Are Pharmaceutical Stocks Defensive When Technology Stocks Decline?

Pharmaceutical stocks can be relatively defensive during a technology-led selloff, but they are not a guaranteed hedge. Demand for essential medicines is usually less sensitive to economic cycles than discretionary technology spending, and healthcare has outperformed information technology in several major drawdowns. However, drugmakers still face patent expirations, trial failures, pricing policy, reimbursement changes, product recalls, currency movements and broad market risk. Whether a particular pharmaceutical stock protects a portfolio depends on the cause of the technology decline, the company’s products and pipeline, its valuation, and how concentrated the investor’s existing technology exposure is.

What “defensive” means for a pharmaceutical stock

A defensive investment is one whose revenue and earnings tend to be less sensitive to economic slowdowns than those of cyclical or discretionary businesses. It does not mean the share price cannot fall. A defensive stock can decline during a market-wide liquidity shock, an industry-specific scandal or a period when investors abandon expensive shares.

Healthcare is conventionally classified as defensive. BlackRock Fundamental Equities wrote in 2026 that “medical needs do not wait on the economy,” with demand generally less sensitive to economic conditions in pharmaceuticals, providers and managed care. Patients still need medicines for chronic and acute conditions even when companies postpone software purchases, consumers reduce discretionary spending or business investment slows.

That defensive characteristic applies to the underlying demand for treatment, not automatically to every listed drugmaker. A company can have stable demand for medicines and still lose value if a key patent expires, a clinical trial fails, regulators reject a product, a payer removes a drug from its formulary or investors decide its valuation is too high.

Why pharma may hold up when technology sells off

Essential demand is less tied to the business cycle

Many technology companies depend on corporate capital spending, advertising budgets, consumer upgrades or optimistic expectations about future growth. Those revenue streams can weaken quickly when businesses cut budgets or investors reassess long-duration growth assets. Prescription medicines, by contrast, are often purchased because a clinician has diagnosed a condition and treatment is medically necessary.

The distinction is not absolute. Some therapies are elective, newly launched or dependent on discretionary spending. Insurance coverage, employment and government budgets also influence access to medicines. Nevertheless, the demand profile of a mature portfolio of essential drugs is usually steadier than that of businesses whose sales depend on rapid technology adoption.

Healthcare and technology are not perfectly linked

BlackRock’s analysis of rolling one-year S&P 500 sector returns from December 29, 1995, through March 27, 2026, found that healthcare and technology have shown low-to-moderate correlation over the longer term. The measured correlation was near a two-decade low during the recent artificial-intelligence-led technology narrative. Lower or changing correlation can give a portfolio different sources of return, although correlation can rise sharply in a broad panic.

A low historical correlation is not an inverse relationship. It does not imply that pharma rises whenever technology falls, or that the relationship will remain low during the next crisis. Investors should calculate correlation against their own technology holdings and review it over several windows rather than relying on a single long-term average.

Healthcare can benefit from different earnings drivers

Drug sales are influenced by disease prevalence, clinical efficacy, physician adoption, insurance coverage and regulatory decisions. Those drivers differ from the factors that move semiconductor, cloud, platform or software companies. That difference can reduce a portfolio’s exposure to one dominant growth narrative.

Long-term demographic demand is another supporting factor. S&P Global Market Intelligence, as cited by BlackRock in 2026, projects that people older than 65 will represent 20% of the U.S. population by 2030. Aging does not guarantee profits for any company, but it can support sustained need for treatments and healthcare services.

What major drawdowns show

A 2024 study by S&P Dow Jones Indices compared sector excess returns during several major market episodes. The figures below are sector-level results, not returns for an individual pharmaceutical company, and they should not be read as a forecast.

Market episode Health Care excess return Information Technology excess return
Dot-com crash (September 1, 2000–October 9, 2002) 27.60% -32.90%
Global Financial Crisis 17.30% 3.00%
COVID-19 2020 crash 5.90% 2.60%
2022 bear market 13.80% -7.20%

Healthcare outperformed information technology in all four episodes in this comparison, but the scale differed substantially. During the 2020 crash, both sectors were positive on an excess-return basis, and the advantage for healthcare was smaller than in the dot-com collapse. S&P also emphasized that no sector was spared in the 2020 crash. A diversified healthcare index can therefore be more resilient without being loss-proof.

