Alternatives to Testosterone for Low Libido and Other Menopause Symptoms

The best alternative to testosterone depends on what is actually reducing sexual interest. Vaginal dryness or painful sex usually calls for local treatment; hot flashes and night sweats may need hormone therapy or a nonhormonal medicine; and persistent, distressing low desire without another explanation may warrant psychosexual care or a prescription such as flibanserin. A clinician should review symptoms, medicines, medical history and personal risks before treatment.

Testosterone is not a general menopause remedy. NICE advises considering it for menopause-associated low sexual desire only when hormone replacement therapy (HRT) alone has not worked, usually with specialist input. The options below address the specific symptom rather than assuming every form of low libido has the same cause.

Start by identifying what is driving the low desire

Menopause-related sexual problems often overlap. Burning, dryness or pain can make sex something a person avoids even when desire is otherwise intact. Hot flashes, disrupted sleep, anxiety, depression, medication side effects, relationship strain and other medical conditions can also reduce interest. Treating only “libido” may fail if one of these factors remains.

Assessment should include the timing of symptoms, menstrual and menopause status, bleeding history, current medicines, alcohol use, mood, sleep, relationship context and any pain with penetration. Persistent or unexplained bleeding after menopause needs medical assessment before or during treatment. Breast or endometrial cancer history, clotting or stroke risk, liver disease, uterus status and personal preferences affect which options are appropriate.

NICE recommends individualized discussion and menopause-specialist referral when HRT is contraindicated, choices are complex or the diagnosis is uncertain. A practical first question is whether the main problem is comfort, vasomotor symptoms such as hot flashes, or desire itself.

When dryness or painful sex is the main problem

Vaginal oestrogen

NICE recommends offering vaginal oestrogen for menopausal genitourinary symptoms, including for people who already use systemic HRT. Products include cream, gel, tablet, pessary and vaginal ring. The treatment acts locally, and systemic absorption is minimal compared with systemic HRT. Serious adverse effects are very rare, but the product and suitability still need to be reviewed with a clinician, particularly after hormone-sensitive cancer or unexplained bleeding.

Vaginal oestrogen is aimed at tissue symptoms such as dryness, irritation and pain with sex. It may make sexual activity comfortable again, but it is not a universal treatment for low desire caused by sleep, mood, medication or relationship factors.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Non-hormonal moisturisers and lubricants

If vaginal oestrogen is contraindicated or not preferred, NICE recommends non-hormonal moisturisers or lubricants. A moisturiser is used regularly to hydrate vaginal tissue; a lubricant is applied during sexual activity to reduce friction. Water-based products are commonly used for friction-related discomfort, while compatibility with condoms and any sensitivities should be checked on the label.

These products improve comfort rather than treating every cause of low libido. Stop and seek advice if a product causes burning, rash, recurrent symptoms or bleeding. A non-hormonal vaginal moisturiser for menopause can be a practical first self-care purchase when the main issue is ongoing dryness, followed by a lubricant for friction during sexual activity. Such products should be described as comfort measures, not guaranteed libido treatments.

Vaginal prasterone (DHEA)

Prasterone is a locally administered vaginal DHEA option. NICE says it can be considered when vaginal oestrogen or non-hormonal moisturisers and lubricants have been ineffective or are not tolerated. Evidence reviewed by NICE found improvement in dryness and pain with sex, but the rationale did not support prasterone as a cost-effective first-line choice. It is a prescription discussion, not a general over-the-counter libido treatment.

Ospemifene

Ospemifene is an oral option when locally applied treatment is impractical. The U.S. FDA label revised in February 2025 specifies 60 mg once daily with food for moderate-to-severe dyspareunia or vaginal dryness caused by menopausal vulvar and vaginal atrophy.

The label carries boxed warnings about endometrial cancer and cardiovascular disorders. It is contraindicated with undiagnosed abnormal genital bleeding and active or previous thromboembolism, among other restrictions. A clinician must check bleeding history, clot risk, medicines and other contraindications before prescribing it. Ospemifene targets vaginal atrophy symptoms; it should not be presented as a treatment for all forms of low sexual desire.

When hot flashes or night sweats are undermining desire

Menopausal hormone therapy

NICE recommends offering HRT for vasomotor symptoms and choosing dose, formulation and route through an individualized benefit–risk discussion. For someone with a uterus, combined oestrogen-and-progestogen HRT is generally used to protect the endometrium. After a total hysterectomy, oestrogen-only HRT is generally used, although the exact plan depends on the clinical history.

