Dropped frames become a clinical problem when choppy video, freezes, or related audio disruption prevent a clinician from reliably seeing or hearing information needed for the assessment. There is no established universal frame-loss count, percentage, or duration that defines the threshold. The key question is whether the connection still supports the specific clinical task.
Why the clinical task matters more than a frame count
A brief stutter during a conversation may have little consequence. The same interruption can matter when an assessment depends on observing movement or appearance. That distinction is a practical inference from clinical and technical guidance, not a measured threshold for frame loss.

Video is only one part of an assessment. The Agency for Healthcare Research and Quality (AHRQ) notes that remote care can make some physical findings difficult to view, allow incidental findings to be missed, and reduce participation by onsite team members. It also identifies benefits such as improved access to clinicians, family involvement, and the ability to bring multiple clinicians into a difficult case. These are limits and benefits of telediagnosis generally—not findings that dropped frames specifically cause diagnostic errors. AHRQ’s telediagnosis material describes both sides.

What dropped frames can signal
Packet loss means data packets are missing in transmission. The National Telehealth Technology Assessment Center’s 2026 guide says it may result in frozen video, garbled audio, or dropped calls. Video usability also depends on frame rate, motion handling, color, and compression—not resolution alone. A clear still image or high resolution does not by itself establish that motion can be assessed reliably. The 2026 Clinician’s Guide to Video Platforms recommends considering latency, jitter, packet loss, connection stability, reconnection behavior, and quality monitoring.

What to do when the encounter is degraded
If the clinician cannot reliably see or hear information needed for the assessment, stop relying on the impaired observation. Pause and repeat it after the connection recovers, or move to another appropriate communication channel or an in-person workflow. For time-sensitive care, follow the service’s escalation plan. This is a practical application of the cited guidance, not a universal protocol.
- Note whether the problem was isolated or recurring.
- Record whether audio was affected and whether the relevant movement could still be assessed.
- Note the network conditions in use, such as wired Ethernet, Wi-Fi, mobile service, or a known coverage-challenged location.
What clinical services should plan and test
Organizations should define clinical protocols, test equipment under expected operating conditions, monitor quality and complaints, and maintain downtime procedures. The 2026 guide recommends testing on the network types and in the locations where the service will actually be used. It also recommends checking whether a platform keeps a session through brief interruptions, reconnects, signals degraded quality, and can preserve audio if video quality must be reduced.

Wired connections can be more stable and predictable in some clinical environments, particularly where many devices share the network. Testing a wired connection may therefore be useful when equipment and organizational policy allow; it is not a guaranteed fix for a particular fault.

For telestroke, the American Telemedicine Association guideline calls for technology that supports clear audio and video for clinical assessment, with minimal latency. It also calls for sufficient bandwidth tested during expected hours and reliable connectivity with redundancy. The guideline describes high-quality pan-tilt-zoom cameras as ideal for acute care interactions. These recommendations are specific to telestroke and should not be treated as a universal frame-loss threshold. Read the ATA telestroke guideline.

How to evaluate a video system
Assess a system against the work it must support, rather than relying on a single resolution or frame-rate specification. Compare:
- Image clarity and the ability to assess motion relevant to the clinical task.
- Audio intelligibility and synchronization with video.
- Latency and behavior during packet loss or other network disruption.
- Recovery after interruption, including whether audio can continue when video degrades.
- Quality monitoring and reporting, bandwidth needs, and device compatibility.
- Security and documented downtime workflows.
For acute telestroke, apply the telestroke-specific requirements rather than consumer-video specifications.

What the safety evidence does—and does not—show
AHRQ’s 2023 rapid evidence review screened 7,155 citations and included 23 studies. Its findings on preventable hospitalizations and emergency department visits were mixed, and it found no studies evaluating specific patient-safety practices intended to reduce harms associated with real-time telehealth. Those figures describe the scope of the review and the broader evidence base; they are not estimates of harm caused by dropped frames. The review does not establish a frame-loss threshold or quantify patient harm attributable to frame loss. See the AHRQ rapid review.