“Excess return” is a sector comparison reported in the S&P study. It does not describe the total return of a particular stock, nor does it account for that company’s debt, dividend policy, product mix or country exposure. Past performance is no guarantee of future results.

Healthcare-sector evidence is broader than pharmaceutical stocks

Sector statistics often combine businesses with very different economics. The MSCI ACWI Health Care Index covers the healthcare sector, not just drug manufacturers. Its large constituents include Eli Lilly, Johnson & Johnson, AbbVie, Merck, UnitedHealth, Roche, Novartis, Thermo Fisher Scientific, AstraZeneca and Amgen.

That mix can include pharmaceutical manufacturers, managed-care companies, diagnostics, research tools and other healthcare businesses. A healthcare index may therefore have steadier diversification than a portfolio containing only a few drugmakers. Conversely, an index can contain risks that an investor seeking pure pharmaceutical exposure did not intend to own.

BlackRock reported that 89% of S&P 500 healthcare companies exceeded earnings expectations in all four quarters of 2025. This is a historical observation about that group and period, not evidence that every healthcare or pharmaceutical company will beat estimates in the future.

Why pharmaceutical stocks can still fall during a technology decline

Patent cliffs and product concentration

A medicine protected by exclusivity can generate substantial cash flow until generic or biosimilar competition arrives. When that protection ends, prices and sales can drop quickly. A company whose earnings depend heavily on one blockbuster faces greater patent-cliff risk than a diversified manufacturer with many products at different stages of their life cycles.

Clinical-trial and pipeline risk

Future growth often depends on medicines that have not yet completed clinical testing or received regulatory approval. A failed trial, an unexpected safety signal or a delayed submission can remove years of anticipated revenue from a valuation. Even a successful product may underperform if physicians adopt it slowly or competing treatments gain market share.

Pricing, reimbursement and litigation

Drugmakers are exposed to government price negotiations, reimbursement rules, insurer and pharmacy-benefit-manager decisions, formulary placement and lawsuits. Policy changes can affect a company even when patient demand remains strong. Litigation over safety, marketing or patents can also create large, unpredictable costs.

Manufacturing, recalls and supply chains

Production problems, contamination, shortages or a product recall can interrupt sales and damage a franchise. Manufacturing concentration matters: a company dependent on one plant, supplier or active ingredient has less operational flexibility than one with multiple qualified sources.

Valuation, interest rates and market liquidity

Defensive businesses are not immune to valuation risk. Investors may bid up stable healthcare earnings during a technology selloff, then reverse that trade when interest-rate expectations change or when a broad liquidity event forces them to sell. A high-multiple pharmaceutical stock can fall even if its next quarter’s medicine demand is steady.

The cause of the technology decline matters

“Technology stocks decline” describes an outcome, not a single economic event. The defensive response from pharma can differ depending on what caused the selloff.

  • Artificial-intelligence valuation reset: If investors simply reduce expectations for expensive technology growth, healthcare may provide diversification because its earnings are driven by treatment demand and research outcomes rather than AI spending. Highly valued biotech or pharmaceutical shares can still be sold if investors are reducing risk across all growth stocks.
  • Recession: Essential medicines may hold up better than discretionary technology services, but unemployment, insurance coverage and pressure on government or employer budgets can still affect utilization and pricing.
  • Inflation and rising rates: Higher discount rates can compress valuations across sectors. Companies with heavy debt, distant pipeline cash flows or expensive acquisitions may be more sensitive than mature drugmakers with strong current cash generation.
  • Falling long-term interest rates: Lower rates can support long-duration technology valuations, so a technology decline caused by something other than rates may have a different effect on healthcare. The direction of rates alone does not determine which sector wins.
  • Broad liquidity shock: When investors sell whatever they can, correlations can converge and defensive sectors can decline too. The 2020 experience, in which S&P said no sector was spared, is a reminder not to treat pharma as cash.

