Doctors do not use a universal rule that one treatment is better. For most adults, they offer psychotherapy, a second-generation antidepressant, or both after assessing symptom severity, suicide risk, possible bipolar disorder, previous treatment response, other health conditions, practical access and the patient’s informed preference. The American Psychological Association (APA) recommends offering either psychotherapy or a second-generation antidepressant initially through shared decision-making. The National Institute for Health and Care Excellence (NICE) advises against routinely making antidepressants the first treatment for less severe depression unless the person prefers them; for more severe depression, individual cognitive behavioral therapy (CBT), antidepressants and combined care are all first-line options.

This guide explains what those choices mean, how clinicians compare them, when combined treatment makes sense, how long an initial trial is usually reviewed, and what to do if the first plan is not enough. It is educational information, not a diagnosis or prescribing instruction.

What the choice between medication and therapy actually means

“Antidepressants versus therapy” is usually not a contest with one winner. It is a treatment-planning decision. Psychotherapy asks you to work with a trained professional on thoughts, emotions, behavior, relationships and problem-solving. Antidepressants are prescription medicines taken on a regular schedule and monitored for benefit, adverse effects and safe changes. Combined treatment uses both approaches.

The appropriate starting point depends on the depression being treated, not simply on a preference for “natural” or “medical” care. Two people with the same diagnosis can reasonably receive different plans because their severity, safety risks, history, health conditions, resources and goals differ.

Comparative studies reviewed by the APA found broadly similar effects among several psychotherapy models. The evidence does not identify one second-generation antidepressant that is best for every adult. That is why clinicians compare fit, prior response, risks, likely burden and personal preference instead of promising a universal success rate. Official sources used for this guidance do not report one comparative percentage that applies to all adults.

How doctors assess you before recommending a treatment

Severity, functioning and urgency

A clinician first establishes whether symptoms meet criteria for depression and how much they interfere with sleep, work, relationships, self-care and safety. Less severe depression often starts with lower-intensity psychological or behavioral options and a shared choice. More severe depression can justify structured psychotherapy, an antidepressant, or both at the outset.

Severity is not the same as how bad a single day feels. Duration, persistent impairment, inability to care for yourself, psychotic symptoms, recurrent episodes and the speed of deterioration all matter. A person with apparently moderate symptoms but rapidly worsening functioning may need a more urgent or intensive plan.

Suicide risk and immediate safety

Every depression assessment should include suicidal thoughts, intent, access to lethal means, previous attempts, self-harm and protective support. A high or rapidly changing risk can require urgent evaluation, a safety plan, involvement of trusted people and a higher level of care rather than an ordinary medication-versus-therapy choice. If you may act on suicidal thoughts or cannot stay safe, contact local emergency services or a local crisis service immediately.

Bipolar disorder and other explanations

Clinicians screen for past periods of unusually elevated or irritable mood, reduced need for sleep, racing thoughts, impulsive behavior or unusually high energy. Depression can also be related to substance use, trauma, medication effects, thyroid or other medical conditions, grief, psychosis or anxiety disorders. Missing bipolar disorder can change the treatment plan substantially, so an antidepressant should not be started as a reflex before the assessment is complete.

Medical conditions, interactions and life stage

Prescribers review other medicines, allergies, alcohol or drug use, pregnancy or plans for pregnancy, breastfeeding, physical illnesses and overdose risk. These factors influence which medicines are reasonable and how closely someone should be monitored. Therapy also needs adaptation for cognitive, sensory, language, mobility or scheduling needs.

Previous episodes and past treatment response

A treatment that helped you before is useful evidence, although it is not a guarantee that it will work the same way now. The clinician asks which therapy model or medicine was tried, at what dose or intensity, for how long, whether it was taken or attended consistently, what improved and which adverse effects occurred. Recurrent episodes, residual symptoms after recovery or a severe previous illness can increase concern about relapse and affect continuation planning.

Preferences and feasibility

NICE treats shared decision-making as part of care. You can ask questions, decline an option and revisit the decision. Preferences may involve concerns about side effects, withdrawal, stigma, daily tablets, talking about painful experiences, speed of relief, privacy, cost, transportation, therapist availability, work or caregiving schedules and the ability to attend regularly. A theoretically suitable treatment that cannot be accessed or followed is not a practical first choice.

