If a dose is missed, don’t rely on the reminder app to decide what to do: follow that medicine’s instructions or ask a pharmacist or prescriber.
Set reminders around the prescribed schedule
Enter the medicine’s prescribed times and frequency rather than accepting an app’s default schedule. A reminder is a cue, not a reason to change when or how often you take a medicine.

Where it suits the person, pair a time-based alert with an established daily activity, such as breakfast or getting ready for bed. NHS Specialist Pharmacy Service (SPS) advises considering regular activities as cues and notes that personal or interactive text and telephone reminders may be more effective than impersonal reminders. NHS SPS: Reminding to take medicines
Some reminder systems let users define a time window and flag a dose as missed if it has not been logged by the end. The FDA manual reviewed for this guidance describes one app that allows up to four reminder periods in 24 hours and does not allow those periods to overlap. Those limits apply to that specific app, not to medication reminder apps generally. FDA manual for the specific app

Make dose status clear
Turn on dose logging or another visible taken/missed status. After an alert, mark the dose as taken only when it has actually been taken. This record helps prevent uncertainty about whether a dose was already taken—the uncertainty that can lead to a duplicate.
If someone else helps with reminders, decide whether they should receive an alert when a dose remains unlogged, and who will follow up. A caregiver notification is useful only if the recipient knows what action to take and can be told promptly when the medicine schedule changes.

Choose an alert the person can notice and manage
Use sound, vibration or a visual notification according to the person’s needs and preferences. Consider whether the alert is accessible and tolerable; a sudden sound or vibration may not suit everyone. The method should also be easy to update and use consistently.

Check that phone alerts can get through
Test the alert on the device the person actually uses. Confirm that the reminder app has permission to send notifications, then check whether Focus, Do Not Disturb or background activity restrictions prevent the alert from appearing or sounding. Recheck after changing phones or installing an operating-system update.

The FDA’s February 5, 2025 communication concerns smartphone alerts from diabetes-related devices: it warns that alerts can be missed or unheard and identifies notification permissions, Focus or Do Not Disturb modes, and background or “deep sleep” behavior as settings to check. These are sensible reliability checks for phone reminders too, but the FDA communication is not a guarantee about how every medication reminder app works. FDA: Keep smartphone alerts loud and clear
Keep missed-dose decisions separate from reminders
Reminder settings can help make a scheduled dose and its status visible, but they cannot determine whether a late or replacement dose is safe. Advice depends on the medicine and circumstances, including how late the dose is; taking doses too close together can increase the risk of side effects.

Make the setup supportive, not blaming
A reminder system should fit the person’s routine, abilities and preferences. NICE recommends assessing adherence in a non-judgmental way and giving patients information about what to do if they miss a dose. Treat an unlogged or missed dose as a cue to review whether the method works—not as a reason to blame the person. NICE: Medicines adherence recommendations
