ChatGPT can give a fluent, confident answer that is nevertheless wrong. It generates text from learned patterns, and likely wording is not proof that a factual claim is true. Treat its responses as a starting point: clarify the question, check important claims against sources that can establish them, and get qualified expert advice for high-stakes decisions.
Why can ChatGPT be wrong?
Fluent text is not the same as verified fact
OpenAI describes a hallucination as a plausible but false statement generated by a language model. A model learns patterns in language and generates likely continuations; those continuations do not necessarily correspond to accurate facts. This is a mechanism-level explanation, not a claim that every response is a database lookup or that training consists only of predicting the next word. OpenAI explains the issue in “Why language models hallucinate”.

Some details are particularly hard to infer from recurring language patterns: a specific date, the wording of a quotation, or a little-known fact may have few reliable patterns to draw on. The result can sound polished even when a detail is mistaken or invented.

An ambiguous question may not specify a date, place, definition, or intended meaning. ChatGPT may fill in those gaps rather than ask what you mean. Complex or unusually specific questions can also make it harder to distinguish a well-supported answer from a plausible one. OpenAI’s guidance on whether ChatGPT tells the truth warns that outputs can be incorrect or misleading.

Facts can change, and citations can fail
Information about current events, policies, products, or other changing subjects may be out of date. Search or deep research can help ChatGPT consult more current material, when those features are available, but using a tool does not establish that the answer interpreted its sources correctly. A citation may also be inaccurate or fabricated. Open the cited page and check whether it supports the exact claim.
Uncertainty is better than a confident guess
OpenAI’s 2025 analysis argues that accuracy-only evaluations can reward guesses while failing to credit a model for admitting uncertainty. A system that says it cannot establish an answer or asks a clarifying question may be more useful than one that confidently supplies an unsupported detail. This is an evaluation problem, not evidence that ChatGPT is designed to lie.

What kinds of mistakes should you watch for?
- Wrong facts: incorrect definitions, names, dates, figures, or explanations.
- Invented support: quotations, studies, citations, or sources that do not exist or do not say what the answer claims.
- Misleading certainty: an answer that states a disputed, ambiguous, or uncertain point as settled.
- Outdated details: information that may once have been accurate but has changed.
These errors can be difficult to spot from tone alone. Specificity and confidence do not make a claim more reliable.
How to reduce the risk of an incorrect answer
1. Make the question specific
Include the details that determine the answer: relevant date, location, edition, definition, or constraints. If the wording could mean more than one thing, ask ChatGPT to list its assumptions or ask you a clarifying question before answering. This reduces ambiguity, but it cannot guarantee accuracy.

2. Ask it to separate claims from evidence
Request the key factual claims, the sources supporting each one, what remains uncertain, and what information is missing. Use the response as a research aid, not as proof that the sources are genuine or have been interpreted correctly.
3. Check timely claims at the source
When the answer depends on current information, use search or deep research if the feature is available in your ChatGPT product. Then open the cited source yourself. Confirm that it is relevant, current, and directly supports the specific date, number, quotation, or other detail—not merely the general topic.

4. Verify consequential details independently
For important work, check names, dates, figures, quotations, and citations against reliable sources, preferably primary sources such as the issuing organization’s own document or official record. The right level of checking depends on the claim’s consequences and whether a source directly establishes it.

5. Invite uncertainty instead of demanding confidence
Ask what ChatGPT cannot establish, which parts of the answer are uncertain, and what evidence would change its conclusion. Do not treat a confident tone as a substitute for support. An honest limitation or request for clarification can be more useful than a guess.
6. Report incorrect or unsafe replies
OpenAI recommends using the thumbs-down or report flow to flag incorrect or unsafe output. Feedback can inform the provider, but it does not automatically correct the answer you have already received; verify or replace that answer before relying on it.

There is no single current, directly comparable error-rate percentage established here for ChatGPT as a whole. Results depend on the model, version, task, evaluation method, and date, so a percentage from one test should not be presented as a universal chance that any given answer is wrong.
OpenAI’s 2023-era “Our approach to AI safety” page reported that GPT-4 was 40% more likely to produce factual content than GPT-3.5. That is a historical comparison between those models, not a current ChatGPT error rate or a guarantee that a particular response is correct. OpenAI’s GPT-4 overview also discusses hallucinations, reasoning errors, and safeguards such as human review and grounding.