What major guidelines recommend

Clinical situation Guidance What it means in practice
Initial treatment for adults APA guideline (2019) recommends offering either psychotherapy or a second-generation antidepressant, using shared decision-making. There is no mandatory medication-first or therapy-first rule for every adult.
Less severe depression NICE (2022) says not to routinely offer antidepressants first-line; offer them when they are the person’s informed preference. Lower-intensity psychological or behavioral choices are commonly discussed first, while respecting a medication preference.
More severe depression NICE lists individual CBT, antidepressants and combined treatment among first-line options. Its rationale found good evidence and average cost-effectiveness for CBT plus antidepressants, individual CBT and individual behavioral therapy. Structured therapy, medication or both may be reasonable from the start; the final choice remains individualized.
Considering combined care APA recommends CBT or interpersonal psychotherapy plus a second-generation antidepressant when combined treatment is chosen. The two components should be planned deliberately rather than added without review.
Early review NICE recommends reviewing treatment response between two and four weeks after starting, including adherence, side effects, harms and suicidal ideation, particularly early on. Do not wait silently for months if symptoms worsen or significant adverse effects appear.

Therapy, antidepressants and combined care compared

Feature Psychotherapy Antidepressant Combined treatment
What you do Attend sessions, practise skills and discuss patterns, emotions, behavior and relationships with a trained professional. Take a prescribed medicine on schedule and report benefit, side effects and safety concerns to the prescriber. Attend therapy while taking and monitoring medication.
Potential advantages Builds coping and relapse-prevention skills; avoids medication adverse effects and withdrawal. Does not require finding a therapist or attending sessions; may be practical when symptoms make intensive psychological work difficult. Addresses biological and psychological factors together and can be useful when symptoms are more severe or one treatment alone is incomplete.
Main burdens Regular appointments, active participation, emotional effort, cost and limited therapist availability. Daily dosing, possible adverse effects, interaction checks, early monitoring and a planned taper rather than abrupt stopping. Combines the scheduling, participation and monitoring demands of both approaches.
Key safety considerations Therapist competence, appropriate treatment for the diagnosis and a plan for escalating risk. Medical history, interactions, pregnancy considerations, overdose risk, mood switching and withdrawal planning. Monitoring both medication effects and therapy progress, with clear responsibility for follow-up.
Evidence position Several psychotherapy models have similar average effects in comparative studies. No single second-generation antidepressant is established as best for every adult. NICE reports good evidence for CBT plus antidepressants in more severe depression, while still requiring personalization.

How a shared decision is made step by step

  1. Confirm the problem and urgency. The clinician evaluates depressive symptoms, duration, functioning, suicide risk, bipolar features, substance use and medical contributors.
  2. Describe realistic options. You should hear the expected benefits, common and serious risks, likely burden, monitoring needs, withdrawal issues and alternatives. NICE specifically calls for discussion of why medication is being offered, choices, dose adjustment, benefits, harms and withdrawal.
  3. Match treatment to the pattern. Severity, recurrence, residual symptoms, prior response and coexisting conditions narrow or broaden the reasonable options.
  4. Check feasibility. Consider appointment availability, travel, work, caregiving, language, privacy, cost, ability to take daily medication and willingness to practise between sessions.
  5. Agree on a review plan. Set the follow-up date, what improvement will look like, which side effects require contact, how suicidal thoughts will be handled and who to call between visits.
  6. Reassess rather than drift. If the response is inadequate, verify that the treatment was delivered at a useful intensity and long enough to judge before changing, adding or stopping it.

When therapy may be the preferred starting point

For less severe depression, NICE advises against routinely making antidepressants first-line unless the person prefers them. A structured psychological treatment can be especially attractive when you want to avoid medication adverse effects or withdrawal, when symptoms are linked to patterns that therapy can address directly, or when you are willing and able to practise skills between sessions.

Therapy is not simply an informal conversation. The clinician should use an evidence-based approach matched to your needs. CBT focuses on links among thoughts, feelings and actions; interpersonal psychotherapy addresses relationships, role changes, grief and communication; behavioral approaches target withdrawal and loss of routine. The APA found similar average effects across several psychotherapy models, so fit with your problems and therapist competence matter more than declaring one model universally superior.

Access can be the limiting factor. Long waits, transportation, lack of privacy, language mismatch or unaffordable fees may make a medication or a different delivery format more feasible. Tell the clinician when these constraints exist; they are part of treatment selection, not a personal failure.

When an antidepressant may be preferred or added

An antidepressant can be a reasonable first choice when depression is more severe, when previous medication helped, when therapy is unavailable or unacceptable, or when symptoms make active psychological work difficult. It can also be added after an adequate therapy trial that has produced only partial improvement. The decision should reflect the individual medicine’s risks and your medical history; the guidelines do not identify one universal medication for every adult.

