ACH Return Codes R01–R85: How to Classify and Handle Returns in Production

ACH return codes describe why an entry came back, but code labels are not complete instructions. Learn how to classify common returns, handle authorization claims, and apply the right timing and controls.

ACH return codes tell you why a receiving institution sent an entry back, but the code label alone is not enough to decide what to do next. For production handling, match each return to the current Nacha Operating Rules Appendix Four entry, then apply its initiator, timing, account-type, statement, and note requirements. R01–R04 cover common account or funds issues; R10 and R11 require different authorization handling; and specialized codes such as R84 and R85 call for their own rule details.

What an ACH return code means

An ACH return is an entry sent back because the receiving institution could not accept it. The return code describes the reason, but it does not by itself establish whether an originator may retry, what information must be corrected, who may initiate the return, or how soon it must be sent.

Appendix Four of the Nacha Operating Rules is the controlling code-by-code reference. Its entries include the code and title, description, initiating party, return type, account type, timeframe, written-statement requirement, cross-reference, and notes. The 2025 Basic Edition includes R01 through R85; check the current rules and any amendments before implementing a code-specific decision, because an edition or amendment may change the applicable requirements.

Do not treat a short code description as a complete operating instruction. A reliable interpretation uses the full Appendix Four row, including conditions and cross-references.

Common codes and distinctions that change the response

Code What it signals Operational implication
R01 Insufficient funds Review the cause and the applicable retry conditions; do not treat a returned entry as permission for unlimited resubmission.
R02 Account closed Stop entries to that account. Obtain authorization for a different account before sending entries there.
R03 No account or receiver cannot be located Obtain corrected receiver account information before attempting a corrected entry.
R04 Invalid account number structure Correct the account number or its structure before another attempt.
R10 Unknown or unauthorized originator Handle as an authorization claim; do not merge it analytically with R11.
R11 Entry was authorized but did not meet the authorization terms Investigate the specific processing or terms problem. A correctable error may call for a different response from an unauthorized-originator claim.
R84 Certain outbound IAT gateway returns Use the code’s complete Appendix Four description and notes; the short label is not a sufficient handling rule.
R85 Incorrectly coded outbound international payment Consult the full rule entry and related requirements before correcting or resubmitting.

The common-code actions above reflect practical originator guidance, not a substitute for the governing rule. For R01 and R09, a 2026 Wespay guide says a new entry may be supported within 180 days of original settlement, subject to the applicable conditions. Verify the current rule and code-specific requirements before using that period in a system or procedure.

R10 is not the same as R11

Nacha distinguishes an unknown or unauthorized originator claim (R10) from an entry that was authorized but failed to comply with the authorization terms (R11). R11 can involve an incorrect amount, settlement earlier than agreed, an incomplete transaction, or improper reinitiation. In some cases, correcting the entry may address the problem; other circumstances cannot be fixed that way. Certain ARC, BOC, or POP source-document issues use distinct codes, so do not collapse them into a generic unauthorized bucket.

Keep the distinction in return analytics, agent scripts, and case routing. It can affect whether a team investigates an existing authorization and processing error or treats the matter as a claim that the originator was unknown or unauthorized.

How to handle returns in production

The following is a recommended operational design synthesized from Appendix Four’s fields and originator guidance; it is not a verbatim Nacha-mandated sequence.

  1. Ingest and correlate. Capture the return code and connect it to the original entry, trace information, account context, and settlement date. Preserve the original record rather than overwriting it.
  2. Classify against the current rule. Retrieve the complete Appendix Four row, including its notes and cross-references. Route authorization claims, account-status problems, IAT cases, and other specialized returns to the appropriate workflow.
  3. Validate conditions and timing. Check who may initiate the return, the account type, applicable timeframe, statement requirements, and any code-specific conditions. Record the relevant dates and the calendar used to compute a deadline.
  4. Choose a permitted action. Stop entries, correct data, obtain authorization, or consider a new entry only when the code-specific rules and facts allow it. Do not automatically retry an unchanged entry.
  5. Notify the right owner. Send the event to the responsible operations, customer-service, risk, or compliance team, and communicate with the customer where the process requires it.
  6. Monitor and retain evidence. Preserve the return, decision, correction, approvals, and any relevant authorization or statement records. Feed the event into return-rate and risk monitoring.

Apply different remedies to different account problems

For a closed account (R02), stop entries to that account and obtain authorization for another account before changing the destination. For R03, obtain corrected receiver account information; for R04, correct the account number or its structure. The 2026 Wespay guide also identifies a Written Statement of Unauthorized Debit obtained by the RDFI for a consumer R05 return. Confirm the specific rule conditions before building any of these actions into a general-purpose retry policy.

Return deadlines are code-specific

Many returns use a two-Banking-Day rule, but that is not a universal deadline for every return. Under the general Appendix Four timing note, the return must reach the RDFI’s ACH Operator by its deposit deadline so it is available to the ODFI by opening of business on the second Banking Day after the original settlement date. Other entries and circumstances have different timing markers; the Appendix Four material also includes a sixteen-calendar-day marker. Apply the exact code row and relevant conditions rather than assigning one deadline to all returns.

Effective January 1, 2026, Nacha clarified that references to Banking Days in the Operating Rules mean days when the ACH Network is open for business—the ACH Operator’s Banking Day. This matters when an institution’s own holiday schedule differs from the network calendar. Build deadline calculations around the applicable ACH Network calendar and the rule’s precise timing language.

Use return-rate levels as monitoring signals

Nacha’s risk guidance identifies R02, R03, and R04 with the Administrative Return Rate Level. It identifies debit return reason codes, subject to stated exclusions, with the Overall Return Rate Level. Exceeding a Return Rate Level can support a preliminary inquiry into origination activity; it is not automatically a Nacha Rules violation. Teams should confirm the applicable denominator, measurement period, exclusions, and current rules with their ODFI or compliance contact.

A separate historical figure should not be confused with those monitoring levels: Nacha’s 2015 risk-rule materials stated a 0.5% Unauthorized Return Rate Threshold for specified unauthorized debit codes. That historical figure is not a threshold for all return codes and should not be treated as current guidance without checking the governing rules.

A return is not a reversal

A return is sent back because the receiving institution could not accept an ACH payment. A reversal is initiated by the sender to correct a qualifying genuine payment error. Nacha’s August 2026 guidance says reversals must be transmitted within five Banking Days of original settlement and include the original SEC code, Company/Originator Identification, and transaction amount. Reversals cannot be used to recover funds over suspected fraud or cancel a legitimate payment.

Because a reversal can affect a customer or counterparty account, Nacha recommends controls such as dual approval, pre-transmission validation, payroll and vendor review, duplicate detection, and fraud monitoring. Use the return process when an entry is returned; do not use a reversal as a substitute for a return or as a general-purpose cancellation.

Build the code reference into your workflow

For each code your operation handles, maintain a mapping to the current Appendix Four row and capture the fields that drive the decision: initiator, return type, account type, timeframe, statement requirement, cross-reference, and notes. Version that mapping alongside your rule updates, and make it easy for staff to see the difference between a rule requirement and an internal operational recommendation.

Do not attempt to reconstruct all R01–R85 requirements from abbreviated labels or a partial list. The appendix contains specialized and contested cases as well as common account and authorization returns; using an incomplete mapping can produce a wrong deadline, an invalid retry, or mishandling of a claim.
