For most teams, start with a readiness toolkit that combines an anonymous questionnaire, scoring, interpretation and an action plan. The Leaders Drive Change Change Readiness Assessment & Action Toolkit is built for that job. Choose the 12-item Organizational Readiness for Implementing Change (ORIC) when you need a short, research-based measure across roles. Use Prosci ADKAR to find the individual adoption stage where a person or role is stuck, but pair it with team questions about leadership, trust, resources and psychological safety.
Readiness is not a single attitude. A low score can indicate unclear goals, low confidence, poor manager support, competing workload, missing skills or a climate in which people do not feel safe raising concerns. The best assessment therefore measures both willingness and the conditions that make adoption possible.

What a change-readiness assessment should reveal
A useful assessment answers five practical questions:
PC Slower Than It Used to Be?
A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Outdated Drivers Are Slowing You Down
One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware match- Do people understand the problem, the desired future state and why the change is necessary?
- How will the change affect each role, team and customer or patient outcome?
- Do leaders and managers show visible, credible support?
- Do people have enough time, staffing, systems, training and authority to work differently?
- Where is adoption likely to stall, and what action should happen before rollout?
Resistance is often a signal about one of these conditions rather than a fixed personality trait. Someone may oppose a new process because the goal is ambiguous, because previous initiatives were abandoned, because the team is already overloaded or because admitting uncertainty feels risky. A survey that only asks whether employees support the change will identify dissatisfaction but not its cause.

Shortlist: which tool fits your situation?
| Tool or approach | Best use | Level measured | Evidence and format | What it shows |
|---|---|---|---|---|
| Leaders Drive Change Change Readiness Assessment & Action Toolkit | A practical baseline and action plan for a department, leadership team or organization | Team, department or organization | Paid digital download with a PDF assessment, Sheets/Excel scoring model, insight and theme guide, and action-planning template | Strengths, pockets of resistance, existing change load and follow-up priorities |
| ORIC | A concise research-based implementation measure before rollout | Multiple organizational roles | 12 items rated on a 1-to-5 agreement scale; PhenX recommends collecting responses from several roles | A defined organizational-readiness construct and differences in barriers or facilitators across roles |
| ADKAR plus a team survey | Diagnosing why particular people or roles are not adopting | Individual adoption, supplemented by team conditions | Five observable elements: Awareness, Desire, Knowledge, Ability and Reinforcement | The adoption stage at which support is needed; the companion survey covers climate and capacity |
| National Collaborating Centre for Methods and Tools templates | Facilitated assessment when you want adaptable institutional questions | Team or organization | Change Assessment Instrument, Change Readiness Survey, and a survey covering goals, anticipated impact, planning and implementation | Perceptions of the change and implementation barriers that can be explored in discussion |
| Government of Zambia readiness tool | Checking organizational foundations and conflicts between groups | Organization, department, team or culture | Readiness defined across strategy, methodology, capability and capacity | Conflicts among personnel, teams, departments or organizational cultures |
| California OCM readiness resources | Running role-specific assessments alongside sponsorship and team-dynamics work | Executives, managers, employees and project teams | Readiness template plus sponsorship and team-dynamics resources | Category-level preparedness and areas needing focused effort |
The institutional templates are useful neutral references, while the commercial toolkit supplies more interpretation and action-planning structure. ORIC favors brevity and a defined implementation construct. ADKAR is strongest for an individual bottleneck. None should be treated as a universal pass/fail test.
Leaders Drive Change toolkit: best when you need a decision-ready package
The Change Readiness Assessment & Action Toolkit is designed for leadership teams, departments and whole organizations. It combines four components:
- A PDF questionnaire for gathering perceptions.
- A Sheets/Excel scoring model for organizing responses.
- An insight and theme guide for interpreting patterns.
- An action-planning template for assigning responses to the findings.
Its distinctive focus is breadth. It asks where strengths exist, where pockets of resistance are concentrated and how much change load people already carry. That last dimension matters when a team appears supportive but has several concurrent initiatives, vacancies or operational deadlines. A high willingness score does not create capacity by itself.
How to use it
- Define the change, the population to be assessed and the decisions the results must inform.
- Distribute the assessment anonymously where possible, using a response window long enough for every shift or location to participate.
- Enter responses in the scoring model and examine themes by role, team or location only when the group is large enough to protect identity.
- Use the guide to distinguish a broad strength from a localized resistance pocket.
- Convert the lowest or most consequential themes into an action plan with an owner, deadline and follow-up measure.
Because it is a paid download, verify the current price, licensing limits and program terms before purchase. Treat its scoring outputs as decision support, not as a validated clinical or psychometric diagnosis.
ORIC: the concise research-based option
PhenX describes Organizational Readiness for Implementing Change as a 12-item instrument. Respondents rate agreement from 1 to 5. It is intended for pre-implementation studies and is most informative when responses come from multiple roles rather than only project sponsors.

