Xbox certification failures most often point to problems with stability, title integrity, incomplete resubmission fixes, or connectivity handling. Start with the exact Xbox Requirement (XR) and test case named in the report, reproduce it under the reported conditions, fix the underlying issue, then verify the fix and every earlier Condition for Resubmission (CFR) before submitting again.
Which failures should you investigate first?
Microsoft’s version 3.0 top-failing-test-cases page, dated August 6, 2026, identifies title stability (XR-001) and title integrity (XR-003) as leading causes within the distribution of Xbox certification failures. Its figures are split by platform; they are not the percentage of all submissions that fail.

| Requirement | Xbox console failures | PC failures |
|---|---|---|
| XR-001: Title Stability | 38% | 34% |
| XR-003: Title Integrity | 14% | 4% |
These are Microsoft’s reported shares of certification failures for the corresponding test case and platform. They do not establish the odds that a particular submission will fail, or explain the cause of any one game’s finding. The title’s own test report is needed to diagnose a specific failure.

The figures are useful for deciding where to put readiness effort, not for replacing a report-led investigation. Stability is the largest of the listed categories on both platforms, while the reported share for title integrity is higher on console than PC. A team should still prioritize the actual findings for its own build: device applicability, severity, repeatability, player impact, and whether the issue blocks resubmission matter more than a broad ranking.

How to turn a certification finding into a fix
A useful remediation loop is to map the finding to its requirement and test case, reconstruct the test conditions, reproduce the player-visible result, fix its cause, and verify the corrected build. A defect that appears only with a fresh profile, no network connection, or after suspend and resume can be missed by a quick run through the main menu.
- Read the test report. Record the XR, exact test case, failure description, and whether the item is a CFR, Standard Reporting Issue (SRI), or Issue of Note (ION).
- Reconstruct the conditions. Note the platform and hardware, account or profile state, connectivity, game mode, DLC or feature being exercised, and any suspend/resume transition.
- Reproduce the reported behavior. Follow the test case on the relevant configuration. Keep the steps and observed result so the team can compare a later build against the same scenario.
- Correct the underlying defect. Verify that the expected behavior is restored from the player’s perspective, not merely that the particular test step no longer produces an error.
- Check packaging and validation. Run the current Submission Validator, resolve packaging issues, and include its logs with the submission.
- Regression-test the full failure list. Verify the new fix and all previously logged CFRs and BVT failures before resubmitting.
For a useful reproduction record, preserve the sequence of actions, the state in which the issue occurs, and what the game did instead of what was expected. Record the platform, relevant hardware, profile state, online or offline condition, feature or DLC, and any transition that precedes the result. This makes it easier to distinguish a defect that is reproducible from one that has not yet been triggered in the local test run. The sources do not prescribe a tracking tool or ticket format; the value lies in carrying each report item through reproduction, ownership, fix, and verification.
Microsoft’s Xbox Requirements page surfaced in this guidance is version 16.4, with a release entry dated September 8, 2026. The certification-tested console requirements page is version 16.4 dated August 11, 2026. Requirements generally apply across Xbox One, Xbox Series X|S, PC, and Xbox Cloud Gaming unless a requirement says otherwise; individual certification procedures and test cases may be device-specific. Check the current requirements and test-case pages when preparing an actual submission, because the documents can change.

XR-001: Crashes, hangs, data loss, and failed recovery
XR-001 covers prompt startup, continued responsiveness, graceful shutdown, and recovery from API exceptions. Microsoft’s test procedures exercise gameplay, menus, features, and downloadable content, and repeat checks with a new profile and while disconnected from Xbox services. A game can therefore appear stable in one familiar playthrough and still fail when tested in a different account or network state.
Recognize the failure patterns
- A crash, console reboot, or game that becomes unresponsive.
- Unintended loss of user data.
- A static, non-interactive pause lasting more than 20 seconds.
- A loading screen lasting more than two minutes without a progress indication, or more than three minutes with one.
- A failure during suspend and resume, such as a crash, hang, restart, loss of access to the last save, or failure to restore Xbox or partner-service connections.
These examples describe explicit test failure conditions, not recommended performance targets for every loading screen. In particular, the loading thresholds distinguish screens with and without a progress indication. Treat a progress indicator as useful feedback, not as permission for an indefinite wait.
Reproduce the state transition, not just the visible symptom
When the report points to a crash or freeze, replay the affected flow with the same menu, gameplay state, feature, or DLC. Then vary the test dimensions Microsoft exercises: use a new profile, disconnect from Xbox services, and test suspend/resume if relevant. For a recovery failure, verify the entire sequence: the game remains usable before suspension, resumes without restarting or hanging, retains access to the last save, and restores the needed service connections.
Keep the reproduction scenario consistent while diagnosing. Otherwise, a fix may appear successful simply because the triggering state was not recreated. Once the corrected case passes, revisit other previously failing areas; a stability change in one path can introduce regressions elsewhere.

