Neuralink said some implanted threads had retracted, reducing the number of effective electrodes. The company adjusted its signal-processing software, and Arbaugh said performance improved. Public accounts do not show that the implant detached or that its threads were physically repaired.
What happened to Arbaugh’s Neuralink implant?
Neuralink said in a May 2024 update that, in the weeks after surgery, “a number of threads retracted from the brain,” resulting in fewer effective electrodes. The company also reported an initial drop in performance. Its public account described a change in the threads’ position and the signals available to the system—not the entire implant coming out of the brain. Neuralink’s May 8, 2024 update is the company’s own account, not an independent clinical case report.
Arbaugh told WIRED that he noticed a problem roughly three weeks after surgery: “I could tell right away that something was wrong. I just started losing control of the cursor.” He initially suspected a software change. He said the device took about two weeks to recalibrate to his prior cursor speed, and that performance continued to improve afterward. WIRED’s May 2024 interview records his account at that time.
How many threads retracted?
The figures in public accounts are not identical in scope. Neuralink said “a number” of threads retracted; it did not give a percentage in its public statement. Scientific American reported that Arbaugh said the company’s signal analysis indicated 85 percent of his threads had retracted. That percentage is his account as reported by the magazine, not a quantified figure in Neuralink’s public update. The device’s design was reported as 1,024 electrodes across 64 threads, but those design figures do not establish how many electrodes remained effective in Arbaugh’s case. Scientific American’s June 2024 report discusses his account and the limits of the available evidence; Fierce Biotech’s May 2024 report gives the N1 design figures.

Why did cursor control improve?
Neuralink said it modified its recording algorithms and increased the system’s sensitivity to signals from the electrodes that remained effective. The company said performance subsequently improved. Arbaugh also described software adaptation in interviews; in a later December 2024 conversation, he recalled the system being changed to recognize groups of neurons rather than individual neuron spikes. That account describes a software response, not a physical repair or replacement of the retracted threads.
While adapting, Arbaugh described using a workaround: holding the cursor over an item to select it rather than clicking. Scientific American reported that much of the functionality returned after algorithm adjustments. These accounts indicate improvement, but do not establish that every thread became functional again or that the physical position of the threads changed back.

Why was Arbaugh alarmed?
Arbaugh said the loss of cursor control made him fear the implant might stop working. In the May 2024 WIRED interview, he said, “Yeah, I’ve had fears about that,” referring to the possibility of more threads pulling out. He also said Neuralink told him it had seen no evidence of continued retraction and that, by the time of that interview, he was no longer worried because the threads appeared stable. Those are his reported feelings and the company’s assessment as described in 2024, not a statement about his condition in 2026.
In a December 2024 interview, Arbaugh recalled initially thinking the news meant he was out of the study and saying he had lost control of the cursor. He also attributed the retraction partly to brain motion being greater than anticipated. That explanation is his recollection; public sources do not independently establish the cause.

What is—and is not—known about the incident?
The N1 is an investigational brain-computer interface used in a clinical-trial setting. Its intended purpose is to decode movement signals so a participant can control a computer cursor. Reporting described Arbaugh using computer functions including games and web browsing. The public record supports a reported reduction in effective electrode signals, an associated period of poorer cursor performance, and subsequent software adjustments. It does not establish a specific medical injury caused by the retraction, a whole-device detachment, or a lasting physical fix.
- Cause: The public accounts do not settle why the threads retracted or precisely how far they moved.
- Stability: Arbaugh reported that the threads appeared stable by his May 2024 interview, but that does not establish their long-term position.
- Medical implications: The available reporting does not define a resulting health risk or establish a specific injury in this case.
- Broader risk: One participant’s experience cannot establish a complication rate for Neuralink or for brain-computer interfaces generally.
Scientific American reported in June 2024 that no formal scientific report on Arbaugh’s case had been published at that point. It quoted University of Cambridge bioelectronics researcher George Malliaras on the need for published data: “We have to wait until papers are published with data.” The public accounts cited here do not settle the unanswered questions above. Neuralink’s incident should also be understood as one case in a broader field: the Associated Press reported in January 2025 that multiple brain-computer-interface approaches were being studied, and cited a researcher who said Neuralink’s advantages over alternatives had yet to be shown. The Associated Press report provides that broader context.
