Not necessarily. A refusal message alone does not prove an action was blocked: the tool may have been paused for approval, denied before execution, skipped by a guardrail, or already run. Check the agent’s execution record and the affected system’s state to find out what happened.
Why a refusal message is not enough
An agent’s final text and its tool execution are separate evidence. “I can’t do that” tells you what the agent said; by itself, it does not show whether a tool call was made, stopped, or completed. The key is to trace the call through the application’s control flow and then verify the intended change in the system that would have been affected.

There is no universal cross-platform log format for doing this. Look for the relevant approval or denial event, the tool call’s execution result, and—when the action matters—the resulting application state.

The call paused for approval
In the OpenAI Agents SDK human-review flow, a tool call that requires approval creates an interruption. The application examines the pending item, approves or rejects it, and resumes the same run from its saved state. The call is held pending that decision; a refusal-like message should not be mistaken for proof that the run ended. OpenAI documents that this flow also applies to tools reached through handoffs and nested agent-as-tool calls. OpenAI Agents SDK: Guardrails and human review.

A guardrail rejected the call before execution
The SDK’s input tool guardrails provide a separate control point for custom function tools. A guardrail can reject a call before the function executes, returning the rejection to the model. When approval is also required, the default input-guardrail timing is after approval and immediately before execution. The documentation also describes an option to run input guardrails before approval and again before execution. OpenAI Agents SDK: Guardrails and human review.
A platform policy denied it
Anthropic’s managed-agent permission policy defines allow, ask, and deny outcomes. Under the documented auto policy, the server may run a call, deny it, or pause for approval. For a denial through this feature, Anthropic says the tool does not run and the agent receives a rejection tool result. That guarantee applies to this documented managed-agent mechanism and configuration—not to every agent or every refusal message. Anthropic: Managed-agent permission policies.

Check whether the agent’s control covered that tool
A control only establishes something about calls it actually intercepts. OpenAI’s SDK documentation describes guardrails for custom function tools, but notes that guardrails do not run at every workflow layer or for every hosted or built-in tool type. A guardrail attached to one function therefore should not be assumed to protect every action in a multi-agent workflow. OpenAI Agents SDK: Guardrails and human review.
When reviewing a trace, identify the specific tool and control point. Was the decision made in an application wrapper, an SDK guardrail, or a platform-managed permission policy? Did it happen before execution, after a human decision, or only after a tool result? Which calls were covered—local functions, handoffs, nested agents, or hosted tools? Similar-looking refusal text can represent very different execution states.

How to verify whether the action took effect
- Find the exact proposed action. Identify the tool, arguments, and target record, file, or service in the run trace.
- Inspect the control event. Check whether the run shows an interruption awaiting approval, a rejection before execution, or an execution result. A pending approval is not the same as a completed denial.
- Confirm the tool’s execution outcome. Look for evidence that the function or server-side tool ran, rather than relying on the agent’s summary.
- Check the affected system. For a consequential action, inspect its system of record—for example, whether the intended record changed or the operation completed. This is a practical verification step; the cited platform documentation does not define one universal verification format.
Keep these pieces of evidence distinct: a policy decision shows what a control decided; an execution record shows whether a tool ran; the affected system shows the resulting state. If the trace is missing or ambiguous, the refusal message alone cannot settle the question.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What the documented controls can and cannot establish
| Control or state | What the documentation establishes | What it does not establish by itself |
|---|---|---|
| OpenAI Agents SDK human review | A tool call requiring review interrupts the run; the application can approve or reject the pending item and resume the same run. | That a refusal sentence means the call was rejected or that no other tool in the workflow ran. |
| OpenAI Agents SDK input tool guardrail | A guardrail can reject a custom function-tool call before execution; documented coverage has workflow and tool-type boundaries. | That every hosted, built-in, or multi-agent action is covered by the same guardrail. |
| Anthropic managed-agent permission policy | For the documented feature, a denied tool does not run and the agent receives a rejection result. | That every agent or refusal outside this mechanism has the same execution behavior. |
These distinctions reflect the platforms’ documented control flows: OpenAI Agents SDK documentation and Anthropic managed-agent permission policies. Neither source supplies a measured failure rate for agent refusals, so a general statistic about how often refusals fail to stop actions is not established here.
