Yes—but only within a controlled development workspace. An AI coding agent can be allowed to revise code and run tests after a failed attempt. That permission should not automatically extend to merging changes, deploying software, accessing production credentials, or altering infrastructure. A human should own each change and approve it before production.
What does “fix their own mistakes” mean?
It can describe several different actions, and they carry different levels of risk. Permission to edit a local patch is not permission to release it.

- Propose a repair: explain or draft a possible change.
- Edit and test: modify files in a scoped workspace and run permitted checks.
- Submit a change: commit the work or open a pull request for review.
- Merge or deploy: change the shared codebase or production system.
Routine edits and test runs can be useful forms of bounded autonomy. Merge and deployment should be separate approval decisions, especially when a change affects security, sensitive data, infrastructure, dependencies, or external services.
Why allow bounded self-correction?
Code generation is iterative: a first attempt may fail a test, miss a requirement, or introduce a defect. Letting an agent inspect that feedback, adjust its patch, and rerun checks can make the workflow more useful than requiring a person to intervene after every failed test. The value is in a quicker repair loop—not in assuming the agent’s next attempt is correct.

There is no established general success rate for coding-agent repairs in the guidance cited here. Treat each proposed change as software that must meet the same review and security expectations as other code.
Set permissions by risk and reversibility
OpenAI’s description of Codex controls calls the sandbox the technical execution boundary; its guidance also distinguishes routine work within constrained boundaries from higher-risk actions requiring explicit handling. Microsoft’s agent security guidance recommends least privilege, least action, and deterministic blocking for prohibited actions. Together, these principles point to a practical rule: the more difficult an action is to reverse, or the larger its potential impact, the stronger the approval gate should be.

| Action | Reasonable default | Why the gate changes |
|---|---|---|
| Suggest a patch | Allow, with the proposal visible to the developer. | No code is changed until someone chooses to apply it. |
| Edit files and run tests in a scoped workspace | Allow within defined paths, tools, and resource limits. | Changes are inspectable and can usually be discarded or revised. |
| Commit or open a pull request | Allow only under repository policy; preserve a clear human owner. | The change becomes part of a shared review process, but is not yet released. |
| Merge, deploy, or change production systems | Require qualified human review and explicit approval. | Errors may affect users, data, availability, security, or cost and can be harder to undo. |
This is a policy framework, not a universal numeric threshold. Teams should set boundaries for their systems and threat model rather than infer that a successful test run authorizes every later action.

Build a repair loop that is safe to review
- Define the task boundary. Specify the goal, allowed files and tools, and actions the agent must not take. Microsoft warns that agents can misunderstand intent or pursue an inferred objective; use explicit policy and deterministic controls to block prohibited actions.
- Constrain execution. Run the agent in a sandboxed or otherwise isolated workspace, restrict credentials to the minimum necessary, and limit network access. VS Code describes sandboxing as an additional security layer, not a guarantee; isolation can be weakened by configuration or access paths.
- Keep feedback in scope. Let the agent use relevant test output and diagnostics, but treat instructions found in untrusted files, web content, or other inputs as untrusted. Prompt injection can influence tool use, so preserve trust boundaries and scrutinize where instructions come from.
- Require evidence for the change. Review the diff, run appropriate tests and security checks, and verify that the repair addresses the reported problem without adding unrelated changes. A passing test is evidence, not proof of correctness.
- Approve release separately. Assign a human owner who can assess the change and approve it before merge or production deployment. The UK Home Office engineering guidance says AI-assisted outputs “MUST be reviewed and approved by a human before reaching production” and that AI-assisted code “MUST meet the same security expectations as human-written code.”
- Keep an audit trail. Record the request, files changed, commands run, test results, approvals, and final disposition. OWASP recommends a named human owner, explicit approval before merge, and audit trails; its guidance also highlights risks from tools and dependencies such as extensions and MCP servers.
What sandboxing and automated review can—and cannot—do
A sandbox can reduce the damage an agent’s commands might cause by limiting what it can reach. It does not establish that the patch is correct, secure, or appropriate. Keep code review and testing as independent controls, and check the sandbox configuration rather than treating its presence as a blanket assurance.
Automated approval review can reduce interruptions by escalating some actions rather than every action. OpenAI’s 2026 account of its internal deployment says Codex sessions in Auto-review stopped for human approval “roughly 200x less often” than under manual approval. OpenAI also reported an approval rate of “around 99%” for the small fraction of actions it reviewed, and described a snapshot of 720 out-of-sandbox actions, of which seven were rejected: four continued by a safer route and three stopped for user input. These are vendor-reported internal deployment figures, not independent benchmarks of repair quality or proof that automated review can catch every risk. OpenAI explicitly notes that Auto-review does not address all in-boundary or concealed behavior.
For that reason, automated escalation should complement—not replace—testing, human accountability, monitoring, and a way to interrupt or recover from an agent’s work.

Who is accountable for an AI-assisted change?
A person or team should be named as owner before the agent starts. That owner is responsible for deciding whether the proposed repair meets the task, whether its tests are adequate, and whether the change is safe to approve. The agent’s ability to produce a diff or pass checks does not transfer that responsibility.
Review the surrounding system as well as the patch itself. Dependencies, extensions, agent tools, and MCP servers may introduce supply-chain risk; restrict and audit them as part of the same security boundary.
