Are Low-Sodium Foods Safe for People With High Blood Pressure or Kidney Disease?

Usually, yes—but kidney disease makes the answer individual. For high blood pressure, replacing salty packaged and restaurant foods with lower-sodium choices is consistent with American Heart Association guidance and can improve blood pressure. For chronic kidney disease (CKD), sodium reduction often helps blood pressure and fluid control, but the right diet also depends on kidney function, potassium and phosphorus results, protein and fluid needs, medicines, swelling, heart conditions, and whether you receive dialysis. A food labeled low sodium is not automatically safe for every person with CKD.

What “low sodium” means on a food label

In U.S. labeling, low sodium means 140 milligrams (mg) of sodium or less per serving. Very low sodium means 35 mg or less per serving. Reduced sodium means the product has at least 25% less sodium than its regular version; it can still contain considerably more than 140 mg. These terms describe one labeled serving, not necessarily the portion you will eat.

Check the serving size first. If a package lists two servings and you eat the whole package, multiply the sodium number by two. Compare the sodium in the portion you actually consume, not just the front-of-package claim. A product can be low sodium per serving yet contribute a large amount when several servings are eaten.

Why lowering sodium helps high blood pressure and kidney disease

Sodium attracts water. A high-sodium intake can increase the amount of fluid in the bloodstream, raising blood pressure. When kidney function is reduced, the kidneys may be less able to remove sodium and water. The resulting fluid retention can cause thirst and swelling and add strain to the heart.

For people with hypertension but no kidney disorder, choosing low-sodium foods and reducing packaged, prepared, and restaurant meals is generally safe. The American Heart Association says that cutting about 1,000 mg of sodium per day can improve blood pressure for many people. More than 70% of Americans’ sodium intake comes from packaged, prepared, and restaurant foods, so changing those sources often has more effect than simply removing the salt shaker.

In CKD, sodium reduction is commonly part of treatment because it can support blood-pressure control and reduce edema. It is not a complete kidney diet, however. A meal can be low in sodium but high in potassium, phosphorus, protein, or fluid—nutrients that may need limits at particular stages of CKD.

Daily sodium targets: which number applies?

Guidance Target How to interpret it
American Heart Association (2025) No more than 2,300 mg sodium per day General upper limit for adults, including people managing high blood pressure.
American Heart Association (2025) 1,500 mg per day Ideal goal for most adults, particularly useful for blood-pressure reduction when it can be achieved safely.
KDIGO CKD guideline (2024) Below 2,000 mg sodium per day (below 90 mmol, or below 5 g of sodium chloride) Suggested target for people with CKD; the recommendation is conditional and must fit the individual’s clinical situation.

These are population-level recommendations, not a substitute for your clinician’s prescription. KDIGO specifically notes that dietary sodium restriction is usually not appropriate for people with sodium-wasting nephropathy. Advanced CKD, low blood pressure, recurrent dehydration, heart failure, edema, or medicines that alter fluid and electrolyte balance may change the plan. Ask your treating clinician or renal dietitian to set a personal target rather than choosing the lowest possible number on your own.

Foods that usually fit a lower-sodium pattern

Build meals from minimally processed ingredients

  • Fresh or frozen vegetables without seasoning sauces; choose unseasoned frozen vegetables when fresh is impractical.
  • Fresh fruit, unless your kidney team has prescribed a potassium or fluid restriction that changes your choices.
  • Unseasoned cuts of poultry, fish, meat, eggs, or plant proteins in the portions recommended for your kidney stage.
  • Plain grains such as rice, oats, pasta, or other unsalted staples.
  • Plain milk or yogurt only when the phosphorus, potassium, and fluid content fit your plan.

Use garlic, onion, herbs, spices, citrus juice or zest, and vinegar to add flavor without relying on salt. “No salt added” means no salt was added during processing; it does not guarantee that the food contains no sodium, so the Nutrition Facts panel still matters.

Foods to limit because sodium accumulates quickly

  • Processed meats such as bacon, sausage, ham, deli meat, and hot dogs.
  • Canned soups, boxed dinners, instant noodles, and frozen meals.
  • Fast food, pizza, takeout, and restaurant sauces or gravies.
  • Salty snack foods, crackers, chips, and salted nuts.
  • Pickles, olives, soy sauce, bottled dressings, marinades, and seasoning packets.

“Reduced sodium” versions can be useful comparisons, but they are not the same as low-sodium products. Compare brands by the sodium per serving and by the number of servings you will actually eat.

