Breast cancer recurrence-risk tests compared: AI tools, Oncotype DX and MammaPrint

Oncotype DX and MammaPrint are established genomic assays used for selected early-stage breast cancers; AI recurrence tools are newer and are not proven replacements. Oncotype DX measures 21 genes and reports a 0–100 Recurrence Score. MammaPrint measures 70 genes and reports a genomic-risk category. Both can add information to pathology and staging, but neither guarantees that cancer will or will not return. AI systems such as Ataraxis Breast use digitized pathology images and clinical data to estimate risk; current studies show promising validation results, not proof that AI-guided treatment improves survival.

The useful question is not “Which brand is best?” It is “Which test, if any, can change this patient’s treatment decision?” Eligibility depends on estrogen- and progesterone-receptor status, HER2 status, tumor size and grade, lymph-node involvement, menopausal status, age and baseline clinical risk. An oncology team must interpret any result alongside those factors, treatment tolerance and the patient’s preferences.

What these tests are designed to answer

After surgery for breast cancer, clinicians may know the tumor’s stage, grade and receptor biology but still face uncertainty about adjuvant chemotherapy. A recurrence assay can add a biologic risk estimate when the likely benefit of chemotherapy is not obvious from routine clinical information.

Prognostic versus predictive information

Prognostic information estimates the chance that cancer will recur over a defined period with the treatments represented in the evidence. Predictive information indicates whether the benefit of a treatment, such as chemotherapy, differs between result groups. A test can be prognostic without proving that changing treatment according to its score improves outcomes.

The American Cancer Society cautions: “These tests cannot tell for certain if a cancer will come back with or without chemotherapy.” A low-risk result lowers estimated risk in the population studied; it does not eliminate risk for an individual. Likewise, a high-risk result does not automatically make chemotherapy appropriate if the cancer’s biology, the patient’s health or the evidence for that subgroup points elsewhere.

At a glance: Oncotype DX, MammaPrint and AI tools

Option Inputs and biology Result Best-established use Important limits
Oncotype DX 21-gene assay from tumor tissue; commonly used for early-stage ER-positive, HER2-negative disease with zero to three positive nodes Recurrence Score from 0 to 100 Estimating prognosis and, in defined populations, chemotherapy benefit Evidence varies by menopausal status and nodal burden; not a guarantee and not validated for every breast-cancer subtype
MammaPrint 70-gene assay from tumor tissue; early-stage disease with zero to three positive nodes in the evidence base Genomic-risk category, generally low or high Helping resolve treatment uncertainty when clinical risk and genomic risk disagree ASCO and NICE recommendations restrict use in several groups; does not provide a universal chemotherapy rule
AI recurrence models Digitized H&E pathology slide plus clinical variables, depending on the product Model-generated calibrated recurrence-risk score or category Emerging risk stratification and research-supported decision support Products differ; retrospective or analytical validation is not evidence that AI-guided treatment improves survival or can replace a genomic assay

Oncotype DX: what the score means and where evidence is strongest

How it works

Oncotype DX analyzes expression of 21 genes in the tumor and returns a Recurrence Score from 0 to 100. The National Cancer Institute describes its use in early-stage, estrogen-receptor-positive, HER2-negative tumors with no more than three positive lymph nodes. The score is interpreted with age, menopausal status, tumor size and grade, and nodal findings rather than in isolation.

When guidelines support it

ASCO recommendations support Oncotype DX in defined node-negative settings and in selected postmenopausal patients with positive nodes. Evidence is insufficient for some patients with a higher node burden, and guidance is not identical for premenopausal patients. In younger or premenopausal patients, a low score may not carry the same chemotherapy-omission implication as it does in older or postmenopausal groups; ovarian-function effects and the biology of the disease must be discussed explicitly.

What a score can and cannot say

A score contributes both prognostic information and, in the populations studied, an estimate of chemotherapy benefit. It does not diagnose the cancer, replace pathology review or predict an individual outcome with certainty. A result near a clinical decision boundary should be discussed as a range of possible benefits, including absolute benefit, side effects and competing health risks, not as a rigid cutoff that applies to everyone.

MammaPrint: a different gene set and a different decision use

How it works

MammaPrint analyzes 70 genes and classifies a tumor into a genomic-risk category. NCI describes it for early-stage breast cancer with zero to three positive nodes and says it helps estimate whether cancer may return within five to 10 years. Unlike Oncotype DX’s 0–100 number, the primary report is a category, so the result should be combined with a formal clinical-risk assessment.

ASCO’s selective indications

ASCO says clinicians may use MammaPrint for patients older than 50 who have high clinical risk and are node-negative or have one to three positive nodes, when the result could clarify whether chemotherapy is worthwhile. ASCO recommends against using it to guide treatment in several lower-evidence groups, including patients age 50 or younger with high clinical risk and patients with four or more positive nodes.

NICE guidance in the NHS

Geography matters. NICE guidance for England’s NHS is more restrictive for node-negative, ER/PR-positive, HER2-negative disease: it says MammaPrint should not be used to guide adjuvant chemotherapy decisions in that setting. For an intermediate clinical-risk group, NICE allows Oncotype DX, EndoPredict or Prosigna with evidence generation. A recommendation from one health system therefore cannot be treated as a worldwide eligibility rule; local guidelines, payer policy and laboratory access control the practical choice.

AI recurrence-risk tools: what is available now

AI is a category, not one standardized test

“AI breast-cancer test” can refer to very different software products. The cited Ataraxis Breast RISK workflow extracts morphological features from a digitized hematoxylin-and-eosin (H&E) slide and combines them with clinical variables to generate a calibrated recurrence-risk score. Other products may use different images, features, endpoints or treatment assumptions. Ask for the exact product name, intended population, regulatory status and validation cohort before comparing an AI score with a genomic assay.

