Researchers are investigating RNA chemical marks as possible disease biomarkers and therapeutic targets, while improved methods can profile some marks in clinical samples. Those advances do not yet establish standardized diagnostic tests, proven patient benefit, or routinely available treatments.
What epitranscriptomics studies
Epitranscriptomics examines chemical modifications on RNA and how they influence RNA behavior and gene expression after transcription. These marks do not change the underlying nucleotide sequence, but they can affect what happens to an RNA transcript.

A prominent example is N6-methyladenosine, usually abbreviated m6A. A useful way to understand its regulation is the “writers, erasers, readers” model: writer complexes add marks, eraser proteins can remove them, and reader proteins bind marked RNA and influence its fate. Depending on the context, m6A can affect RNA splicing, maturation, stability, localization, transport, and translation. Cell types can have distinct m6A profiles, so the same regulatory machinery may have different effects in different biological settings. A 2024 review of m6A research describes these mechanisms and their context-dependent effects.
Why RNA marks could matter in clinical care
If a disease is associated with altered RNA modifications—or altered activity of the machinery that writes, erases, or reads them—those patterns could help researchers identify biomarkers or therapeutic vulnerabilities. A biomarker might eventually support diagnosis, prognosis, or treatment selection; a therapeutic target might offer a way to alter disease-related RNA regulation.

These are research opportunities, not established uses in routine care. A 2024 clinical-perspectives review by Santos-Pujol, Quero-Dotor, and Esteller discusses biomarker discovery and drug development as potential applications; it does not establish epitranscriptomic testing or treatment as a clinical standard. Read the clinical-perspectives review.
How researchers detect RNA modifications
Methods have progressed from antibody-based transcriptome profiling approaches such as m6A-seq and MeRIP-seq toward techniques designed for finer mapping. The 2024 m6A review reports that m6A-SAC-seq and eTAM-seq can be used with clinical samples to produce detailed maps at single-base resolution. That is a research capability; it does not by itself show that a method is a standardized, validated diagnostic assay or that using it improves patient outcomes. The review discusses these profiling approaches.

Any eventual clinical use will depend on more than resolution. Researchers and clinicians would need to establish what is measured, whether the assay works reliably with the relevant sample, how results should be interpreted, and whether those results are useful for a specific decision in a specific disease context.
Can RNA modifications diagnose cancer today?
The cited reviews describe RNA-modification patterns as possible biomarkers, not as validated tests for routine cancer diagnosis. A promising association in research is not enough to establish a test: clinical use requires validation, reliable interpretation, and evidence that results help guide care. The available reviews do not establish a standardized epitranscriptomic test or a clinical threshold that doctors can use routinely.

Are epitranscriptomic drugs available yet?
The evidence described in the reviews is investigational. A 2024 review of therapeutic strategies reported that the METTL3 inhibitor STC-15 had entered a clinical trial for advanced cancer when the review was published. This publication-era account does not establish the trial’s present status, results, approval, or availability. The 2024 review describes the therapeutic landscape.
The same review describes PROTACs and site-specific RNA-engineering tools as approaches with preclinical proof of concept. A separate 2024 review discusses improved modification-detection methods, drug-discovery efforts, and CRISPR/Cas or endogenous-machinery-based engineering as active research directions. Such progress supports translational momentum, but it is not equivalent to clinical validation or an established therapy. The review of emerging strategies summarizes these directions.

What would show that an approach is clinically ready?
Clinical readiness depends on the particular assay or treatment, disease, sample, and intended decision. Useful questions for evaluating a proposed application include:
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitches- What is measured? The RNA modification itself, a modification-related enzyme or reader, or a downstream expression signature can each tell a different story.
- How precise and practical is the method? Resolution matters, but so do sample requirements and reliable performance in the setting where the test would be used.
- What level of evidence supports it? Mechanistic and preclinical findings, clinical research, and routine validated use are distinct stages.
- What decision is it meant to inform? A candidate may be intended for diagnosis, prognosis, treatment selection, or as a therapeutic target; success in one role does not establish another.
- For which disease and cell context? RNA-modification patterns and effects can vary by biological context, so a finding should not be treated as universally informative.
Why clinical necessity remains a goal
Epitranscriptomics offers a way to study regulation that conventional sequence-focused approaches do not capture: chemical marks can change the fate of RNA without changing its sequence. Better mapping methods and early therapeutic strategies make clinical applications plausible, but the reviews published in 2024 do not establish routine tests or treatments. For now, the field is moving toward clinical relevance; whether any particular approach becomes necessary in care depends on validation and demonstrated utility for patients.
