Free booking form templates for websites range from simple appointment request forms to live scheduling forms that show available time slots. The right choice depends on whether you want to review and confirm requests yourself, let visitors book a currently available slot, or build a complete booking-enabled website. The examples below cover those use cases; template availability and features can vary by provider and plan.
Choose the kind of booking flow you need
Before choosing a design, decide what should happen after a visitor submits the form. An appointment request collects preferred dates and service details for your team to review. A live scheduling form displays availability so the visitor can select a slot. A booking-enabled website combines scheduling with the surrounding site and business tools.

- Request: The visitor states a preference; staff confirm or propose another time.
- Live booking: The visitor chooses from slots presented as available by the scheduling system.
- Booking website: The website and booking flow are designed to work together, potentially including availability, payments, and business management.
A form that asks for a date is not necessarily connected to a live calendar. Check the behavior, not just the template name.

Use an appointment request form when availability needs staff review, the service requires an intake conversation, or you do not want a visitor to confirm a slot automatically. Jotform’s Appointment Request Form is a customizable example. Its suggested fields include contact details, preferred and alternate dates or times, requested services, and marketing preference. The form can be embedded on a website or shared by email or social link. Jotform lists hospitals, photographers, and trainers among potential users; healthcare providers should assess applicable privacy, security, and legal requirements for their jurisdiction and data before collecting sensitive information.

Keep the form focused: request the information needed to assess and respond to the appointment, and make clear that submission is a request rather than a confirmed booking if that is how the business operates.
Calendar and time-slot booking forms
If visitors should choose an available slot themselves, look for a booking form with calendar or time-slot controls. - Medical appointments: A medical appointment template can provide a starting point for requests or scheduling. Do not treat a generic template as proof that the form or its integrations meet the privacy, security, or legal requirements that apply to the data and jurisdiction.
- Tutoring and lessons: Ask for the subject or service, student or client contact details, and preferred times. Confirm whether the template supports recurring sessions if those are needed.
- Salon and beauty appointments: Include the service and, where relevant, staff preference. Check that duration and availability reflect the selected service.
- Photography and tattoo bookings: A request flow can collect the project or service type and preferred dates for follow-up. If you use an intake field or upload, collect only what is necessary and check storage and access controls.
- Car service: A request form can gather the service sought and preferred appointment time. A request alone does not manage workshop capacity or confirm a bay.
Jotform’s booking form catalog includes examples such as tattoo, tutor, photography, and time-slot forms; its appointment-request materials include medical examples. Catalog contents can change, and the existence of a template does not establish that every version is free or offers the same functions.
Hotel, bed-and-breakfast, and reservation forms
Jotform’s booking catalog also lists hotel and bed-and-breakfast form examples. A basic reservation request can collect guest contact details, requested dates, party size, and preferences, then send the details to staff to confirm. That is different from a system that maintains room inventory and confirms reservations against it.

If guests need to see current room availability or pay online, verify that the chosen product supports inventory, reservation management, and the required payment flow. A request form by itself does not establish those capabilities.
Meetings and consultations
For consultations, sales calls, or remote meetings, a scheduling form can capture attendee details and arrange a conversation. Jotform describes appointment and Zoom meeting templates, as well as reminders and follow-up emails on its scheduling page. Check the chosen form’s connection to the meeting service and calendar, and confirm what confirmation or reminder messages the selected plan supports.

Booking-enabled website templates
If you need a website as well as a form, Wix describes hundreds of free booking website templates covering areas including beauty and wellness, fitness, health, education, tours and activities, and business professionals. Its page describes built-in booking functionality, real-time availability, bookings through multiple channels, payments, and business management. See Wix booking website templates.
This option is different from embedding a standalone form into a site you already have. Check the plan and feature requirements for the specific booking tools, payments, and business functions you intend to use; a template being described as free does not mean every associated feature is included at no cost.
How to compare free booking form templates

Set up the form for your website
- Define the outcome. Decide whether a submission is a request awaiting confirmation or a booked slot.
- Choose a suitable starting template. Select an appointment request, calendar or time-slot form, industry-specific form, reservation form, meeting form, or booking-enabled website based on the flow above.
- Trim and configure the questions. Keep only the fields needed to schedule and deliver the service. Add branching only when it helps visitors see questions relevant to their choices.
- Connect the calendar and notifications, if needed. Verify the actual account, integration, and plan requirements. Test time zones, buffers, and overlapping bookings rather than assuming the template handles them automatically.
- Test the visitor experience. Submit a test request or booking on the page where it will appear. Check what the visitor sees next, what staff receive, and whether the slot or request is represented correctly in the calendar.
- Publish with accurate expectations. State whether the visitor has a confirmed appointment or is waiting for the business to confirm. Review the provider’s current free-plan limits and privacy obligations before collecting data.
What “free” does and does not tell you
Providers may label a template or website design free while associated capabilities depend on the selected product, account, or plan. The cited product pages establish that these template types and described features are offered, but they do not establish that every example is currently free with unlimited use or that all integrations, payment options, and controls are included. Check the live provider terms for the exact template and workflow you plan to publish.