How to shop and cook safely

  1. Start with the serving size. Note both the listed portion and how many portions are in the container.
  2. Calculate your real intake. Multiply sodium per serving by the number of servings you plan to eat, then account for sauces, sides, and beverages.
  3. Compare alternatives. Look for no-salt-added, low-sodium, or reduced-sodium versions, and choose the option with less sodium per actual portion.
  4. Scan the ingredient list. Sodium can appear in broths, curing agents, sauces, flavor enhancers, and seasoning blends even when a food does not taste very salty.
  5. Check potassium and phosphorus additives. In CKD, a lower-sodium product may contain potassium chloride or phosphorus-containing ingredients that are unsuitable for your laboratory results.
  6. Cook without automatic salting. Taste food after cooking and use herbs, spices, garlic, onion, citrus, or vinegar for flavor.
  7. Recheck the whole meal. A low-sodium entrée can become a high-sodium meal when paired with soup, a packaged side, pickles, or a salty condiment.

Are salt substitutes safe with kidney disease?

Not automatically. Many salt substitutes replace sodium chloride with potassium chloride. If your blood potassium is high, your kidneys cannot excrete potassium effectively, or you take medicines that raise potassium, a potassium-based substitute can be dangerous. The National Kidney Foundation advises checking with your care team because these products may not be safe when potassium is elevated.

Do not start a potassium-containing salt substitute, “lite salt,” or potassium seasoning based only on the front label. Ask your clinician or renal dietitian to review the exact product and your current potassium result, kidney function, and medicines. A substitute that is acceptable for one person may be inappropriate after a medication change or a change in kidney function.

Why a low-sodium product can still be wrong for CKD

Potassium

Some people with CKD need to limit potassium, while others do not. Potassium chloride additives, certain fruits and vegetables, dairy foods, and some packaged products can matter more than the sodium number. Your blood potassium and treatment plan determine whether a food is suitable.

Phosphorus

Phosphorus additives are common in processed foods and may be absorbed readily. A low-sodium frozen meal or beverage can still be a poor choice if your phosphorus is high. Look for phosphorus-containing ingredients when your kidney team has prescribed a phosphorus limit.

Protein

Protein needs vary by CKD stage, nutritional status, and dialysis. Avoid making broad changes—such as eliminating protein or replacing every meal with processed meat alternatives—without individualized advice.

Fluids and swelling

Sodium can worsen thirst and fluid retention. People with edema, heart failure, or dialysis may have a separate fluid allowance. NIDDK notes that reducing sodium can help edema and blood pressure in people with congestive heart failure or edema, but the amount you may drink remains a clinical decision.

Medicines and dialysis

Diuretics, blood-pressure medicines, potassium-affecting drugs, phosphate binders, and dialysis schedules all influence dietary advice. A food plan that worked before dialysis or before a medication change may need revision. Bring your medication list and recent laboratory results to a dietitian visit.

When to ask for an individualized plan

Arrange guidance from your treating clinician or a renal dietitian if you have CKD, especially if you have advanced disease, abnormal potassium, edema, heart failure, dialysis, sodium-wasting nephropathy, or medicines that affect potassium or fluid balance. Ask for four concrete items:

  • Your daily sodium target and whether it is a firm limit or a range.
  • Whether potassium, phosphorus, protein, or fluid also require limits.
  • Which salt substitutes and packaged-food additives are acceptable for your laboratory results.
  • How often to repeat blood tests or review swelling, blood pressure, and weight.

Contact your care team if swelling, thirst, or fluid-related symptoms are worsening, or if you are unsure whether a new product contains potassium chloride. Do not compensate for a high-sodium meal by skipping prescribed medicines or drastically restricting fluids unless your clinician tells you to do so.

A practical way to answer “Can I eat this?”

Use a two-stage check. First, evaluate sodium: serving size, sodium per serving, number of servings, and the rest of the meal. Second, evaluate kidney compatibility: potassium and phosphorus additives, protein and fluid content, your latest laboratory values, medicines, CKD stage, and dialysis instructions. If any of those kidney-specific factors are unknown, the safest choice is to ask your clinician or dietitian before making the product a regular part of your diet.

For someone with high blood pressure alone, a pattern centered on fresh or unseasoned foods, lower-sodium packaged choices, and less restaurant food is a sound starting point. For someone with CKD, the same pattern is often useful, but it must be fitted to the person’s laboratory results and treatment plan rather than followed as a universal menu.
