Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →For most students carrying school supplies, a well-fitted backpack with two shoulder straps is the more practical default: it can distribute the load across both shoulders, and pediatric guidance recommends wearing both straps. A messenger bag can also work for a student who prefers it, but it should be worn across the body. Fit, load, and the student’s route and habits matter more than the bag’s label; the available guidance does not establish that one style is safer or more comfortable for every student.
How backpacks and messenger bags compare for school
| Factor | Backpack | Messenger bag |
|---|---|---|
| How the load is carried | Two shoulder straps let the student carry the bag on both shoulders. Pediatric guidance recommends using both straps evenly. | Typically carried on one shoulder or across the body. Johns Hopkins Medicine recommends wearing a messenger-style bag across the body rather than on one shoulder. |
| Fit and adjustment | Choose a bag proportional to the student, with broad adjustable straps and a padded back; a waist or chest belt may also be considered. | Ensure the strap and bag sit comfortably when worn across the body. The cited guidance does not give a comparative fitting standard for messenger bags. |
| Packing and weight | Carry only what is needed, put heavier items closest to the back, and clear out unnecessary contents regularly. | The same practical rule applies: avoid carrying unnecessary weight. No reviewed source establishes a messenger-bag-specific weight limit. |
| Comfort and injury evidence | Guidance supports fit and carrying practices, but does not prove that backpacks prevent injury for every student. | There is no established head-to-head evidence here showing that messenger bags cause more pain or injuries than backpacks. |
| Access and school route | Consider how often the student needs to open the bag, and whether its size is manageable in crowded halls or on stairs. | Consider whether the student can access contents easily without repeatedly shifting the bag, and whether carrying it crossbody works on the daily route. |
The practical distinction is how each bag is supported: a backpack can be carried on both shoulders, while a messenger bag’s single strap calls for a crossbody position. Neither label alone tells you whether a particular bag is the right size, comfortable over a full school day, or suitable for a student’s routine.

When a backpack is the better fit
A backpack is a sensible starting point when the student carries several books, a laptop, or other supplies and can use both shoulder straps. The American Academy of Pediatrics (AAP) recommends a backpack sized in proportion to the child, with two broad, adjustable shoulder straps and a padded back. It also suggests compartments and consideration of a waist or chest belt. AAP backpack safety guidance

- Pack heavier items nearest the back.
- Carry only what is needed that day and clean out the bag regularly.
The AAP says a child’s backpack should not weigh more than 15% of their body weight. Treat that as the AAP’s recommendation, not a universally established cutoff: recommendations can differ, and the percentage is not a direct backpack-versus-messenger comparison. If the bag feels unmanageable or causes discomfort, reduce the load and reassess the fit rather than relying on a number alone.

When a messenger bag can work
A messenger bag may suit a student who prefers its style or access to contents. Johns Hopkins Medicine advises students who choose a messenger-style bag to wear it across the body instead of on one shoulder. Its broader advice is to use both backpack straps when carrying a backpack, remove unnecessary contents, and seek medical advice if a child has pain, numbness, or tingling. Johns Hopkins Medicine’s carrying advice
The crossbody recommendation is practical guidance, not proof that messenger bags are either harmful or equivalent to backpacks in every situation. The reviewed material does not establish comparative comfort or injury rates for all students and bag types. Pay attention to how the student actually wears the bag and whether it remains comfortable through the school day.

How to choose for the student’s day
- Start with the real load. List what the student carries on a typical day, then remove anything not needed. A lighter bag is easier to manage regardless of style.
- Check fit while loaded. A backpack should be proportional to the student and sit close to the body with both straps used. For a messenger bag, check comfort with the strap worn across the body.
- Think through the route. Consider stairs, crowded corridors, distance between classes, and whether the student needs to retrieve items frequently. These practical factors may make one style more convenient than another.
- Ask the student about comfort after a full day. A bag that seems manageable when empty may feel different when loaded and carried between classes. Adjust the load or fit if discomfort develops.
- Consider a rolling bag only if it suits the school. The AAP says a rolling backpack may be considered when the load is too heavy and the school allows it. Check school rules and whether stairs or crowded hallways make wheels impractical. The AAOS also discusses backpack safety in its August 24, 2026 back-to-school guidance.
What injury figures do—and do not—show
The US Consumer Product Safety Commission estimated an annual average of 1,200 backpack-related emergency-room injuries among children under 19 from 2019 through 2021. That is a broad estimate about backpack-related injuries, not a comparison with messenger bags and not a measure of the risk for an individual student. CPSC’s 2022 backpack safety release
An AAP Grand Rounds commentary published in 2008 describes a small study involving ten young teens. In the tested backpack conditions, the commentary reports higher perceived pain at loads of 30% of body weight than at 10% or 20%, and higher pain with one strap than with two. The small sample and backpack-only setup do not establish a messenger-bag comparison or a universal effect. AAP Grand Rounds commentary

AAOS quoted Sumeet Garg, MD, FAAOS, a pediatric orthopaedic and spine surgeon at Children’s Hospital Colorado, in an August 24, 2026 release: “A backpack may seem harmless, but when it is too heavy, poorly fitted or carried incorrectly day after day, it can put unnecessary strain on a child’s back, neck and shoulders,” AAOS release. The practical takeaway is to focus on fit and carrying habits; that statement does not compare backpacks with messenger bags.
