When to Stop Using Eye Drops and Call an Eye Doctor

Mild dry-eye discomfort may respond to artificial tears, but persistent or worsening symptoms, significant pain or redness, light sensitivity, or vision changes call for an eye doctor.

Stop self-treating and contact an eye doctor if symptoms persist, worsen, interfere with daily activities, or come with significant pain, redness, light sensitivity, or a change in vision. Severe or sudden symptoms are not a reason to keep switching over-the-counter drops. If you use prescribed glaucoma drops, do not stop them on your own; contact the prescribing ophthalmologist about side effects or changes.

When eye drops are reasonable for mild dry-eye discomfort

For familiar, mild dryness or irritation, over-the-counter artificial tears may provide comfort. Possible dry-eye symptoms include burning or stinging, a gritty feeling, irritation, redness, mucus, contact-lens discomfort, excess tearing, and sometimes blurred vision. Those symptoms can have different causes, so they do not confirm that dry eye is the problem. The American Academy of Ophthalmology (AAO) says an eye doctor can assess tear quality or production.

Artificial tears are available without a prescription and may be used as needed for dry eye. If you use them more than six times a day, or have an allergy to preservatives, AAO advises choosing preservative-free tears; preservatives in frequent-use drops may irritate some people. Follow the product label and any clinician’s directions.

Comfort care is not a substitute for an assessment when symptoms persist, worsen, affect daily activities, or are not relieved by lubricating drops. Significant redness, pain, or a vision change also calls for an eye doctor. See AAO’s dry-eye guidance.

Red-eye symptoms that need prompt medical advice

Do not try another bottle instead of seeking advice if you have moderate-to-severe eye pain, blurred vision, increased sensitivity to light, intense redness, or symptoms that are getting worse. AAO advises contacting an ophthalmologist immediately for these symptoms, and also if you have a health condition that weakens your immune system. This guidance identifies warning signs; it is not a complete emergency triage protocol.

For conjunctivitis, AAO says to see an ophthalmologist if symptoms persist beyond the usual one-to-two-week course. Artificial tears and a cool, wet washcloth may ease discomfort from viral conjunctivitis, but they do not cure the virus or establish the cause of a red eye. Bacterial conjunctivitis may require prescribed antibiotics; antibiotics do not treat viral or allergic conjunctivitis. Read AAO’s conjunctivitis guidance.

Do not stop prescribed glaucoma drops without advice

Glaucoma drops lower eye pressure. They can cause side effects such as stinging, itching, red eyes, or blurred vision. If you suspect a drop is causing a problem, contact your ophthalmologist; AAO says never to change or stop glaucoma medication without talking to the ophthalmologist. Do not apply that glaucoma-specific instruction as a substitute for directions about other prescriptions: follow the prescribing clinician’s instructions and ask them what to do if a medicine causes symptoms. See AAO’s glaucoma guidance.

Redness-relief drops are not the same as artificial tears

Nonprescription decongestant drops can whiten red eyes, but AAO warns that using them every day can make redness worse after you stop. That warning is specific to decongestant redness-relief drops; it does not mean every eye drop has the same rebound effect. Use drops according to the label or clinician’s directions, and call a doctor if you have a problem, since it may signal an allergic reaction. Corticosteroid drops can be helpful when prescribed, but overuse can cause serious eye problems. AAO’s patient guide to eye drops explains these cautions.

Contact lenses change what to do

If you have conjunctivitis, do not wear contact lenses until it resolves; ask your ophthalmologist how to clean or replace lenses. For photokeratitis, AAO advises contact-lens users to remove lenses immediately. Artificial tears may help with comfort while the eyes heal, but follow the clinician’s instructions for your situation. These are condition-specific recommendations, not a universal schedule for drops. See AAO’s guidance on photokeratitis and conjunctivitis.

What to do when you are unsure

  • For mild, familiar dryness without warning signs, use lubricating drops as directed; choose preservative-free tears if frequent use or preservative allergy applies.
  • For persistent or worsening symptoms, or discomfort that affects daily activities or is not relieved by lubricating drops, arrange an eye-care assessment.
  • For significant pain, intense redness, light sensitivity, blurred or changed vision, or worsening symptoms, contact an ophthalmologist promptly rather than continuing to experiment with drops.
  • For suspected side effects from prescribed glaucoma medication, call the prescribing ophthalmologist before making a change.
