Gas and other mild digestive symptoms are the effects reported most often. Rare bloodstream or invasive infections have occurred, mainly in people who are severely ill or immunocompromised. Premature infants are a special high-risk group: the U.S. Food and Drug Administration (FDA) warns that probiotic organisms have caused potentially fatal invasive disease in preterm and very-low-birth-weight infants.
The exact microorganism, dose, purpose, age, and care setting all matter. A healthy child or adult may have a different risk-benefit balance from a hospitalized patient with a central line, a person receiving chemotherapy, or an infant born extremely early. Anyone in a high-risk category should ask the treating clinician or pharmacist before taking a probiotic supplement.

What “safe” means with probiotics
Probiotics are live microorganisms sold with the intention of providing a health benefit. Products can contain bacteria, yeast, or combinations of strains. Safety cannot be inferred from the word “probiotic” alone: two products with different strains can have different evidence, effects, storage requirements, and risks.
In healthy users, temporary gas, bloating, abdominal discomfort, constipation, or loose stools are the usual complaints. These symptoms often settle, but a new or severe symptom should not be dismissed as a normal adjustment. The evidence base is also incomplete. Many studies are small, use different products, or enroll people who are healthier than the person considering a supplement.

The main serious concern is an invasive infection, in which an organism associated with the product reaches the bloodstream or another normally sterile site. Reported complications include bacteremia (bacteria in blood), fungemia (fungi in blood), and severe infection. Such events are uncommon overall, but the risk is higher when the intestinal barrier, immune defenses, or medical devices are compromised.
Safety by population
Healthy children
Trials in generally healthy children have not shown an excess of serious adverse events overall, but pediatric evidence is limited and applies only to the strains and conditions studied. A product tested for one type of diarrhea should not be assumed to be safe or useful for a different purpose.
A CDC report described six cases of probiotic-strain bacteremia among 5,034 probiotic-exposed children who underwent cardiac surgery during 2019–2024. A 2021 review summarized 49 pediatric cases of invasive probiotic-associated infection. These reports do not show that infection is common, but they demonstrate that it can occur, particularly in medically complex settings.

For a child who is hospitalized, recovering from surgery, has an intravenous or central catheter, has a serious chronic illness, or is receiving immune-suppressing treatment, the supplement should be discussed with the hospital team rather than started independently. Seek urgent care for fever, chills, unusual sleepiness, breathing difficulty, persistent vomiting, or a child who appears acutely unwell after taking a probiotic.
Older adults
Older age by itself does not make probiotics automatically dangerous. However, older-adult evidence is sparse, and a person’s medical conditions often matter more than the number of years lived. Frailty, hospitalization, a central line, cancer treatment, organ disease, malnutrition, or immune suppression can increase the consequences of an invasive infection.
An AHRQ evidence review found no increased adverse-event risk in the limited adult and elderly trial data, while noting that very few studies specifically enrolled elderly people. The safety finding therefore cannot be treated as proof for every frail or medically complex older adult.

Benefit is also uncertain for a common use. NCCIH cites a 2016 review of 30 studies involving 7,260 participants that found no demonstrated reduction in antibiotic-associated diarrhea among elderly people. Only five of those studies focused on participants aged 65 or older. An older adult taking antibiotics should ask a clinician whether a particular strain is worthwhile instead of assuming that any probiotic will help.

NIH’s Office of Dietary Supplements (ODS) and the National Center for Complementary and Integrative Health (NCCIH) identify people with compromised immune systems or severe illness as a higher-risk group. Examples of weakened immunity or reduced ability to fight infection include HIV, autoimmune disease, chemotherapy or radiation, diabetes, liver disease, kidney disease, and alcoholism. The list is not exhaustive; the treating team should assess the individual situation.
For these patients, a probiotic decision should be individualized. A clinician may consider the person’s diagnosis, current infection risk, gut condition, medications, devices, nutritional status, and the exact strain and dose. Do not self-start a supplement for someone receiving cancer therapy, recovering from major surgery, critically ill, or being treated for an immune disorder. A supplement also must not replace antibiotics, rehydration, nutrition support, or another proven treatment.

“The risk of harmful effects from probiotics is greater in people with severe illnesses or compromised immune systems.”
—NCCIH, Probiotics: Usefulness and Safety
Premature or very-low-birth-weight infants
Preterm infants require a separate, stricter answer. The FDA states that live bacteria or yeast in probiotic products have caused invasive, potentially fatal disease in preterm infants. It also warns that these products have not undergone the agency’s rigorous premarket review for safety and effectiveness and that no probiotic is FDA-approved as a drug or biological product for infants.

The American Academy of Pediatrics statement reproduced by the FDA says current evidence does not support routine universal administration, particularly for infants with birth weight below 1,000 grams. Parents and caregivers should not give an over-the-counter probiotic to a premature or very-low-birth-weight infant unless the neonatal team explicitly directs and monitors its use. Ask the neonatal unit about the product identity, preparation, infection-control procedures, and stopping rules.