ORIC is useful when survey fatigue is a serious risk or when you need a consistent implementation-readiness measure across several sites. Compare distributions by role or unit, not only an overall average: executives, supervisors and frontline staff can experience the same change very differently.
What ORIC does not do
Its brevity means it cannot explain every reason for a low result. It does not, by itself, map psychological safety, manager behavior, staffing, training adequacy or the details of a particular workflow. Add a small set of context questions and an open-text prompt, then use interviews or workshops to investigate the barriers that the 12 items surface.
The broader evidence base supports that approach. A 2020 systematic review in Implementation Science mapped 1,370 organizational-readiness survey items to the CFIR framework; 897 items, or 68%, mapped to the inner-setting domain. The most common constructs were readiness for implementation, networks and communication, implementation climate, structural characteristics and culture. The review also cautioned that many items are specific to the change context and should be tailored rather than copied as a universal questionnaire.
ADKAR: locate the individual adoption bottleneck
Prosci describes ADKAR as five observable elements:
- Awareness: the person understands why the change is needed.
- Desire: the person is willing to support and participate.
- Knowledge: the person knows what to do and how to do it.
- Ability: the person can apply the new skills or process in real work.
- Reinforcement: the new behavior is supported so it continues.
Use the sequence diagnostically. Someone who understands the case for change but lacks desire needs a different conversation from someone who is willing but has not received training. A person who completed training but cannot perform the workflow may need practice time, system fixes or manager coaching rather than another presentation.

Institutional and government templates
National Collaborating Centre for Methods and Tools
The centre lists a Change Assessment Instrument, a Change Readiness Survey and a survey covering opinions about change goals, anticipated impact, planning and implementation. These are useful when a facilitator wants a starting set of questions that can be adapted to a specific service, policy or workflow. They also support a conversation about what respondents believe will happen, not just whether they approve.
Government of Zambia readiness tool
This tool defines readiness across strategy, methodology, capability and capacity. That structure is helpful for diagnosing a disagreement that looks interpersonal but is actually structural: teams may support the goal while disagreeing about the method, lacking capability or having no capacity to absorb the work. It also says the assessment can locate conflicts among personnel, teams, departments or organizational cultures.
California OCM resources
California’s organizational-change-management framework provides a readiness template for executives, managers, employees and project teams, together with sponsorship and team-dynamics resources. Use the role-specific versions when expectations differ by level. An executive may be prepared to fund a change while a manager is not prepared to coach it and an employee is not prepared to use the new system.

Comparison by decision criteria
| Criterion | What to look for | Best fit among the options |
|---|---|---|
| Evidence and validation | A defined construct, documented development or a framework that explains the items | ORIC for a concise implementation construct; the systematic review for context-sensitive design; institutional frameworks for facilitation |
| Level measured | Whether the instrument addresses an individual, team, department or organization | ADKAR for individuals; ORIC and the toolkit for organizational roles; templates for team and organizational workshops |
| Resistance visibility | Ability to locate a role, unit or stage where adoption is stalling | ADKAR for stages; the toolkit for pockets of resistance; segmented ORIC or custom surveys for role differences |
| Item length | Burden on respondents and ability to repeat the measure | ORIC is 12 items; a custom pulse can be 10 to 15 items; broader toolkits and templates require more interpretation |
| Scoring and reporting | Whether calculations and interpretation are supplied | The toolkit includes a Sheets/Excel model and theme guide; ORIC supplies a fixed response scale; templates depend on the facilitator’s scoring design |
| Customization | How easily questions can reflect the specific change and local workflow | Institutional templates and a custom pulse are easiest to tailor; the review cautions that context-specific items are often necessary |
| Action planning | Whether findings translate into owners and interventions | The toolkit includes an action-planning template; other options require the team to create one |
| Repeat measurement | Whether the same items can be used before rollout and after early implementation | ORIC or a short custom pulse supports a repeat baseline; use identical core items and add stage-specific questions |
A practical 15-item pulse survey
The following is a custom, reader-language pulse rather than a validated replacement for ORIC. Use a 1-to-5 scale such as 1 = strongly disagree, 2 = disagree, 3 = neither agree nor disagree, 4 = agree and 5 = strongly agree. Keep the wording tied to one defined change.
Awareness and impact
- I understand the problem this change is intended to solve.
- I understand what the desired future state will look like for our team.
- I understand how this change will affect my day-to-day work.
Desire, leadership and trust
- I believe this change will improve an outcome that matters to our team or customers.
- Senior leaders have communicated a clear and credible reason for the change.
- My manager supports this change and is prepared to help us through it.
- People on this team can raise concerns about the change without negative consequences.
- Peer norms on this team will encourage the new way of working.
Knowledge and ability
- I know what I will need to do differently.
- I have access to the training, guidance or practice needed to do it.
- I am confident that I can perform the new process or use the new system.
Capacity and reinforcement
- Our team has enough time and staffing to adopt this change.
- We have the tools, systems and data needed to support the new process.
- Competing initiatives will not prevent us from implementing this change.
- There is a clear plan to reinforce the new behavior after launch.
Finish with two optional open questions: What is the biggest barrier to adopting this change? and What question has not been answered? Do not present the pulse as a psychometric instrument. Use it to identify themes, then validate important findings through listening sessions, workflow observation or manager interviews.

