Why AI Agent Guardrails Need a Deterministic Action Boundary

LLM guardrails can reduce prompt-injection risk but cannot guarantee safe actions. A deterministic boundary can constrain mediated side effects when paired with least privilege, information-flow controls, and oversight.

LLM guardrails can detect or discourage prompt injection, but they cannot guarantee that an agent will not carry out a privileged action. A deterministic action boundary can enforce specific rules before a tool call takes effect—if every relevant action passes through it and the policy is correct. It limits consequences; it does not make an agent immune to malicious instructions.

Why do LLM guardrails fail to stop every risky action?

An AI agent may read a user request alongside web pages, email, issue reports, or tool output. An attacker can put instructions in that untrusted content. The model may then treat those instructions as relevant to its task, even when they conflict with what the user intended.

Prompt rules and model-based detectors can lower risk, but their decisions are not guarantees. A classifier must infer intent from context; a prompt must reliably steer a model that is processing conflicting instructions. OpenAI describes this difficulty in its March 11, 2026 guidance, Designing AI agents to resist prompt injection: the aim should be to constrain the impact of manipulation even when detection fails.

OpenAI reported that a particular prompt-injection example involving a deep-research request and email worked 50% of the time in its testing. That figure applies to that example and test context only. It is not a general prompt-injection success rate, a measure of all guardrails, or evidence that every agent is vulnerable at the same rate.

Reading malicious content is different from enforcing an action

A detector may flag a suspicious page, and a defensive instruction may tell the agent to ignore it. But neither alone decides whether the agent can send a message, reveal a file, delete data, or make a network request. If the agent has the relevant authority and the action path has no independent check, a mistaken decision can still become a real side effect.

What does a deterministic action firewall do?

A deterministic action firewall checks a structured operation before it reaches the tool or service that performs it. It can apply explicit rules to an operation such as writing a file or sending a network request, then allow it, deny it, or require approval. Unlike a prompt, a policy rule does not depend on the model correctly interpreting the attacker’s intent at that moment.

The enforcement point matters. A rule can block only the actions it sees. If an agent can use an alternate tool, credential, network route, or direct operating-system call outside the boundary, the policy does not control that route. Nor does an action firewall stop the model from reading malicious content or reasoning incorrectly; it constrains a mediated action.

Example: an issue report asks for a secret

Imagine an agent asked to summarize a software issue. The issue body contains an instruction to read a secret file and publish it. A defensive prompt can tell the agent that issue text is untrusted, but that remains a model-level defense. An action policy can separately deny access to the secret or block sending its contents to an external destination. The summary task can still proceed if the permitted read and response actions remain available.

Microsoft’s Agent Framework article on FIDES, published May 20, 2026, describes this separation: the issue body is treated as untrusted, and a sensitive tool action is prevented while summarization and classification remain possible. Microsoft describes FIDES as experimental. The example illustrates a design approach, not proof that every integration using it blocks every route to sensitive data.

How do different guardrail approaches compare?

“Guardrail” can refer to controls at different points in an agent system. These approaches can complement one another, but they do not provide identical enforcement.

Approach Where it acts What it can do Key limitation
Prompt rules Instructions given to the model Tell the model how to treat untrusted content and which actions to avoid Depend on the model following instructions in context; do not independently block an action
Input or jailbreak detection Content before or during model processing Flag content judged malicious or suspicious Detection is a judgment, and a missed or misclassified input can still influence the agent
Reasoning or output checks Model reasoning or generated response Identify potential policy violations before a response or action proceeds Coverage depends on what the check observes and whether the action path enforces its result
Deterministic action policy Structured tool call or other mediated action boundary Allow, deny, limit, or require approval for operations matching defined rules Cannot constrain actions that bypass the boundary; protection depends on policy coverage and correctness
Operating-system or network controls Host, credential, or network boundary Limit access to files, services, destinations, and other capabilities beneath the agent Must be configured for the resources and routes that matter; does not interpret every task’s intent

Meta’s LlamaFirewall is a useful example of why “guardrails are just prompts” is too simple. Meta describes an open-source framework that combines PromptGuard 2 for jailbreak detection, experimental Agent Alignment Checks that inspect reasoning, CodeShield for code analysis, and customizable scanners. Meta says it uses the system in production. Those components form a layered guardrail framework; that description should not be confused with a deterministic policy at every action boundary.