What the evidence says about possible benefits
Safety and effectiveness are linked: a product with no demonstrated benefit exposes a person to risk without a clear reason to accept it. Evidence is strongest only for certain strain-and-condition combinations.
Antibiotic-associated diarrhea
In an ODS-cited meta-analysis of 12 randomized trials involving 1,499 participants, Lactobacillus rhamnosus GG (LGG) reduced the incidence of antibiotic-associated diarrhea from 22.4% to 12.3%. When children and adults were analyzed separately, the statistically significant result was in children. The outcome depended on the strain, antibiotic, age, dose, and care setting; it cannot be generalized to every probiotic capsule or food.
A separate ODS summary reports that Saccharomyces boulardii reduced antibiotic-associated diarrhea in pooled studies of adults and children. That result is likewise strain-specific. It does not establish that another yeast, bacterium, mixture, or dose will produce the same effect, nor that an older adult or immunocompromised patient should use it.

Compare the complete product and the intended user, not just the number of colony-forming units (CFUs). A higher CFU count is not automatically better, and a large count cannot compensate for a missing strain identity or an unsuitable storage history.
| What to check | Why it matters |
|---|---|
| Genus, species, and strain | Clinical evidence is tied to the exact microorganism. “Probiotic blend” without strain details makes evidence matching difficult. |
| Purpose or indication | Evidence for antibiotic-associated diarrhea does not prove benefit for constipation, eczema, colic, or another condition. |
| Dose and duration | Use the amount and schedule studied or recommended by a clinician; do not infer a dose from another product. |
| Age suitability | Infant, child, adult, and older-adult evidence are not interchangeable. |
| Use-by date and storage | Heat, moisture, and time can reduce viability or change the product. Follow the label’s refrigeration and handling instructions. |
| Medical review | A pharmacist or clinician can screen for immune suppression, severe illness, devices, drug interactions, and a better-supported alternative. |
Check that the label identifies the genus, species, strain, dose, use-by date, and storage requirements. A product being listed on a marketplace does not establish FDA approval or suitability for an immunocompromised person. If the label omits the strain or gives no clear storage instructions, ask a pharmacist before buying it.
A practical decision process
- Identify the reason for use. Write down the symptom or diagnosis, the treatment already prescribed, and what improvement you expect. If there is no defined goal, the case for taking a supplement is weak.
- Screen for higher risk. Note prematurity, birth weight below 1,000 grams, hospitalization, recent surgery, a central or intravenous catheter, severe illness, cancer therapy, immune-suppressing medicines, HIV, autoimmune disease, diabetes, liver or kidney disease, and alcoholism.
- Ask the treating clinician or pharmacist. Provide the full product label, not just its brand or front-panel CFU number. Ask whether the exact strain has evidence for the intended condition, what dose and duration are appropriate, and when to stop.
- Follow handling instructions precisely. Observe the use-by date, refrigeration or room-temperature requirements, and contamination precautions. Do not share a container between people.
- Monitor and report. Mild gas can occur, but stop and seek medical advice for fever, chills, worsening abdominal pain, persistent vomiting or diarrhea, blood in stool, confusion, breathing problems, or any sign of systemic infection. For a premature infant, contact the neonatal team immediately about any concerning symptom.
When a clinician should review the choice first
- A child is hospitalized, postsurgical, catheterized, or medically complex.
- An older adult is frail, hospitalized, has cancer, organ disease, a central line, or takes immune-suppressing treatment.
- The person has HIV, an autoimmune condition, diabetes, liver or kidney disease, alcoholism, or another cause of impaired immunity.
- The person is critically ill, has a serious infection, or cannot reliably report symptoms.
- The intended user is a premature or very-low-birth-weight infant.
- The label does not identify the exact strain, dose, storage conditions, or use-by date.
Questions to ask before starting
- What exact genus, species, and strain is in this product?
- What evidence supports that strain for this person’s age and condition?
- What dose and duration should be used, and what result would justify stopping?
- Could the person’s illness, immune status, catheter, or medicines increase infection risk?
- How should the product be stored, and what should we do if it was left out of range?
- Which symptoms require same-day medical advice or emergency care?
Bottom line
Most healthy people experience only minor digestive effects from probiotics, but safety is strain- and person-specific. Evidence is limited for many children and older adults, and the risk of bacteremia, fungemia, or severe infection is higher in people who are immunocompromised or seriously ill. The FDA warns of potentially fatal invasive disease in preterm infants and does not approve probiotics as infant drugs or biological products. Match the exact strain and dose to a defined purpose, verify storage and labeling, and obtain professional advice before use in any medically vulnerable person.