What the 2025 external validation showed

At the 2025 ASCO Annual Meeting, a validation abstract evaluated 3,502 patients across five external cohorts and reported a pooled concordance index (C-index) of 0.71. Among 858 patients who also had Oncotype DX results, Ataraxis reported a C-index of 0.67 versus 0.61 for Oncotype DX and added independent prognostic information. A C-index measures ranking ability—how often higher predicted risk corresponds to earlier events. It is not a percentage chance of recurrence and does not show that using the AI model to choose chemotherapy improves survival.

What the 2026 analytical validation adds

A 2026 analytical-validation paper reported intra-operator intraclass correlation (ICC) of 0.99 with 100% risk-category agreement, and inter-laboratory ICC of 0.97 with 94.7% agreement. A bridging study reported a C-index of 0.63 for the updated workflow versus 0.62 for the prior version. These results address reproducibility between operators and laboratories and consistency across a software version change. They do not establish universal clinical utility, prove benefit from AI-directed treatment or show that every AI model can replace Oncotype DX or MammaPrint.

Regulatory status is product-specific

FDA records include classifications and submissions for specific pathology-software devices, including ArteraAI Breast. That status belongs to the named device and its authorized claims. It should not be generalized to all recurrence models marketed as AI, nor does regulatory clearance by itself prove that a model improves patient survival.

Which test is “better” for a particular patient?

There is no universal winner because the assays measure different gene sets, report different outputs and were validated in different clinical groups. Use this sequence with the oncology team:

  1. Define the decision. Is the immediate question whether to add adjuvant chemotherapy, estimate baseline recurrence risk, or consider extended endocrine therapy? A test validated for one decision may not answer another.
  2. Confirm the clinical inputs. Review ER and PR results, HER2 status, tumor size, grade, lymph-node count, menopausal status, age and the clinical-risk category. Correcting a pathology or staging detail can change eligibility more than switching assays.
  3. Check the guideline population. Determine whether the patient fits the specific ASCO, NICE or other national recommendation. Pay particular attention to node number and menopausal status, where evidence differs.
  4. Ask what action each possible result would trigger. If chemotherapy is clearly indicated or clearly unnecessary from clinical factors alone, testing may add cost and delay without changing treatment. A test has the greatest value when a result could move the plan in either direction.
  5. Separate risk prediction from treatment-benefit evidence. A prognostic AI score should not be presented as proof that chemotherapy will or will not help. For Oncotype DX, ask whether the chemotherapy-benefit evidence applies to this age and node subgroup. For MammaPrint, ask whether the high- or low-risk category is being used within the population studied.
  6. Check access and coverage. Laboratory availability, payer rules, specimen requirements and turnaround time vary by country and institution. A test that is guideline-supported in one jurisdiction may be unavailable or not reimbursed in another.
  7. Make a shared decision. Discuss the absolute expected benefit, chemotherapy toxicities, endocrine-treatment plan, other illnesses, fertility or ovarian-function considerations, and the patient’s tolerance for uncertainty.

Why a low result does not automatically mean “no chemotherapy”

A low genomic-risk category or low Recurrence Score means the patient resembles a lower-risk group in the assay’s validation studies. It does not account perfectly for every tumor feature, treatment adherence issue or competing medical condition. The meaning can also change with age, menopausal status and lymph-node involvement. In some premenopausal settings, chemotherapy may provide benefit through ovarian suppression or mechanisms not captured by a simple low-risk interpretation.

Conversely, a high result does not compel chemotherapy when the patient cannot safely receive it, when the evidence does not cover that subgroup, or when the expected absolute benefit is very small. The oncologist should explain the estimate as an absolute difference—how many people like the patient benefited in the relevant study—not merely label the result “high” or “low.”

Practical limitations, pitfalls and questions to ask

Common interpretation errors

  • Comparing unlike outputs: a 0–100 Oncotype score, a MammaPrint category and an AI C-index are not interchangeable scales.
  • Ignoring clinical risk: genomic information supplements, rather than erases, tumor size, grade, nodes and receptor status.
  • Assuming validation equals utility: retrospective discrimination and analytical reproducibility do not prove improved survival from test-guided treatment.
  • Generalizing across products: “AI” does not identify one algorithm, specimen type, endpoint or regulatory claim.
  • Using an outdated guideline: recommendations can differ by country and change as prospective evidence accumulates.

Questions for the appointment

  • What treatment decision is uncertain, and would this test realistically change it?
  • Does my age, menopausal status, node count and receptor profile match the population in which the test was studied?
  • Is the result prognostic, predictive of chemotherapy benefit, or both?
  • What is the estimated absolute benefit of chemotherapy for me, and what assumptions produce that estimate?
  • Which laboratory or software version will process the specimen, and what are the expected turnaround and coverage rules?
  • If the result conflicts with pathology or the clinical picture, how will the multidisciplinary team resolve the discrepancy?

Bottom line for choosing among the options

Oncotype DX has the clearest chemotherapy-benefit evidence in selected hormone-receptor-positive, HER2-negative early breast cancers, especially the node-negative and postmenopausal groups covered by guidelines. MammaPrint can be useful selectively when clinical risk is high and treatment benefit remains uncertain, but ASCO and NICE limit its use in several populations. AI recurrence tools are promising decision-support technologies with encouraging external and analytical validation, yet no cited evidence shows that AI-guided treatment improves survival or that an AI model should replace either genomic assay. The safest interpretation of any result is a joint clinical decision that integrates the assay, pathology, stage, guideline population, treatment tolerance and the patient’s values.