What makes an action firewall effective in practice?

A firewall is only as strong as the scope of its mediation and the quality of its rules. Before relying on one, map the agent’s capabilities and decide which operations need controls.

Cover every consequential route

  • List the tools, credentials, file access, destinations, and external services the agent can reach.
  • Identify which operations can disclose data, communicate externally, spend money, change permissions, or delete or overwrite information.
  • Route those operations through the enforcement point. Check for alternate execution channels rather than assuming one tool wrapper covers the system.

Make policy decisions explicit

Rules should distinguish actions that can proceed automatically from those that must be blocked or approved. For example, a policy might allow a summary of an issue but deny reading a credential file; it might allow a draft email but require confirmation before sending it externally. The useful policy is specific about the action, resource, destination, and authority involved—not merely “be careful with sensitive data.”

Constrain information flow as well as actions

Blocking a known secret file is not enough if the same information can arrive through another tool and leave through an unfiltered sink. Systems should track where data came from and where it may go, limit external destinations, and prevent sensitive or untrusted information from crossing into prohibited channels. Least privilege reduces the harm if a rule is missing: an agent that cannot access a resource cannot disclose it through that access path.

Keep a person involved where consequences warrant it

Require user confirmation for actions whose consequences are high-impact or difficult to reverse, such as sending sensitive material, transferring funds, or deleting important data. Confirmation should cover the actual proposed action and its destination, not just ask the user to approve a vague task. For lower-risk work, narrow permissions and deterministic limits can reduce unnecessary interruptions.

Log decisions and test failure paths

Record the requested operation, policy decision, relevant approval, and execution result so an operator can review what happened. Test what the system does when a policy is malformed, a checking component is unavailable, a call times out, or an agent tries another route. A secure design should define whether the operation is denied, held for review, or allowed in each failure case rather than leaving behavior implicit.

How should you evaluate a claimed agent firewall?

Ask for evidence about the control boundary, not just a list of detection features. Architecture descriptions can explain intended behavior, but they are not independent proof of effectiveness. Google Research’s 2025 secure-agent framework advocates combining deterministic controls with reasoning-based defenses, limited powers, human oversight, and observable actions. Google’s description of security for agentic capabilities in Chrome gives a vendor example of that layered approach: action metadata for an alignment critic, origin restrictions, deterministic URL checks, and confirmation for consequential actions. Google also describes prompt-injection checks and ongoing red-teaming; these are vendor architecture claims, not an independent evaluation.

  • Enforcement point: Does the control inspect input, model output, structured tool calls, network traffic, or operating-system operations?
  • Coverage: Which tools, credentials, origins, destinations, and side effects are actually mediated? What can bypass the boundary?
  • Policy semantics: Can rules allow, deny, limit, or require approval? Can they distinguish a draft from a sent message or a read from a disclosure?
  • Information flow: Are sensitivity and trust labels carried across tools and checked at data sinks?
  • Failure behavior: What happens on timeout, malformed policy, unavailable enforcement, or an alternate execution channel?
  • Observability: Can reviewers inspect actions and decisions? If tamper resistance is claimed, has that claim been independently validated?
  • User friction: Which actions require confirmation, and how often are legitimate tasks blocked?

Project Guardian’s June 2026 version 0.1.0 whitepaper describes a user-space action firewall with allow, ask, and deny decisions and an audit log. The paper itself says the design does not control model reasoning or unmediated channels and is not a replacement for host authentication and authorization. These are the project’s own design and threat-model claims, not an independent audit or a benchmark of efficacy, bypass resistance, overhead, or false positives.

What a deterministic firewall can—and cannot—promise

A correctly placed and configured action boundary can enforce specific limits on operations that pass through it, even when an agent has been influenced by hostile content. That is a meaningful security control because it can prevent some model decisions from becoming side effects.

It cannot guarantee that the model will recognize prompt injection, that every relevant action is mediated, or that a policy contains no mistakes. The practical defense is layered: reduce the agent’s authority, mediate consequential actions, constrain data flows and destinations, require confirmation where appropriate, and make behavior reviewable. Treat any broader claim of immunity as unsupported unless the system’s coverage and failure behavior have been demonstrated.