How to assess a pharmaceutical stock before rotating out of technology

A sector switch should begin with the portfolio problem you are trying to solve, not with the assumption that one industry will rise when another falls. Use the following checks for each company or fund under consideration.

What’s actually slowing this PC down?

1. Measure your existing technology exposure

  • Identify direct holdings, broad-market funds and employer plans that own the same large technology companies.
  • Separate business exposure from factor exposure. A portfolio can be concentrated in high-growth, high-multiple or long-duration companies even when no single technology stock dominates.
  • Review how the holdings behaved during prior drawdowns, while recognizing that historical relationships can change.

2. Test the pharmaceutical company’s earnings durability

  • List the products that generate current revenue and estimate how much sales depend on the largest one or two medicines.
  • Map patent and exclusivity dates, expected generic or biosimilar competition and the replacement products in development.
  • Review trial stages, regulatory milestones and the consequences of a failure for the company’s valuation.
  • Check manufacturing capacity, supplier concentration, recall history and geographic exposure.

3. Examine policy and payer exposure

  • Determine which markets set prices or negotiate reimbursement for the company’s major products.
  • Assess dependence on public programs, commercial insurers, pharmacy-benefit managers and hospital formularies.
  • Consider pending legislation, regulatory reviews and litigation as separate risks rather than assuming that “healthcare” removes policy uncertainty.

4. Compare valuation with the quality of the cash flows

Compare forward valuation measures with the durability, growth rate and concentration of earnings. A lower price-to-earnings multiple is not automatically safer if earnings are about to fall after a patent expiry. Conversely, a premium valuation can be reasonable for a company with diversified products, a strong balance sheet and credible replacement growth, but the premium still creates downside if expectations disappoint.

5. Decide whether you need a sector or a company

A diversified healthcare allocation can reduce single-product and single-trial risk, while an individual pharmaceutical stock can produce a much wider range of outcomes. An index may also include insurers, diagnostics and research-tool companies, so read its methodology and holdings before treating it as a pure pharma position.

Pharmaceutical stocks versus a diversified healthcare allocation

Pure pharmaceutical exposure offers a direct claim on medicines, but it concentrates risk in clinical development, patents, pricing and manufacturing. A broader healthcare allocation spreads exposure across drugmakers and other healthcare businesses. That can reduce the impact of one failed trial or product recall, although it also introduces different risks, such as insurer regulation, medical-device demand and laboratory-equipment cycles.

The right comparison is therefore not “technology versus pharma” in isolation. It is the risk profile of the proposed healthcare holding versus the risks already embedded in the investor’s technology exposure. A portfolio dominated by a handful of expensive technology shares may benefit from adding businesses with different earnings drivers; a portfolio already diversified across sectors may gain little from an additional concentrated pharmaceutical position.

Common mistakes to avoid

  • Assuming an automatic inverse move: Historical low-to-moderate correlation is not a rule that pharma rises whenever technology falls.
  • Confusing healthcare with pharmaceuticals: Sector indexes include several industries, so their results may not match a drugmaker’s results.
  • Ignoring valuation: A defensive label does not protect an overpriced share from a multiple contraction.
  • Buying after a headline-driven rally: If many investors have already rotated into healthcare, expected resilience may be reflected in the price.
  • Overlooking company concentration: A single blockbuster, trial or manufacturing site can dominate a stock’s risk.
  • Using crisis returns as a promise: The S&P study covers selected historical episodes; it does not predict the next technology selloff.

Bottom line

Pharmaceutical stocks are potentially defensive when technology stocks decline because medicine demand is generally less cyclical, healthcare and technology have historically shown low-to-moderate correlation, and healthcare outperformed information technology in the four drawdowns reported by S&P Dow Jones Indices. The evidence supports “historically more resilient in some drawdowns,” not “safe,” “inverse” or “guaranteed hedge.”

Before rotating, identify why technology is falling, measure your actual concentration, distinguish a diversified healthcare index from a pure drug portfolio, and examine each company’s patents, pipeline, policy exposure, valuation and balance sheet. That process addresses the risks a defensive label cannot remove.
