Are Artificial Blood Products Safe? What Clinical Trials and Regulators Require

Artificial blood products cannot be called broadly safe today. No hemoglobin-based oxygen carrier (HBOC) or perfluorocarbon-based oxygen carrier (PFOC) has U.S. Food and Drug Administration (FDA) approval for routine human transfusion. Human HBOC trials repeatedly found serious toxicity or failed clinically important endpoints, while PFOCs remain investigational. Safety has to be demonstrated separately for each product, intended use and jurisdiction.

The practical question is therefore not whether “artificial blood” is safe in the abstract. It is whether a particular formulation can deliver oxygen without causing unacceptable cardiovascular, inflammatory, neurologic, kidney or other organ injury, and whether regulators accept the manufacturing and clinical evidence for a defined indication.

What “artificial blood” means

Artificial blood is an umbrella term for products intended to transport oxygen or otherwise support circulation when donated red blood cells are unavailable, unsuitable or insufficient. The major oxygen-carrying approaches are HBOCs, which use purified or modified hemoglobin outside intact red cells, and PFOCs, which use emulsified perfluorocarbon compounds that dissolve and transport gases.

These products are not interchangeable. Their chemistry, particle or droplet size, oxygen-release behavior, persistence in the body, clearance and toxicities differ. A favorable result for one formulation would not establish the safety of another. Regulators therefore assess each product for a specific indication, dose, route and patient population.

Current safety and approval status

No FDA-approved oxygen-carrying substitute

FDA scientists reported in 2018 that every HBOC tested in humans had failed clinical studies because of toxicity, and that FDA had approved no HBOC for human use. The NIH Blood-Based Products Regulatory Knowledge Guide states: “Currently, no FDA-approved oxygen-carrying blood substitute product exists commercially.” A 2025 review likewise concluded that no HBOC had obtained FDA approval for clinical use after large trials showed excess adverse events or failed prespecified endpoints.

The same regulatory position applies to PFOCs: a 2024 review reported that no HBOC or PFOC had been approved by either FDA or the European Medicines Agency (EMA) at the time of publication. Products in these classes may be studied in clinical trials or made available under tightly controlled access pathways, but that is not the same as routine marketing authorization.

Hemopure is not routine U.S. approval

Hemopure (HBOC-201, also called hemoglobin-glutamer-201) illustrates why approval language matters. A 2025 review describes it as approved for canine anemia in the United States and European Union, approved for human use in South Africa in 2001 and Russia in 2006, and permitted for expanded-access use in the United States. Those permissions do not make it FDA-approved for routine human transfusion in the United States. Expanded access allows use under specified conditions when no comparable option exists; it does not establish commercial approval or broad safety.

Why early hemoglobin products caused harm

Free hemoglobin behaves differently from hemoglobin inside red cells

Intact red cells package hemoglobin inside a membrane and regulate how it interacts with blood vessels. When hemoglobin is released into plasma, it can oxidize after releasing oxygen and become chemically reactive. FDA explanations identify several linked hazards:

  • Nitric-oxide scavenging: cell-free hemoglobin can remove nitric oxide, a signaling molecule that normally helps blood vessels relax.
  • Vasoconstriction and hypertension: reduced nitric oxide can narrow vessels and raise blood pressure, sometimes severely.
  • Cardiovascular events: excessive vascular constriction and oxidative stress can contribute to heart attack or stroke.
  • Organ injury: reactive hemoglobin and downstream inflammation can damage kidneys, heart, brain and other tissues.

The 2025 review notes that purified native hemoglobin produced significant hypertension and evidence of multiple-organ toxicity in early studies. Chemical modification can change circulation time and oxygen delivery, but it also creates new structural variants and impurities that must be characterized and controlled.

Oxygen delivery alone is not an adequate endpoint

An HBOC may raise a measured oxygen-delivery variable while worsening outcomes that matter to patients. Historical trials showed why regulators require mortality, cardiovascular events, organ injury and other serious adverse events to be evaluated alongside physiologic measurements. A product cannot be considered successful merely because it carries oxygen if patients experience more heart attacks, strokes, kidney injury or deaths.

What remains unresolved for perfluorocarbon carriers

PFOCs dissolve oxygen and carbon dioxide rather than binding oxygen with hemoglobin. Their small droplets are administered as emulsions, so performance depends on formulation, droplet size, stabilizers, dose and how the body distributes and clears the material.