Medication requires a prescriber who can review interactions, medical conditions, pregnancy considerations, overdose risk and possible mood switching. Ask how often follow-up will occur, what benefit should appear, which adverse effects are expected, and how the medicine would eventually be tapered. Do not start, change the dose, combine medicines or stop abruptly without prescriber guidance.

Early treatment is a monitoring period, not a test of willpower. NICE calls for review between two and four weeks after starting, with attention to adherence, side effects, harms and suicidal ideation. Contact the prescriber sooner for severe or alarming symptoms, rapidly worsening mood, agitation, unusual elevated energy, allergic reactions or thoughts of self-harm.

When doctors recommend combined treatment

Combined care is often considered when depression is more severe, recurrent, associated with substantial impairment, or only partly improved with one modality. NICE’s rationale found good evidence and average cost-effectiveness for CBT plus antidepressants in adults with more severe depression. The APA specifically names CBT or interpersonal psychotherapy combined with a second-generation antidepressant.

Combination does not mean every person needs two treatments indefinitely. The clinician should define the purpose of each component, coordinate follow-up and decide what will happen if one component causes problems. The added appointments, daily dosing, cost and emotional effort are real burdens, so the expected benefit should justify them.

How long to try a treatment before changing it

There is no single deadline that fits every person, because dose, treatment intensity, adherence, diagnosis and safety alter the timetable. NICE recommends an early review for anyone receiving treatment between two and four weeks after starting. At that review, clinicians look for early benefit, worsening symptoms, side effects, suicidal thoughts and obstacles to following the plan.

For a limited response, NICE advises reassessing after about four weeks of antidepressant treatment, or four to six weeks of psychological therapy or combined treatment. The review should check whether sessions or doses were actually received and consider personal stress, social circumstances, physical health and coexisting conditions before labeling a treatment ineffective.

A change may be appropriate sooner when safety is deteriorating or adverse effects are unacceptable. Conversely, a partial but steady improvement may justify continuing with closer monitoring while refining the plan. Only the treating clinician can determine whether a dose or therapy course has been adequate for you.

What happens if the first choice is not enough

After checking diagnosis, adherence, treatment intensity and other factors, clinician and patient can choose among several next steps:

  • Continue and optimize the current therapy or medication if there is meaningful improvement and tolerable burden.
  • Switch to a different psychotherapy approach or therapist when the model, therapeutic relationship or practical format is not a good fit.
  • Switch or adjust medication under prescriber supervision, including a safe taper or cross-taper when appropriate.
  • Add psychotherapy to medication, or medication to psychotherapy, when one modality has produced only partial response.
  • Seek specialist assessment when depression is severe, recurrent, diagnostically uncertain, resistant to reasonable trials or complicated by bipolar features, psychosis, substance use or significant medical illness.

Shared decision-making continues after the first appointment. A failed or poorly tolerated trial is information about the next choice, not proof that treatment cannot work.

Planning after improvement: continuation and relapse prevention

Feeling better is not always the point at which treatment should stop. After full or partial remission, continuing an antidepressant, psychological therapy or both may reduce relapse risk. NICE identifies recurrent episodes, residual symptoms or incomplete response, severe depression, chronic mental or physical illness and ongoing social stressors as factors that increase that risk.

If you do not want to continue an antidepressant, discuss psychological relapse-prevention therapy rather than stopping without a plan. If medication is continued, schedule periodic reviews and agree in advance how long it will be maintained and how tapering would occur. The right continuation plan depends on episode history, current symptoms, adverse effects, preferences and safety.

Questions to bring to your appointment

  • How severe is my depression, and what findings support that assessment?
  • Have we screened for bipolar disorder, substance-related causes and relevant medical conditions?
  • What are the reasonable therapy, medication and combined options for me?
  • What benefits should we look for, and by when?
  • Which side effects or warning signs require an urgent call?
  • How will suicide risk be monitored, especially early in treatment?
  • When is the first follow-up, and what happens if improvement is partial?
  • If I improve, how will we decide whether to continue, taper or use relapse-prevention therapy?

Bottom line

Doctors choose antidepressants, therapy or both by balancing severity and urgency with safety, prior response, other health conditions, practical access and your informed preferences. APA supports offering either psychotherapy or a second-generation antidepressant initially for adults. NICE favors psychological or behavioral choices for less severe depression unless medication is your informed preference, while treating CBT, antidepressants and combined care as first-line possibilities for more severe depression. Expect an early review within two to four weeks, a fuller reassessment after roughly four weeks for medication or four to six weeks for therapy or combined care, and a change in plan when benefit, safety or feasibility requires it.
