Akiram Therapeutics says its investigational radiopharmaceutical AKIR001 produced no observed dose-limiting toxicities (DLTs) among the first eight patients treated in a Phase I study. In the company’s October 13, 2026 update, treatment-related adverse events were limited to grades 1–2, and four patients received another administration. These are preliminary sponsor-reported findings from a small early-stage study—not proof that the treatment is safe or effective.
What is AKIR001?
AKIR001, also styled [177Lu]Lu-AKIR001, is an investigational radiopharmaceutical. It combines lutetium-177, a therapeutic radioactive isotope, with an antibody directed at CD44v6, a cell-surface target. The approach is intended to deliver radiation to tumors expressing CD44v6.

The study is for people with irresectable or metastatic CD44v6-positive solid tumors who have no remaining standard treatment options, according to Akiram and the Karolinska University Hospital study record. The record describes a prospective, open-label, first-in-human Phase I study assessing safety, tolerability, biodistribution and anti-tumor effect.
What did the first eight patients experience?
In its October 13, 2026 announcement, Akiram reported that eight patients had been treated by the data cutoff. The company reported zero observed DLTs and said treatment-related adverse events were grade 1 or 2. Four patients had received repeat treatment after their first administration.
#1 Best Overall
A DLT is a treatment side effect serious enough, under a study’s protocol-defined criteria and observation period, to limit further dose escalation. Reporting no DLTs does not mean patients had no side effects: Akiram did report treatment-related adverse events. Nor does it establish that the treatment is safe for a broader population. The announcement is an early safety snapshot, not a completed assessment.
What do the imaging and biomarker observations suggest?
Akiram said quantitative imaging showed sustained retention of the radiopharmaceutical in tumors, with clearance from blood and normal tissues. The company also reported that low CD44v6 expression measured by immunohistochemistry was consistently associated with low absorbed tumor doses.
Rank #2
These are preliminary observations. They do not establish a validated diagnostic test, a threshold for selecting patients, or clinical benefit from selecting patients by CD44v6 expression. The announcement did not report an established efficacy result or a controlled comparison.

The Karolinska record describes an infusion followed by six weeks of safety follow-up, which may be extended to twelve weeks. Akiram has described the trial as an ongoing dose-escalation study at Karolinska University Hospital, which it identifies as the sponsor. In a May 13, 2026 update, the company said a safety review permitted progression to cohort 3 and that no DLTs had been observed in the cohorts evaluated at that point.
Rank #3
Early dose-escalation studies are designed primarily to assess tolerability and gather information such as biodistribution; they are not, on their own, designed to establish whether a treatment improves outcomes. The eight-patient update does not provide detailed patient characteristics, activity levels, dose-by-dose results, or longer-term follow-up.
What happens next?
Akiram said the preliminary results were selected as a Top Rated Oral Presentation at EANM 2026 in Vienna, scheduled for October 17–21. The company listed the AKIR001 presentation for October 18 as OP-172, with Joachim Nilsson named as speaker. That is a scheduled presentation; the announcement does not itself provide the underlying abstract or a full clinical dataset.
Rank #4
Akiram CEO Marika Nestor described the findings as an “important milestone” and said the company was encouraged by the safety profile, tumor retention and repeat treatment reported so far. Those comments reflect the sponsor’s interpretation of preliminary results, not an independent efficacy conclusion.
What the results do—and do not—show
- Reported so far: Eight patients treated at the data cutoff, no observed DLTs, treatment-related adverse events limited to grades 1–2, and repeat treatment in four patients, according to Akiram’s October 13, 2026 announcement.
- Still unresolved: Whether the treatment benefits patients, how its safety profile holds up in more people and with longer follow-up, and whether CD44v6 expression can guide patient selection in a clinically validated way.
The results are preliminary and sponsor-reported. They should be read as an early update from a small first-in-human study, not as evidence that AKIR001 is an established treatment or is available for routine care.
