The studies available here test different populations, message designs, schedules and outcomes—not WhatsApp directly against SMS. Clinics should choose based on which channel their patients can receive and act on, then measure results in their own workflow.
What the studies show—and what they do not
Reminder design can affect attendance, but results from separate studies cannot be ranked as a channel contest. A study’s population, appointment type, baseline no-show risk, message content and comparison group all influence its result.

WhatsApp evidence
A 2024 randomized trial by Favaretti and colleagues enrolled 388 patients with uncontrolled blood pressure at outpatient wards in two public sub-district hospitals in Punjab, India. Patients in the intervention group received WhatsApp messages three and one days before a physician-requested follow-up visit. The messages combined a standard reminder with statements intended to debunk misconceptions; the control group received usual care without messages. Attendance was 21.8% with the intervention and 19.6% with usual care, an estimated difference of 2.2 percentage points (p=0.603). The trial found no evidence that this enhanced intervention improved attendance. It did not compare WhatsApp with SMS. Favaretti et al. (2024)
A 2026 medRxiv preprint by Sanchez and colleagues analyzed 475,214 eligible first appointments in the Autonomous City of Buenos Aires public health system, using data from June 2023 to May 2024. Its target-trial emulation compared WhatsApp reminders around 72 hours and/or 24 hours before appointments with no reminder. All studied active schedules were associated with fewer no-shows than no reminders; the largest estimated reduction was for a single reminder around 24 hours before the visit. The no-reminder comparison arm had a reported baseline no-show risk of 34.6%. This is a large observational analysis, not a randomized WhatsApp-versus-SMS trial, and it does not establish that a 24-hour reminder is best in other settings. Sanchez et al. (2026 preprint)

SMS evidence
SMS results also depend on message content and scheduling. In one of two randomized trials at a single NHS Trust in England, Hallsworth and colleagues tested reminder wording. The message that stated the health system’s cost of a missed appointment had an 8.4% did-not-attend (DNA) rate, compared with 11.1% for the existing reminder. That finding concerns SMS wording in that study setting, not a comparison with WhatsApp. Hallsworth et al. (2015)
A randomized study of 161,587 Clalit Health Services members across 14 hospitals compared nine SMS reminder formats. The strongest tested framing had a 14.2% no-show rate, versus 21.1% for the control format. This supports the possibility that framing affects SMS outcomes in that health system; it does not show that SMS outperforms WhatsApp. Huf et al. (2020)

A pragmatic randomized study at Kaiser Permanente Washington compared one with two text reminders for primary care and mental health visits. The additional reminder was associated with a modest reduction in no-shows for both groups. This suggests schedule and targeting may matter, but the study did not compare messaging channels. The studies differ in patients, health systems, appointment types, reminder content, timing, baseline risk, comparison groups and outcome definitions. Some report attendance; others report no-shows or cancellations. A lower percentage in one study than another cannot be attributed to the channel when those conditions differ. The evidence supports testing reminders and their design, but does not establish a general winner between WhatsApp and SMS.

How a clinic can choose and evaluate a channel
Compare channels in the clinic’s own patient base and workflow. If both are feasible, pilot them among comparable appointments and keep content and timing as consistent as possible. If the clinic can operate only one, start with the channel that has better demonstrated reach and workflow fit among its patients, then measure its results rather than assuming a universal advantage.
- Patient reach and preference: Find out which channel patients use, can access and want for routine reminders. Measure this rather than assuming.
- Delivery and action: Track delivery where available, and assess whether patients can confirm, reschedule or cancel—and whether staff can handle replies promptly.
- Timing: Pilot lead times suited to the appointment and clinic workflow. The studied schedules vary; none establishes a best schedule for every clinic.
- Message clarity and accessibility: Include the date, time and clinic identity, use appropriate language and readable wording, and give patients a clear way to change or cancel an appointment. SMS trials indicate that wording can matter.
- Operational and financial fit: Compare integration needs, staff effort, reply handling and local costs. The evidence here establishes no universal cost or software advantage for either channel.
- Outcomes: Record attendance, advance cancellations, late cancellations and no-shows separately. These are not interchangeable measures.
For a useful pilot, monitor those outcomes alongside delivery, replies, patient complaints and staff effort. Define the measures before launch and compare similar appointments so that changes in patient mix or appointment type do not masquerade as a channel effect.

Privacy and compliance depend on location
In the United States, the Department of Health and Human Services Office for Civil Rights says, “The HIPAA Privacy Rule permits health care providers to communicate with patients regarding their health care.” HHS treats appointment reminders as part of treatment under HIPAA, so a separate authorization is not required for them. The guidance also recommends reasonable safeguards, such as limiting the information disclosed in a reminder. This is US-specific guidance, not a general legal rule for every country, messaging provider or use of WhatsApp. Clinics should review applicable local rules, patient communication preferences and platform or vendor terms.