A 2024 review identified several translation bottlenecks:

  • Pharmacologic side effects unrelated to oxygen transport itself.
  • Nanomaterial and formulation complexity that can affect stability and tissue interactions.
  • Persistence in the body and uncertainty about clearance over clinically relevant time periods.
  • Clinical-trial designs that have not always matched the intended emergency or perioperative use.

These issues do not prove that every PFOC is unsafe; they mean that safety and efficacy remain unestablished at the class level. Developers must show what happens to the emulsion after dosing, how long it remains, which organs receive it, and whether adverse effects increase with dose or repeated exposure.

How regulators define an acceptable product

FDA and EMA requirements are built around a product’s chemistry, manufacturing process, pharmacology, clinical benefit and risks. The following evidence is needed before a sponsor can seek marketing authorization.

1. Mechanistic and product characterization

Developers must specify the active material and demonstrate how it behaves. For an HBOC, that includes the source and chemistry of hemoglobin, cross-linking or polymerization, oxygen-binding and release curves, molecular-size distribution, impurities, oxidation state, stability and interactions with vascular tissues. FDA research emphasizes that HBOCs are chemically heterogeneous: structural differences can change both oxygen delivery and toxicity.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For a PFOC, characterization includes the perfluorocarbon compound, emulsion composition, droplet-size distribution, surfactants or other excipients, gas-dissolution properties, storage stability and degradation products. Regulators need validated analytical methods that can detect changes between lots and during shelf life.

2. Preclinical safety testing

Animal and laboratory studies should probe the failure modes suggested by the mechanism, not just confirm that oxygen levels rise. Typical assessments include:

  • Blood pressure, vascular resistance and evidence of vasoconstriction.
  • Oxidative stress, inflammatory signaling and complement or immune activation.
  • Thrombosis, platelet interactions and effects on coagulation.
  • Kidney, heart, brain and lung injury, with appropriate tissue and laboratory measurements.
  • Immunogenicity, hypersensitivity and repeat-dose reactions.
  • Distribution, metabolism, clearance and persistence.
  • Interactions with aging, diabetes, atherosclerosis, sepsis, trauma or other intended-use conditions.

FDA guidance identifies unresolved safety problems in both hemoglobin products and perfluorochemical emulsions. A formulation that appears tolerable in healthy animals may behave differently in patients with damaged vessels, impaired kidneys or systemic inflammation, so the model and exposure range must reflect the proposed clinical use.

3. A justified clinical indication and comparator

A trial must state exactly when the product is intended to be used: for example, a defined surgical blood-loss setting, oxygen delivery during a shortage, or another narrowly specified circumstance. The comparator should represent accepted care for that indication, and the protocol should explain when rescue transfusion or other therapy is allowed.

Investigators must prespecify clinically meaningful endpoints, enroll enough participants to detect important harms, and systematically capture serious adverse events. Depending on the indication, endpoints may include survival, organ-failure rates, myocardial infarction, stroke, kidney injury, transfusion avoidance, functional recovery and duration of effect. Oxygen tension or another laboratory surrogate can support the evidence, but it cannot replace patient-centered outcomes when toxicity is plausible.

4. Manufacturing, quality and lot release

Every dose must have reproducible composition, sterility, potency, stability and impurity limits. Sponsors submit chemistry, manufacturing and controls information describing raw materials, production steps, equipment, in-process testing, packaging, storage conditions and release specifications. Small changes in polymer size, oxidation, particle size or excipient concentration can alter pharmacology, so regulators expect process controls that keep clinically relevant attributes within validated ranges.

Stability studies must support the proposed shelf life and show what happens after transport, temperature excursions or container opening. A product that is theoretically useful in a remote emergency is not practical unless its quality can be maintained through the logistics of that setting.

5. Marketing-authorization review

FDA’s oxygen-therapeutics guidance is intended to help evaluate the safety and efficacy of red-blood-cell substitutes. EMA blood-product guidelines likewise help developers prepare marketing-authorization applications and assess clinical efficacy and safety. Reviewers weigh the totality of evidence for the proposed indication; a product might have a different risk-benefit balance in a short-term, life-threatening circumstance than in elective or repeated use, but the sponsor must demonstrate that balance rather than assume it.

