That threshold mattered because specific powers and effects could target bloodied creatures or work better against them. The term also appears in D&D’s 2024 rules, but there it has no effect on its own: another rule must refer to Bloodied for anything to happen.
How Bloodied worked in D&D 4th Edition
In 4th Edition, a creature’s bloodied value was generally half its maximum hit points. When its current hit points reached that number or fell below it, the creature was bloodied. The 4e Dungeon Master’s Kit describes the point of the threshold: “Certain powers and effects work only against a bloodied target or work better against such a target.” (Wizards of the Coast, 4e Dungeon Master’s Kit)

Bloodied was therefore a rules breakpoint, not a universal condition with one fixed penalty. A particular power might become usable against a bloodied target or gain an extra effect, but reaching half hit points did not automatically make every creature weaker, remove its actions, or change its behavior.

The bloodied value and character death
For player characters, the 4e bloodied value also played a role in the death rules: it was relevant to how far into negative hit points a character could go before dying. That is a separate use of the value, not an automatic consequence of becoming bloodied. The specific effect depended on the applicable death rule.

How the 2024 rules use Bloodied
The official 2024 Basic Rules restore the term and keep the half-or-fewer threshold: “If you have half your Hit Points or fewer, you’re Bloodied, which has no game effect on its own but which might trigger other game effects.” (D&D Beyond Basic Rules, 2024)
In practical terms, the threshold is available for another rule to use, but the general Bloodied rule does not itself impose a penalty. This is different from 4e’s more explicit integration of the bloodied value into powers and character death rules, even though both use the half-hit-point mark.

How this differs from 2014 fifth edition
In 2014 fifth edition, Dungeon Master guidance allowed a DM to describe a monster at half its hit points as bloodied, giving players a broad sense of its condition without revealing an exact hit-point total. That descriptive use should not be mistaken for 4e’s explicit bloodied value and keyed mechanics, or for the 2024 glossary rule. The 2014 use was descriptive; it was not a general status that automatically caused rules effects.
Does “bloodied” reveal a creature’s exact hit points?
No. Calling a creature bloodied communicates that it has reached the half-hit-point threshold, not its precise current or maximum hit points. In the 2014 descriptive use, the point was to give players a general indication of condition without stating the exact total. Whether a DM shares even that description is a table choice, not a requirement to disclose a number.
