Six review rounds can leave every verdict untouched and still make a technical comparison more dependable. In Mahiro Hirakawa’s account, the rounds uncovered misleading wording, ambiguous counts, and citation ranges that did not fit their claims. The verdicts stayed the same; the evidence became easier to check.
What “12 of 3” meant—and why it mattered
Hirakawa describes a ledger row marked probes=12/3. It did not mean twelve divided by three, or a ratio. It meant twelve probes had run and three conditions had been declared. The slash made two separate counts look like a relationship between them.

As Hirakawa puts it, “A count and a ratio look identical and mean different things.” The practical fix is to name each quantity and identify its source instead of depending on a reader to infer meaning from punctuation. For example, a row should distinguish “probes run: 12” from “conditions declared: 3,” if those are the values being reported.
Why the verdicts did not change
The review rounds found defects in how the ledger expressed and supported its conclusions, but Hirakawa says none changed an ahead, level, or behind verdict. That distinction matters: a conclusion can remain stable while the path a reader takes to verify it improves.

One defect was a sentence that described only one of three outcomes. It said an operation returned “not contained” for a failing name, even though the behavior also included “absent” for a name outside the root and “unsettled” when a race exceeded its bound. Naming one case as though it covered all cases compressed away meaningful behavior.
Another defect involved citation scope. One row cited nine lines where seven were needed; another cited six lines where twelve were needed. A citation that is too broad can make checking cumbersome. A citation that is too narrow is more serious when the cited fragment cannot support the full claim attached to it.

In the essay’s comparison, these defects did not alter the verdict, but they did affect whether a reader could check it. That is a different quality measure from whether the verdict itself is correct.
How review findings became repeatable checks
Hirakawa says the rounds led to three readers being added to the checking apparatus. One resolves cited ranges and compares their extent with the associated claim. Another rejects a paired count unless both values come from declared sources. A third parses ledger tables instead of trusting their visual shape.

These checks address the specific defect classes that appeared in the review. They do not establish that all citation or data errors will be caught. The important shift is from repairing a single row to making a recurring problem detectable in later rows.
Hirakawa’s proposed measure of progress is: “After a review round, name what now runs that did not run before. If the answer is nothing, that round produced an opinion.” A useful review record therefore identifies the new check, rule, or test added—not just the feedback exchanged.

When claim review becomes evidence auditing
Reviewing a claim asks whether the conclusion follows from the implementation and its evidence. Auditing the evidence apparatus asks whether the ledger, citations, counts, and checks reliably represent that support. Hirakawa’s account suggests the activity can shift from the first task to the second without changing any verdict.

Once that shift happens, name it. The team can then choose reviewers and a stopping condition suited to evidence quality—for example, resolving whether cited ranges cover their claims or whether every paired value has an explicit source—rather than expecting another pass to produce a new verdict.
The figures here describe Hirakawa’s particular case: six rounds, twelve probes and three declared conditions in the ambiguous row, the cited-line examples, and three added readers. They are not population-level statistics or evidence that a particular number of review rounds is generally useful. The account appears in Mahiro Hirakawa’s DEV Community essay, indexed as published September 14, 2026.