Improving hot flashes, sleep and general comfort can indirectly improve sexual interest. HRT is not automatically a treatment for isolated low desire, and it may be unsuitable for some people. Cancer history, unexplained bleeding, thromboembolism, stroke risk, liver disease and other conditions must be considered.

Menopause-specific cognitive behavioural therapy

NICE recommends considering menopause-specific CBT alongside HRT, or when HRT is contraindicated or declined. CBT can address hot-flash distress, sleep problems, anxiety and mood symptoms, and may be useful when these symptoms are suppressing sexual interest. It is not a substitute for evaluating pain, medication effects or a separate sexual-health condition.

Fezolinetant

Fezolinetant (Veozah) is a nonhormonal prescription for moderate-to-severe menopausal hot flashes. It does not treat vaginal dryness directly and is not approved as a general libido medicine.

The FDA warns of rare serious liver injury. Liver tests are recommended before starting, monthly during the first three months, and again at months 6 and 9. A person taking it should stop the medicine and seek medical care for possible liver-injury symptoms such as yellowing of the skin or eyes, dark urine, unusual tiredness, nausea, vomiting, itching or right-upper-abdominal pain. The FDA’s clinical program randomized 1,028 postmenopausal participants in Trials 1 and 2 (1,022 received treatment) and 1,831 participants in Trial 3; those trial populations do not guarantee an individual response.

When low desire persists without a pain or symptom explanation

Psychosexual, relationship and behavioral care

Low desire should be assessed in context rather than reduced to a hormone level. Counseling, sex therapy, couples therapy or CBT may help when distress, anxiety, depression, trauma, communication problems or relationship circumstances are contributing. A medication review is essential because some antidepressants, blood-pressure medicines and other drugs can affect sexual function. Treating depression, improving sleep or changing a troublesome medicine may be more useful than adding a libido drug.

Clinicians generally distinguish occasional low interest from acquired, generalized hypoactive sexual desire disorder (HSDD): a persistent reduction in desire that causes personal distress or interpersonal difficulty and is not better explained by another medical or psychiatric condition, a relationship problem or a medication. The distinction matters because a desire medicine is not intended to compensate for untreated pain, a medication adverse effect or a relationship problem.

Flibanserin (Addyi)

The FDA label revised in December 2025 indicates flibanserin for women younger than 65 with acquired, generalized HSDD that causes distress or interpersonal difficulty and is not due to another medical or psychiatric condition, relationship problems or a medication. The Office on Women’s Health describes FDA approval for women younger than 65 both before and after menopause.

The labeled dose is 100 mg at bedtime. Treatment should be stopped after eight weeks if there is no improvement. Flibanserin is not for improving sexual performance and is not a treatment for hot flashes or vaginal atrophy.

Its boxed warning concerns hypotension and syncope, with particular risk when combined with alcohol, moderate or strong CYP3A4 inhibitors or in people with hepatic impairment. Prescribers and pharmacists must review all medicines, alcohol use and liver history. Dizziness, sleepiness and fatigue can also affect safety, especially when driving or combining sedating substances.

How the main options differ

Option Primary target Hormonal exposure Key safety or monitoring issue
Vaginal oestrogen Dryness, irritation and painful sex Local oestrogen; minimal systemic absorption compared with systemic HRT Review bleeding and cancer history; serious adverse effects are very rare
Moisturiser or lubricant Dryness and friction-related discomfort Nonhormonal, local self-care Use regularly for moisturiser; during sex for lubricant; stop if irritation occurs
Vaginal prasterone Dryness and dyspareunia after first-line options fail or are not tolerated Local DHEA Prescription assessment; not considered cost-effective first line by NICE
Ospemifene Moderate-to-severe dyspareunia or vaginal dryness Systemic oral selective oestrogen-receptor activity Boxed warnings; contraindications include unexplained bleeding and active or prior thromboembolism
Systemic HRT Hot flashes, night sweats and other menopause symptoms Systemic oestrogen, with progestogen when indicated Individual benefit–risk review; regimen depends partly on uterus status
Menopause-specific CBT Hot-flash distress, sleep, mood and coping Nonhormonal Can be used with HRT or when HRT is unsuitable or declined
Fezolinetant Moderate-to-severe hot flashes Nonhormonal systemic prescription Liver tests before treatment, monthly for three months, then at months 6 and 9
Flibanserin Acquired, generalized HSDD in women under 65 Nonhormonal systemic prescription 100 mg at bedtime; stop after eight weeks without benefit; hypotension and syncope warnings

Route, monitoring and realistic expectations

Route is a useful way to compare choices. A moisturiser or lubricant is local, nonhormonal self-care. Vaginal oestrogen and prasterone are locally administered prescription treatments aimed at genital symptoms. Ospemifene, HRT, fezolinetant and flibanserin are systemic medicines, so the prescriber must consider whole-body contraindications, interactions and medical history.