Why trial design has determined the fate of blood substitutes

Endpoints must reflect the real clinical decision

Patients and clinicians need to know whether an oxygen carrier improves outcomes compared with available treatment, not simply whether it changes a number on a monitor. Trials should define how much transfusion avoidance, improved survival or reduced organ injury would justify exposure to product-specific risks. If mortality or major cardiovascular events rise, a favorable surrogate cannot rescue the result.

Safety monitoring must be active and long enough

Blood-pressure spikes may appear soon after infusion, whereas kidney injury, inflammation or persistence may emerge later. Protocols therefore need frequent early vital-sign checks, laboratory and organ-function monitoring, adjudication of serious events, and follow-up long enough to detect delayed toxicity. Independent safety oversight and prespecified stopping rules are especially important when earlier products have shown class-related hazards.

Population and indication matter

Results in healthy volunteers cannot establish safety for older adults with atherosclerosis, patients with sepsis, or people undergoing major trauma. Enrollment criteria, dosing and rescue treatment should mirror the population in which the product will actually be used. Analyses should examine whether age, diabetes, kidney impairment, vascular disease or inflammatory states change risk.

Comparing the two main artificial-blood approaches

Dimension Hemoglobin-based oxygen carriers (HBOCs) Perfluorocarbon-based oxygen carriers (PFOCs)
Oxygen transport Purified or modified hemoglobin carries and releases oxygen outside red cells; exact performance depends on molecular design. Perfluorocarbon emulsions dissolve and transport gases; performance depends on compound and emulsion formulation.
Principal safety concerns Vasoconstriction, hypertension, oxidative and inflammatory injury, cardiovascular events and organ toxicity. Pharmacologic side effects, formulation or nanomaterial effects, persistence and clearance concerns.
Clinical evidence Human products have had serious adverse-event findings or failed endpoints in trials. Still investigational; reviews identify design and translation barriers.
FDA/EMA status No FDA approval for human clinical use; no FDA or EMA approval reported in the 2024 review. No FDA or EMA approval reported in the 2024 review.
Manufacturing challenge Controlling hemoglobin structure, oxidation, size distribution, impurities and lot consistency. Controlling compound purity, droplet size, excipients, stability and clearance-related attributes.

Both approaches must ultimately demonstrate adequate oxygen delivery, acceptable toxicity, predictable circulation and clearance, useful storage life, consistent manufacturing and benefit in a defined clinical setting. Neither class has yet met that standard for routine U.S. transfusion.

What “safe” should mean for a future product

Regulatory safety does not mean zero risk. It means that, for a specified indication, the known and potential risks are reasonable in relation to demonstrated benefits, with manufacturing and dosing sufficiently controlled to reproduce the result. A product could conceivably be acceptable for a narrowly defined emergency use while being inappropriate for elective surgery or repeated administration.

Before adoption, clinicians would need clear instructions on dose, infusion rate, contraindications, monitoring, rescue transfusion and management of hypertension, allergic reactions or organ injury. Hospitals would also need validated storage, compatibility and adverse-event reporting procedures. Those operational requirements are part of safe use, not an afterthought.

How to interpret headlines about “synthetic blood”

  • “Approved abroad” may refer to a different regulator, indication or patient population. It does not imply FDA approval.
  • “Expanded access” is controlled permission for individual or limited use, not a commercial license.
  • “Successful oxygen delivery” may describe a surrogate endpoint rather than survival or organ safety.
  • “Artificial blood” may refer to an early laboratory formulation, an animal study or a clinical product; the stage must be specified.
  • “No blood type needed” does not remove risks from chemistry, vascular effects, immune reactions or clearance.

Bottom line for patients and clinicians

There is no broadly proven, FDA-approved artificial blood product that can routinely replace donated red-cell transfusion in the United States. HBOCs taught developers that carrying oxygen is not enough when free hemoglobin can constrict blood vessels, promote oxidative injury and damage organs. PFOCs remain experimental and face unresolved pharmacologic, formulation, persistence and trial-design questions.

The credible path to approval is product-specific: define the chemistry, test the relevant toxicities in realistic models, run adequately powered trials with meaningful clinical endpoints, control every manufacturing lot, and show regulators that benefits outweigh risks for the proposed indication. Until that evidence exists, claims that artificial blood is simply “safe” are too broad to be medically or regulatorily accurate.
