not cover health information that a person voluntarily enters into a consumer app that was not developed by or for a regulated entity. That does not mean the information is unprotected: the FTC Act, the FTC’s Health Breach Notification Rule, state privacy laws, contracts, and other rules may apply.
The FTC Health Breach Notification Rule
The FTC states that many health apps outside HIPAA can still be subject to its Health Breach Notification Rule when they draw information from multiple sources, such as a connected fitness tracker and a user’s own entries. Whether a particular app is covered requires examining its data flows and business role; an eSIM does not decide the answer.
Why jurisdiction and policy details matter
Retention periods, sharing practices, deletion rights, and access controls differ by country and service. Read the carrier, device, operating-system, app, and cloud-account policies separately. Look for background collection, named recipients, data-sale or advertising language, account recovery, breach notices, and how to request deletion. A health app’s promise to delete records may not remove network logs held by a carrier, and deleting an app may not delete an account or copies already shared with another service.
Is an eSIM safer than a physical SIM for privacy?
For security of the SIM credentials, GSMA considers an eSIM equivalent to a removable SIM. That does not make it anonymous. A carrier can still associate network activity with the subscription and device identifiers, whether the credentials reside in an embedded secure element or on a removable card.

The practical difference is management: an eSIM profile is downloaded and managed electronically, while a physical SIM can be removed from the device. Neither format prevents a carrier from observing the device while it is connected. Neither format controls the location permissions granted to apps or the data practices of a wearable and its cloud service. Judge privacy by the complete setup rather than by the word eSIM.

-
List every connected component
Write down the carrier account, phone, eSIM profiles, paired watches or trackers, medical devices, operating-system account, health apps, and cloud services. Include devices that have their own cellular plan; they may create network records independently of the phone.
-
Ask what the carrier records
Read the carrier’s current privacy and data-retention terms. Identify references to network location, IMEI, IMSI, MSISDN, temporary identifiers, analytics, advertising, sharing, law-enforcement requests, and account access. If the policy is unclear, ask the carrier which network events and identifiers are retained and for how long in your jurisdiction.
-
Review operating-system permissions
Open the phone’s privacy or permissions settings and inspect every app with location access. Distinguish one-time or foreground access from continuous background access. Review nearby-device, Bluetooth, health-data, and motion permissions as well as location, because an app may combine several signals.

Check the wearable or medical-device account
In the device and companion app, inspect sensor permissions, automatic uploads, connected services, family or clinician sharing, and account recovery. Confirm whether measurements are stored locally, in the phone, in a vendor cloud, or in more than one place.
-
Trace each health-data recipient
Use the app’s connected-apps, export, and sharing screens to identify destinations. Remove integrations you do not need, disable background collection where practical, and use available deletion or access requests. Check whether a service keeps de-identified, aggregated, backup, or legally retained copies.
-
Separate cellular controls from app controls
Disabling an app’s location permission addresses that app, not carrier network records. Reducing cellular connectivity addresses the carrier path, not Wi-Fi, Bluetooth, or a previously synchronized cloud record. Apply the control to the component that actually collects the information.
-
Recheck after changes
Permissions and sharing can return after an operating-system update, device reset, or account migration. Review the privacy dashboards and connected-service lists periodically, especially after adding a new watch, tracker, or medical app.

Common scenarios
Phone with an eSIM and a fitness watch
The watch sensors create the health readings. The watch or phone app may upload them to a cloud account. The phone’s carrier can infer the phone’s network location from cellular events, while the watch’s own cellular plan, if it has one, can create a separate set of network records. The eSIM is the authentication and transport component in each device, not the source of the measurements.
GPS tracker with an eSIM
The tracker’s positioning hardware calculates coordinates. Its eSIM authenticates a data connection so the coordinates can be sent to a server. Calling the product an eSIM tracker is shorthand for a tracker that uses cellular connectivity; it does not mean the SIM profile performs the positioning.
Cellular medical monitor
A medical monitor can sense a patient variable, save it, and transmit it over a cellular link. The monitor manufacturer, app operator, clinician system, and carrier may therefore handle different portions of the data. Determine which entity receives the actual measurement and which sees only connection metadata.
Quick Recap
Key points to remember
- The eSIM is a secure network-authentication component, not a GPS or health sensor.
- Carriers can estimate location from cellular connections and identifiers tied to a subscription and device.
- Health measurements originate in sensors and apps; an eSIM may only carry the data.
- Apps and health services can combine health, device, and location information subject to permissions and their policies.
- HIPAA coverage depends on the regulated entity and data flow; consumer health apps may instead implicate FTC and state rules.
- Actual visibility, retention, sharing, and user controls vary by carrier, device, app, plan, and jurisdiction.