How to protect anonymity while finding resistance pockets
Segmentation is valuable only when people trust the process. Publish the purpose, sponsor, response window, scale and reporting rules before sending the survey.
- Use an anonymous link or a trusted third party when feasible; do not collect names merely because a platform allows it.
- Set a minimum reporting group size and combine small teams, locations or demographic categories that could identify a respondent.
- Report themes and score ranges rather than verbatim comments that reveal an individual.
- Separate demographic questions from the response record when practical.
- Tell managers that results are for removing barriers, not ranking or disciplining employees.
- Share the findings and the actions that will follow; silence after a survey quickly reduces trust in the next one.
Compare roles or teams only when the same questions, time window and scale were used. A difference can reflect workload, exposure to the change or local leadership rather than a willingness problem.
Scoring and interpreting results
For a custom pulse, calculate the average for each dimension and display the response distribution. An average can hide a split team: ten scores of 5 and ten scores of 1 equal an average of 3, but require a different intervention from twenty scores of 3. Review the percentage selecting 1 or 2, the percentage selecting 4 or 5 and the open-text themes.
Use ORIC’s 1-to-5 responses consistently if you repeat it, and document whether you report item-level results, a mean or another summary. Do not invent a universal readiness cutoff. The California guide’s 4.0 category rule is a local interpretation, while other instruments may use different conventions.
Prioritize barriers using three tests: how low the score is, how many people are affected and how directly the barrier threatens safe adoption. A modest score on training may be less urgent than a very low score on psychological safety if people will not report defects after launch.

Pick the symptom - the matching free tool is one click away.
Turn findings into an action loop
- Set the baseline. Run the assessment early enough to change the plan, ideally before finalizing training, staffing and launch timing.
- Share themes promptly. Explain what was heard, what cannot be concluded and how anonymity was protected.
- Choose owners. Assign each priority barrier to a leader who controls the relevant lever: communications, workflow, staffing, systems, training or sponsorship.
- Match the intervention to the diagnosis. Clarify the case for change when Awareness is low; involve people in design when Desire is low; provide demonstrations and job aids when Knowledge is low; create practice time or fix the system when Ability is low; use coaching, metrics and recognition for Reinforcement.
- Address team conditions. Resolve conflicting priorities, publish decision rights, make manager expectations explicit and create a safe route for escalation.
- Sequence the rollout. AHRQ states that readiness assessment should be a team activity because team buy-in is key to implementation success. It also recommends using culture surveys to identify strengths and weaknesses and considering incremental implementation in units that appear more ready.
- Repeat the pulse. Reuse core items before rollout and after early implementation, then add questions about actual use, defects, workload and support. Look for movement in the barrier, not just a higher overall score.
Common mistakes and how to correct them
Treating resistance as a personality problem
Replace labels such as difficult or negative with a specific hypothesis: unclear outcome, low trust, insufficient capacity, missing skill or conflicting incentive. Test the hypothesis with a targeted question and a conversation.
Best Value
Surveying only senior leaders
Readiness is distributed. Include managers and people who perform the work, then compare perspectives without exposing small-group identities.
Using ADKAR as the entire organizational assessment
ADKAR can locate an individual adoption stage, but it cannot measure communication networks, implementation climate, staffing or psychological safety. Add team-level items.
Launching a survey without an action owner
Before collecting responses, decide who will receive the themes, who can change each barrier and when progress will be reviewed. Otherwise the survey becomes another uncompleted initiative.
Assuming a high average means readiness
Check the spread, subgroup differences and change load. A team may be enthusiastic but unable to absorb more work, or may report agreement because it does not feel safe disagreeing.
Copying a generic instrument unchanged
The systematic review found that many readiness items are specific to the change context. Preserve the construct you need to measure, but adapt examples, terminology and workflow references so respondents can answer from real experience.