XR-001 test coverage described by Microsoft includes navigating gameplay, menus, features, and DLC, and repeating checks with a new profile and without Xbox services. That breadth matters because a feature may rely on data, account state, or a service connection that is absent in another test condition. A startup pass alone cannot establish that shutdown, offline behavior, or recovery after suspension is also sound.

XR-003: Incomplete, untestable, or low-quality submissions
XR-003 requires Xbox games to meet Xbox quality standards and be fully functional and testable. It applies to the client code, submission artifacts, and downloadable content. Microsoft’s version 3.0 XR-003 page, dated November 1, 2025, states: “XBOX games must meet XBOX quality standards and be fully functional and testable.”
Check both the game and the submitted package
The submission package must pass the current Submission Validator, and the submission must include the validator logs. A clean local launch is not a substitute for validating the actual artifacts submitted for testing. Confirm that the package being checked is the same build and content set intended for certification.
Quality findings can involve crashes or freezes, unplayable frame rates, major hindrances to progression, graphical corruption, settings or controls that do not apply correctly, inaccessible menus, or unstable multiplayer. Diagnose the exact failure in context: for example, confirm whether the issue is tied to a particular control configuration, progression point, menu path, or multiplayer state, rather than relying on a general smoke test.
Think of XR-003 as a completeness and testability check as well as a quality check. A title may launch but still be difficult to certify if a menu cannot be accessed, a progression blocker prevents further play, or required downloadable content is not functional in the tested configuration. Likewise, a package that does not pass validation or arrives without its validator logs is not made ready by the fact that a developer can run a local build. Test the client, the intended submission artifacts, and the included content as the build will be presented.
Use BVT as a required gate
Build verification tests (BVTs) are a readiness suite that titles must pass before they enter full certification testing. All BVT failures must be fixed before resubmission. Microsoft identifies submitting a new build without fixing every previously logged CFR and/or BVT failure as a main cause of BVT failure.
Carry the prior report forward as part of the build checklist. Track each finding, its reproduction condition, the responsible owner, the code or content change, and the verification result. Microsoft does not prescribe a particular tracking tool or project-management format; the important operational point is that no old required fix is silently dropped when a new build is prepared.
BVT and full certification are related but not interchangeable stages: BVT is the readiness gate that comes first. A team that has prepared a new build should compare it with the full set of previously recorded failures before it is sent back, rather than assuming the latest change resolves all prior blockers. If a new submission still contains an old BVT failure or CFR, that unresolved issue can prevent the build from progressing as intended.

XR-074 requires games to handle Xbox and partner-service connectivity errors gracefully, follow Xbox retry policies for Xbox service requests, and tell players appropriately when services are unavailable. The game must not blame Xbox network trouble when the unavailable service is a partner service, and slow or intermittent network services must not make it crash or hang. Microsoft’s XR-074 requirement, version 2.0 dated November 1, 2025, states: “Titles must gracefully handle errors with XBOX and partner services connectivity.”

Test degraded service behavior
Check how the game behaves when a request is slow, intermittent, or unavailable, including what the player sees and whether gameplay remains stable. The correct response depends on the feature, but Microsoft’s guidance calls for responding to network-operation performance or using Windows.Xbox.Networking network-quality APIs, then degrading gracefully. Examples include transferring less data, choosing appropriate user roles, or using cloud services.
Make the player-facing message match the failure. If a partner service is unavailable, do not present it as an Xbox network outage. For Xbox service requests, follow Xbox retry policy rather than applying an arbitrary retry loop. Confirm that timeouts and retries leave the game responsive instead of producing a crash, hang, or misleading state.
In practice, verify the distinct conditions that the requirement addresses: a service that is unavailable, one that responds slowly, and one that is intermittent. They can create different player experiences, but none should leave the game crashed or hung. The correct fallback varies with the feature; the supported examples are reducing transferred data, choosing appropriate user roles, or using cloud services. The requirement is graceful handling, not a single universal fallback design.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsValidate required partner dependencies in the submission configuration
If a partner service is required for the game to run, the broader Xbox requirements say that service must be operating and configured when the title is submitted. Verify the dependency and credentials in the certification configuration; a developer-only environment can conceal a missing configuration or a service that certification cannot reach.
This is a separate readiness check from handling an outage gracefully. A game still needs resilient behavior when connectivity degrades, but a service required for operation must also be available and configured for submission. Check that the certification environment uses the expected partner-service setup and credentials instead of assuming that a successful developer environment proves the submission configuration is ready.
What CFR, SRI, and ION mean for your next build
Microsoft’s Failure Mode Analysis (FMA) scores an issue for severity, probability, and repeatability, then classifies it as a CFR, SRI, or ION. A CFR prevents shipping until it is fixed in a resubmission or addressed through an allowed content update within the specified timeframe. Prioritize CFRs because they block shipment, while retaining and addressing every logged issue required for the next test pass.
The classification does not replace reproducing and understanding the finding. Use it to order the work, then use the report’s XR and test case to identify the condition that must be corrected. If an item’s disposition or permitted handling is unclear, rely on the report and applicable Microsoft submission guidance rather than assuming that a lower-severity label means it can be ignored.
For triage, keep two questions separate: what is the player-facing failure, and what does its classification require from the next build? Severity, probability, and repeatability inform FMA, while the XR and test case give the technical condition to investigate. Whether a finding blocks shipment is a different axis from whether it is easy to reproduce or limited to a particular device. A CFR should lead the remediation queue, but teams should preserve other logged findings so they do not disappear between test passes.