Monitoring also differs. Vaginal products require attention to symptoms, irritation and any bleeding. Systemic HRT needs an individualized regimen and review of ongoing benefits and risks. Fezolinetant has a defined liver-testing schedule. Flibanserin requires review of alcohol, hepatic status and interacting medicines, with a stopping point at eight weeks if there is no improvement. There is no equivalent monitoring plan that makes an unregulated hormone preparation predictable or safe.

Speed and durability cannot be assumed to be identical across options. Comfort products are used at the time or frequency specified for their purpose; prescription treatments need consistent use and follow-up. Improvement in pain may remove one barrier to intimacy without creating spontaneous desire, while treating hot flashes may improve sleep without resolving relationship distress. Set a symptom-specific goal and reassess rather than judging every option by libido alone.

Cost, access and practical trade-offs

Nonhormonal moisturisers and lubricants are practical comfort options, but product availability, formulation and price differ by location. Ask a pharmacist about ingredients, condom compatibility and alternatives if burning occurs. Prescription access and insurance coverage for vaginal therapies, HRT, fezolinetant or flibanserin also vary by country and individual circumstances.

NICE’s rationale specifically says prasterone improved dryness and pain but was not cost-effective as a first-line option. That is a population-level consideration, not a judgment that it can never be useful. If first-line treatment is ineffective or not tolerated, a clinician can weigh expected benefit, safety, convenience and expense for the individual. The same principle applies to choosing an oral option when a locally applied treatment is impractical.

A practical decision path

  1. Describe the symptom precisely. Record whether the problem is absent interest, reduced arousal, pain, dryness, difficulty with orgasm, hot flashes, poor sleep or mood. Note when it began and whether it is generalized or situation-specific.
  2. Check for treatable contributors. Review medicines, alcohol, sleep, anxiety, depression, relationship stress, medical illnesses and pelvic or urinary symptoms. Ask about postmenopausal bleeding rather than assuming all bleeding is a menopause symptom.
  3. Relieve pain and dryness first when present. Discuss vaginal oestrogen. If it is unsuitable or unwanted, use a non-hormonal moisturiser regularly and lubricant during sex; consider prasterone if these approaches fail or are not tolerated. Ospemifene may be discussed when local treatment is impractical, after its boxed warnings and contraindications are reviewed.
  4. Match hot-flash treatment to risk and preference. Discuss HRT when appropriate, including the correct regimen for uterus status. Consider menopause-specific CBT with HRT or when HRT is declined or contraindicated. Fezolinetant is a nonhormonal prescription option requiring the specified liver monitoring.
  5. Evaluate persistent distressing low desire separately. Psychosexual or relationship therapy and medication review may address the cause. If the clinical picture meets acquired, generalized HSDD criteria and the person is under 65, discuss whether flibanserin is appropriate and review its alcohol, interaction and liver restrictions.
  6. Set a measurable goal. Track pain, dryness, sleep, hot-flash frequency, desire and distress separately. A treatment may succeed on one measure while another problem remains.
  7. Reassess the result. Lack of improvement should prompt a review of the diagnosis, adherence, side effects and other contributors—not automatic escalation to testosterone.

What to avoid or approach cautiously

Unregulated hormone preparations

NICE says the efficacy and safety of unregulated hormone preparations are unknown. Complementary preparations can vary in constituents, quality and purity, and interactions with medicines have been reported. “Natural” does not establish a predictable dose or a lower risk.

Vaginal laser outside research

NICE advises not offering vaginal laser treatment for menopausal genitourinary symptoms unless it is part of a randomized controlled trial. Marketing claims should not replace evidence-based assessment and established treatments.

One product for every type of low libido

A vaginal comfort product cannot be expected to correct medication-related desire loss, depression or relationship distress. Conversely, a drug aimed at HSDD will not repair untreated painful sex, and a hot-flash medicine will not treat vaginal atrophy. Matching the intervention to the symptom is the central safety and effectiveness decision.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

Where testosterone fits

NICE’s wording is narrow: “Consider testosterone supplementation for people with low sexual desire associated with menopause if HRT alone is not effective.” This places testosterone after an HRT trial when clinically appropriate and does not make it a treatment for hot flashes, mood symptoms, vaginal pain or every form of low libido. Specialist assessment is important because dosing, formulation, monitoring and access vary by country and indication.

