Why AI Agents Fail: A Layer-by-Layer Guide to Finding and Fixing the Cause

A plausible response is not proof an AI agent completed its task. Trace the run, find the first meaningful divergence, and test the fix at the layer that failed.

An AI agent can return a convincing answer and still fail the task. To find out why, inspect the whole run—not just the final response—including retrieved context, tool choices and results, orchestration, policy checks, and the state the agent was meant to change. Fix the first meaningful failure in that path, then test the change against task success.

Why an AI agent can fail even when its answer looks right

An agent is a system made up of a model, instructions, context, tools, control flow, and safeguards. Any of those parts can contribute to a failure, and more than one can be involved in the same run. A model may misunderstand good evidence, but it may also have received irrelevant evidence, been given an ambiguous tool interface, or followed a broken handoff.

A successful HTTP response only confirms that a request received a response. It does not establish that the agent completed the user’s task. For an action-taking agent, check the resulting state in the environment: for example, whether the requested record was actually changed. Anthropic’s agent evaluation guidance, published January 9, 2026, distinguishes the agent’s trajectory from the outcome, and notes how mistakes can propagate across turns and state changes.

That propagation matters. An early wrong assumption can shape later tool calls, and each additional step creates another opportunity for error. A transcript that ends with “done” is not proof that the intended result was achieved.

Debug the run in order

  1. Reproduce and preserve the run

Save the user input, model and version, instructions and context, retrieved material, ordered agent steps, tool names and arguments, tool results, errors, guardrail decisions, latency, cost, and final state when relevant. Keep the action order intact: it shows how a mistaken early step may have affected what came next. OpenAI’s Evaluate agent workflows documentation describes a trace as the end-to-end record of model calls, tool calls, guardrails, and handoffs for one run.

  2. Find the first meaningful divergence

Compare what happened with what should have happened. Check whether retrieval supplied useful evidence, whether the agent chose the right tool and arguments, whether the tool or API returned an error, and whether routing or a handoff sent control to the right place. Also check whether instructions conflicted or left scope unclear, and whether policy checks handled the action appropriately.

  3. Write a testable hypothesis

State which layer you think caused the failure and what change should address it. When practical, change one relevant layer at a time. If the evidence was missing or noisy, investigate retrieval; if the tool’s purpose was unclear, revise its description and examples; if a branch or handoff failed, inspect the control flow.

  4. Measure task success

Define what a correct result means before comparing versions. Grade the outcome, not only the transcript or the agent’s claim of completion. For actions that affect an environment, verify that the intended state change occurred.

  5. Turn recurring failures into regression tests

Keep representative tasks, including incidents and corrections from domain experts, as evaluation cases. Run repeated trials when outputs vary, and compare prompt or workflow changes on the same dataset or evaluation run rather than relying on intuition alone.

  6. Monitor production runs

Track task quality alongside tool calls, errors, latency, token use or cost, and user feedback. Where possible, connect negative feedback to the specific trace so that a report can be investigated rather than treated as an isolated score.

Match the fix to what the trace shows

Use this map to choose where to investigate first. These are diagnostic starting points, not guaranteed fixes; multiple layers may contribute to one failure.

What the trace shows Layer to inspect First response to evaluate
The task is understood, but evidence is missing or irrelevant Retrieval and context Check what was retrieved, its relevance and noise, and whether enough useful material reached the model.
The right evidence is present, but interpretation or behavior is wrong or inconsistent Instructions and model behavior Improve instructions and examples, then evaluate the same cases. Consider fine-tuning only when the evidence points to a learned-behavior or consistency issue and a representative dataset is available.
The agent chooses the wrong tool or supplies bad arguments Tool interface, selection, or model behavior Clarify each tool’s purpose and boundaries, add examples where useful, and grade both tool choice and argument correctness.
A tool, API, or dependency fails or stalls Tool integration and runtime Inspect inputs, outputs, errors, and latency; add bounded recovery where appropriate.
The agent takes the wrong branch, hands off incorrectly, or repeats a loop Orchestration and control flow Inspect routing and handoffs. Prefer explicit, code-driven transitions where predictable control is important.
Untrusted outside content affects an action Policy, permissions, and runtime controls Treat external content as untrusted; review guardrails and tool authority, with checks suited to the action’s risk.

OpenAI’s Optimizing LLM Accuracy guidance separates retrieval problems—such as incorrect or excessive irrelevant context—from cases where the model has the right context but uses it incorrectly. That distinction is useful: changing the model cannot supply evidence that retrieval omitted, while adding more context will not necessarily fix a model that already has the right evidence and misinterprets it.

Why agents call the wrong tool

Tool descriptions are part of the interface the model must interpret. If two tools have vague, overlapping purposes, or their boundaries are unclear, the agent may select the wrong one or use the right one for the wrong intent. Partnership on AI’s 2025 report identifies ambiguous or overlapping tool interfaces and user-intent interpretation as operational risks.

Make each tool’s purpose, inputs, limits, and expected effects clear. Add concise examples for cases that are easy to confuse. Then evaluate tool selection and argument validity separately from the final answer: an agent might choose the right tool but pass an invalid argument, or call a valid tool that does not match the user’s request.

Choose an orchestration pattern deliberately

More agents do not automatically make a workflow more reliable. Routing, handoffs, and coordination introduce their own failure points, so evaluate them as part of the system.

Pattern Who owns the rest of the turn? When it fits
Agents as tools A manager agent retains control and calls specialists. Use it when one agent should combine specialist work and own the final answer.
Handoff The routed specialist becomes active for the rest of the turn. Use it when the specialist should directly own what happens next.

These patterns are described in OpenAI’s Agents SDK orchestration documentation. The same documentation notes that code-based orchestration can be more predictable for speed, cost, and performance. Use explicit control flow when the route is predictable and needs tight control; let agent judgment handle decisions that genuinely depend on interpretation. In either case, test routing and handoffs with representative tasks.

When to change prompts, retrieval, orchestration, or the model

  • Change retrieval when the trace shows missing, irrelevant, or noisy context.
  • Change instructions or tool descriptions when the agent misunderstands scope, tool purpose, or tool boundaries.
  • Change orchestration when routing, state transitions, or handoffs cause the divergence.
  • Consider fine-tuning when evaluation evidence shows a task-consistency or learned-behavior problem, and you have representative examples to train and assess against.

Do not choose an intervention just because the final answer was wrong. OpenAI’s accuracy guidance treats retrieval tuning, prompt engineering, and fine-tuning as distinct options for distinct failure patterns. Establish a baseline first, then compare the proposed change against defined tasks and outcomes.

What to evaluate before shipping a change

  • Does the task meet a clear success criterion, including the expected environment state for actions?
  • Does the agent retrieve and use the evidence needed for the task?
  • Does it choose the intended tool and provide valid arguments?
  • Do routing, handoffs, and state transitions follow the expected path?
  • Do policy checks behave appropriately for the action and the trustworthiness of the input?
  • Do quality, latency, errors, and cost remain acceptable across representative tasks and repeated trials where outputs vary?

OpenAI and LangChain describe traces and evaluation as ways to inspect agent runs and locate problems across prompts, retrieval, tool use, policy, and orchestration. Treat platform guidance as a method for investigation, not as proof that any particular product or technique will fix a failure.