Test on the hardware and environment that certification uses
Final console submissions are tested on retail Xbox consoles, and device-specific cases use equivalent retail hardware. Optional submissions use development kits in the CERT.DEBUG sandbox. A team can use retail console hardware for its own pre-submission checks, but owning a retail console does not replace the GDK, certification access, required submission artifacts, or functioning partner services.
Match pre-submission verification to the relevant device-specific case and environment as closely as possible. A pass on a development setup is useful, but it does not by itself establish that a retail-hardware certification case will pass, particularly when the report concerns device behavior or service configuration.
Use the report to decide which platform and device conditions matter; requirements generally span Xbox One, Xbox Series X|S, PC, and Xbox Cloud Gaming unless an individual requirement says otherwise, but test procedures can be device-specific. A retail console can support local pre-submission checks when a team needs them. It is not a guarantee of certification and does not supply developer access, the GDK, a validated package, or working services. The same principle applies to any pre-submission environment: it should match the relevant test, but it cannot stand in for the submission process itself.

A resubmission can fail when a team fixes the most visible new finding but leaves a prior CFR or BVT defect unresolved. Another risk is verifying only the individual code path that changed, without checking the original conditions or the rest of the reported failure list. Package changes can also leave the submitted build inconsistent with the one that was locally validated.
- Use the prior report as a mandatory release checklist, not just a reference for the latest code change.
- Reproduce each failure using its stated account, connectivity, mode, and transition conditions.
- Run the current Submission Validator on the intended artifacts and attach its logs.
- Recheck each earlier CFR and BVT failure against the corrected build.
- Confirm that required partner services and credentials are available in the certification configuration.
The exact cause and fix for a particular title cannot be determined from a general list of common failures. That diagnosis depends on the title’s report, build, platform configuration, and reproducible steps.
Before resubmitting, compare the candidate build against the prior report item by item. Record which test condition was repeated, what result occurred, and whether the fix is present in the exact artifacts being submitted. Then run the current validator and keep its logs with the submission. This closes the gap between “the developer says the fix worked” and evidence that the corrected package passed the relevant check. If the build or content changed after validation, validate the intended submission artifacts rather than relying on results from a different package.
Frequently asked questions
Why did my Xbox game fail BVT?
BVT failures can result from submitting a new build without fixing all previously logged CFRs and BVT failures. Compare the current report with the prior one, reproduce each remaining condition, and verify each fix before resubmitting.
What Xbox certification failures should I check first?
Microsoft’s August 6, 2026 top-failing-test-cases page reports XR-001 title stability at 38% of console failures and 34% of PC failures, and XR-003 title integrity at 14% and 4%, respectively. These are shares of certification failures by test case and platform, not the percentage of all submissions that fail.
Best Value
Does passing the Submission Validator guarantee certification?
No. The submission package must pass the current validator and include its logs, but certification also tests whether the title is functional, stable, and meets the applicable requirements. Validator success does not establish that test-case failures such as crashes, inaccessible menus, or connectivity problems have been fixed.
Can I use a retail Xbox console to test before submission?
Yes. Retail Xbox hardware can be useful for a team’s own pre-submission checks, and final console certification cases use retail consoles. Owning a console does not replace the GDK, certification access, submission artifacts, or functioning partner services.
What should the game do when a partner service is unavailable?
Handle the error gracefully, keep the game responsive, and show an appropriate message that does not misidentify the problem as Xbox network trouble. Required partner services must also be operating and configured when the title is submitted.