In the United States, the FDA’s 2026 announcement summarized 2020 utilization data showing about 41 million women aged 45–64, while about 2 million women aged 46–65 received a hormone-therapy prescription. Those figures describe population use, not what any individual should take, and they do not establish that testosterone or another hormone is suitable for a particular person.

Questions to take to a clinician

  • Is my main problem pain or dryness, hot flashes and sleep, mood, or desire itself?
  • Could a medicine, medical condition, depression, anxiety or relationship issue be contributing?
  • Do I need assessment for postmenopausal bleeding before starting treatment?
  • Do my uterus status, cancer history, clot or stroke risk, liver disease or current medicines change the safest option?
  • Would a local treatment be sufficient, or do I need a systemic medicine?
  • If considering flibanserin, do I meet the HSDD criteria, and can I avoid the alcohol and interacting medicines covered by its warning?
  • If considering fezolinetant, can I complete the required liver testing and recognize symptoms that require stopping treatment?
  • When should we reassess benefit, side effects and whether a specialist referral is needed?

Frequently Asked Questions

What can I use instead of testosterone for low libido after menopause?

First identify the cause. Treat vaginal dryness or painful sex with vaginal oestrogen, a nonhormonal moisturiser or lubricant, and sometimes prasterone or ospemifene. Address hot flashes, poor sleep or mood with appropriate HRT, menopause-specific CBT or another clinician-selected treatment. Persistent acquired, generalized HSDD in women younger than 65 may lead to a discussion of flibanserin.

Is there a nonhormonal treatment for menopause-related low sex drive?

Yes, depending on the cause. CBT, counseling, sex therapy, couples therapy and medication review are nonhormonal approaches. Flibanserin is a nonhormonal prescription option only for the specific FDA-labeled HSDD population; it does not treat pain, vaginal atrophy or hot flashes.

What helps painful sex and vaginal dryness?

NICE recommends vaginal oestrogen, including for people already using systemic HRT. If it is unsuitable or not preferred, use a regular nonhormonal moisturiser and lubricant during sexual activity. Prasterone can be considered when those options fail or are not tolerated; ospemifene is an oral option when local treatment is impractical.

Can flibanserin be used after menopause?

The FDA label revised in December 2025 indicates flibanserin for women younger than 65, including women before or after menopause, with acquired, generalized HSDD that causes distress and is not explained by another condition, relationship problem or medication. It is taken at bedtime, and treatment is stopped after eight weeks without improvement.

Can prasterone or ospemifene improve libido?

They are aimed primarily at menopausal vaginal dryness, vulvar or vaginal atrophy and pain with sex. Improving comfort may remove a barrier to intimacy, but neither should be presented as a universal treatment for low desire caused by sleep, mood, medication or relationship factors.

What can I take for hot flashes if HRT is not suitable?

Menopause-specific CBT can be considered when HRT is declined or contraindicated. Fezolinetant is a nonhormonal prescription for moderate-to-severe hot flashes, but it requires liver tests before treatment, monthly for the first three months, and at months 6 and 9.

Is vaginal oestrogen safe with systemic HRT?

NICE recommends offering vaginal oestrogen for genitourinary symptoms even to people already using systemic HRT. Systemic absorption is minimal compared with systemic HRT, but bleeding, cancer history and individual contraindications still require clinical review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I buy an unregulated hormone or vaginal laser treatment?

NICE says the safety and effectiveness of unregulated hormone preparations are unknown and advises against vaginal laser treatment for menopausal genitourinary symptoms except in a randomized controlled trial. Product marketing is not a substitute for assessment and established treatments.

The Bottom Line

The safest alternative to testosterone is the one matched to the actual problem. Use local vaginal care for dryness or painful sex, treat hot flashes and sleep disruption with an individualized HRT, CBT or nonhormonal plan, and evaluate persistent distressing low desire as a separate sexual-health concern. Flibanserin, fezolinetant, prasterone and ospemifene are prescription discussions with distinct eligibility, contraindications and monitoring. Do not self-treat unexplained bleeding or rely on unregulated hormones; ask a clinician or menopause specialist to review risks and follow-up.
