When an AI Code Reviewer Finds Nothing, Is Your Software Actually Safe?

Months without AI code-review findings may mean a clean codebase—or a check that misses what it should catch. Here’s how to test the difference.

Not necessarily. A quiet AI code reviewer may have found no problems—or it may not be detecting the problems it was meant to catch. Months without findings are not, by themselves, evidence that software is safe.

That distinction became clear to FromZeroToShip, a self-described non-developer who uses AI to write much of the code for internal hospital tools. In a 2026 account, the author said separate agents implemented, tested, and inspected code for security issues. The reviewer flagged almost nothing for months. Then outside commenters identified roughly eight defects over three days. The examples and count are the author’s report, not independently audited findings or a measure of AI-review performance.

What the quiet reviewer did—and did not—tell its author

The author initially read the reviewer’s sparse output as reassurance. The later comments exposed a more cautious interpretation: an empty result only describes what the configured checks ran and could detect. It does not establish that the system is free of defects.

Among the reported examples were a check aimed at the wrong condition, an exclusion guard that did not verify the actual shipping run, and an exception list that did not affect pass-or-fail results. The author also described a manually performed drill whose result had expired and a scheduled job that had never been registered. These are details from one personal account, not independently verified findings.

  • BUILT TO LAST: Made from tough, waterproof, and crush-resistant materials. Manufactured in Sweden and programmed in the USA with the highest security standards.

The author’s explanation was that the implementer and reviewer shared the same underlying model and framing. Giving agents different roles did not necessarily give them independent assumptions or perspective. That is an interpretation of this episode, not proof that AI review is generally ineffective. As the author put it, “Agreement inside the room is not evidence.”

Why a passing check is not a safety verdict

A check can only provide evidence about the conditions it actually exercises. If it evaluates the wrong condition, never runs, or excludes relevant cases from its result, a clean report can coexist with a defect. The useful question is not only “Did the checks pass?” but also “What did they check, and have we demonstrated that they can catch the failures we care about?”

NIST’s Guidelines on Minimum Standards for Developer Verification of Software recommends complementary verification techniques rather than treating a single check as conclusive. NIST also says the guidance does not address the totality of software verification. OWASP’s Secure Coding with AI Cheat Sheet makes the point directly: “Measure security confidence by adversarial testing results and independent analysis, not by ‘all tests pass.’”

Prove that a check can detect the failures it is meant to catch

A practical way to interpret silence is to challenge the check with a controlled, known failure. NIST includes historical test cases—cases designed to show a bug’s presence and later absence—among its verification techniques. That gives teams a way to test the detector, not just the code under review.

  1. Pick a failure the check is meant to catch. Use a prior bug, a relevant security case, or a deliberately constructed fixture.
  2. Introduce it in a controlled setting. Keep the change isolated so it cannot affect production or unrelated tests.
  3. Run the check and inspect why it fails. A red result is useful only if it is triggered by the intended condition, not an unrelated error.
  4. Restore the safe behavior and confirm the check passes. This shows the test distinguishes the failure from the corrected case.
  5. Keep the case in the regression suite. Rerun it reliably so a future change cannot silently reintroduce the same defect.

If the check stays green when the known failure is present, its silence has been tested—and shown to be uninformative for that case. That does not prove it misses every similar bug, but it gives the team a concrete gap to fix.

Use checks with different methods and scopes

Verification layers are useful because they look for different kinds of problems. NIST describes multiple techniques, and OWASP’s AI-assisted coding guidance calls for adversarial testing and independent analysis. A layered process can include:

  • Threat modeling: identify important assets, trust boundaries, and plausible ways the system could be misused.
  • Automated tests: check expected behavior, including negative cases in which access or an operation should be denied.
  • Static code scanning: inspect code for patterns associated with security or reliability problems.
  • Black-box or adversarial tests: probe the running system with inputs and actions that challenge its assumptions.
  • Fuzzing, where appropriate: try many generated inputs to expose unexpected behavior in components that can be tested this way.
  • Dependency checks: examine third-party components as well as the code written for the application.
  • Human review: examine design and business logic that automated tools may not understand in context.

These methods are complementary, not interchangeable. A scanner does not establish that a scheduled job is registered; a passing unit test does not establish that an exception list affects the release decision. Choose checks based on the risks and system behavior that need evidence.

When does another reviewer count as independent?

A different job title or agent prompt is not enough to establish independence. Consider whether the reviewer shares the authoring process’s model, context, assumptions, or framing. The FromZeroToShip account suggests those shared inputs can matter, but it does not quantify how much they affect review quality.

Outside readers may bring useful distance because they do not carry the same assumptions or project context. Human reviewers can also contribute judgment about business logic and complex security implementations that automated tools may miss. OWASP’s Secure Code Review Cheat Sheet treats secure review as part of a broader testing approach, not a substitute for other verification.

When assessing any review process, ask whether it is meaningfully separate from the authoring process, what it covers (code, dependencies, runtime behavior, or business logic), whether it has been validated against known failures, whether it runs reliably, and whether someone can inspect and act on its findings.

How to read a reviewer’s silence

Few findings can mean the code is in good shape. They can also mean that the review’s coverage is narrow, a check is misconfigured, a test never runs, or the reviewer shares the same blind spots as the implementation process. Without evidence about what the checks cover and whether they catch known failures, silence cannot distinguish among those possibilities.

The author’s roughly eight reported defects over three days are an anecdote, not an accuracy rate or industry-wide result. The account does not establish a general performance level for AI reviewers. Its more useful question is one every team can ask of its own process: when did your reviewer last tell you something you did not want to hear?
