That does not make every food that raises TMAO unhealthy. If you want to lower the biomarker, the best-supported practical step is to replace some red-meat meals with plant proteins such as beans, peas, lentils or tofu—not to follow a blanket avoid list.
What TMAO is—and why food lists can mislead
Trimethylamine N-oxide (TMAO) is produced through both gut-microbial and human metabolism. Some foods provide compounds that gut microbes can convert into trimethylamine, which the body then processes into TMAO. Seafood can also supply TMAO directly. As a result, a blood or urine measurement can reflect a mix of diet and individual biology rather than a simple tally of “good” and “bad” foods.

Measured TMAO is a biomarker, not a stand-alone score for a food’s healthfulness. In randomized dietary trials, fish and whole-grain intake have both been associated with higher TMAO, even though that finding alone does not show those foods are harmful. The available sources do not establish that lowering TMAO through a specific food substitution prevents heart attacks or other cardiovascular events.
Which foods can raise measured TMAO?
Red meat: the clearest controlled-diet signal
Red meat contains carnitine, which gut bacteria can convert into trimethylamine. In a controlled feeding study summarized by the U.S. National Institutes of Health (NIH), 113 healthy men and women followed red-meat, white-meat and non-meat protein diets. After one month, TMAO levels were about three times higher on the red-meat-rich diet than on either comparison diet. Levels fell when participants stopped eating the red-meat diet. This is a study-specific result, not a guaranteed response for every person.

The broader evidence is less uniform than that single comparison might suggest. A review of 15 publications reporting 13 randomized trials, involving 553 participants with a median trial duration of 28 days, found mixed results across red-meat comparisons. The evidence supports limiting a red-meat-rich pattern, but it does not provide a precise ranking of every meat or predict an individual’s TMAO change.
Fish and shellfish: a rise does not equal a reason to avoid them
Fish and shellfish can raise circulating TMAO, partly because seafood may contain TMAO itself rather than only compounds that microbes turn into it. Randomized trials have recorded increases in plasma TMAO after dietary fish, and an international pooled analysis found that seafood associations differed among populations, with stronger associations in Asian populations than in some Western populations.

These are findings about a biomarker, not proof that fish is harmful. The reviewed evidence does not justify avoiding fish solely to lower a TMAO reading.

Eggs, poultry, dairy and plant foods: evidence varies
Associations for eggs varied across populations in a pooled analysis, and a review of food-level evidence found specific-food research limited. Animal-protein patterns have been associated with higher TMAO in some evidence, but that does not establish the same effect for every animal food or every person.

Plant-based dietary patterns often lowered measured TMAO in a review of human diet studies, but this does not mean one particular plant food will reliably lower it. The same evidence base includes links between TMAO and vegetables or whole grains, a reminder that an isolated biomarker change is not a reliable way to label foods as healthy or unhealthy.
What to eat instead of some red-meat meals
Try making some meals around plant proteins rather than red meat. NIH’s heart-healthy eating summary names beans and peas; lentils, tofu, nuts and other minimally processed plant proteins are also practical options in a varied eating pattern. The controlled feeding study supports a short-term TMAO change when participants shifted away from a red-meat-rich pattern, but it did not test every individual substitute as a treatment.

- Easy to use sperm detection test: Moisten the stain, apply the test strip, wait, and compare the colour to the colour guide on the package. An ideal infidelity test for home use.
- Versatile semen presence test: The semen detection test detects the presence of semen on textiles, bedding, ceramics, and paper; also suitable for old or dried stains.
- Forensic-style method (home screening): Inspired by forensic analysis (presumptive AP method); like any at-home semen presence test, a positive result requires laboratory confirmation.
- Contents & discreet packaging: 10 test strips, a spray bottle (fill with water), and instructions for use – shipped in discreet packaging.
- Beans, peas and lentils: Use them in soups, salads, curries, tacos or grain bowls in place of some beef or lamb meals.
- Tofu: Add it to stir-fries, noodle dishes or roasted-vegetable meals when you want a plant-based protein.
- Nuts: Include them as part of meals or snacks rather than treating them as a one-for-one meat replacement.
- White meat or non-meat meals: These were comparison patterns in the controlled feeding study; the study’s results do not establish that every white-meat choice has the same effect.
A randomized crossover trial in generally healthy adults found improvements in several cardiovascular risk factors, including TMAO, during a plant-based-meat condition compared with an animal-based-meat condition. That is evidence from one defined study, not proof that all commercial plant-based meat products are equivalent or beneficial.
What the TMAO evidence can—and cannot—tell you
A human dietary-pattern review screened 447 studies and included 25, with literature searched through July 1, 2021. Six of eight studies of plant-based diets in that review found beneficial effects on plasma or urinary TMAO. Its authors called for more long-term comparisons among Mediterranean, vegetarian and vegan diets. A separate 2021 food-association review analyzed 27 articles and said plant-based foods were less investigated; it also called for more research into cardiovascular risk in relation to fish-associated TMAO.

An international pooled analysis of 16 population-based studies found animal protein consistently associated with circulating TMAO, while associations for fish, shellfish, eggs and red meat varied among populations. Taken together, the evidence supports discussing dietary patterns and measured biomarker changes—not declaring a universal list of foods to ban.
- The studies do not establish that TMAO itself causes cardiovascular disease.
- They do not show that lowering TMAO with a specific food prevents heart attacks.
- They do not provide a universal clinical TMAO target, a routine testing recommendation or a reliable prediction of an individual’s response.
For broader food choices, NIH’s 2019 summary of the red-meat study reinforces a heart-healthy pattern that limits red meat and includes vegetables, fruits, whole grains, low-fat dairy foods and plant proteins such as beans and peas. The study’s short-term biomarker result should not be mistaken for a clinical outcome.
