What to Say When Someone Is Seriously Ill—and What to Avoid

You do not need perfect words for someone who is seriously ill. Acknowledge what they are facing, listen, follow their lead, and offer specific help without promising outcomes you cannot know.

You do not need a perfect sentence. Start simply: “I’m sorry you have to go through this,” or “I’m here with you.” Then let the person guide what comes next—talking about the illness, discussing practical needs, sharing ordinary news, or sitting quietly together.

How to begin without searching for perfect words

A brief, sincere acknowledgment is usually more helpful than a polished speech. You might say:

As an Amazon Associate I earn from qualifying purchases.

  • “I’m really sorry you have to go through this.”
  • “I’m here with you.”
  • “Would you like to talk, or would you rather have a distraction?”

These are starting points, not lines to deliver regardless of the situation. Pay attention to the person’s answer and follow their lead.

What to say when they share fear or sadness

Respond to the feeling they have expressed instead of trying to argue it away. “That sounds frightening,” or “Tell me more about what is worrying you,” recognizes what they are experiencing and gives them room to continue. If you do not know why this is happening, you can say so: “I don’t know why this is happening, but I’m here with you.” Carilion Clinic offers that as an alternative to imposing a reason on someone’s illness.

Hallmark Assorted Sympathy Cards (Flowers, 12 Cards and Envelopes)
  • This sympathy card boxed set includes four lovely floral designs, each containing a unique heartfelt condolence message.
  • Boxed set includes 12 cards (3 of each design) and 12 envelopes. Each includes a compassionate sentiment, with plenty of room to write your own supportive message.
  • Keep this sympathy card assortment on hand so when your loved ones need you the most, you'll have a beautiful card that lets them know you're thinking of them.
  • Cards measure 5" x 7.2" and come with coordinating envelopes.
  • Printed on high quality paper stock, Hallmark's greeting cards are made with paper from responsibly managed forests.

You can also be candid about your own uncertainty: “I’m trying to understand, but I’m having trouble right now.” You do not have to fill every pause. Listening, and allowing silence when the person needs time, can be more supportive than trying to find another thing to say.

When they do not want to talk about the illness

Let the person choose the subject. They may want to talk about treatment or feelings, but they may also prefer a film, a shared memory, everyday news, or simple company. Ordinary conversation does not mean you are ignoring the illness; it gives the person a say in what this time together is like. Canadian Virtual Hospice notes that “There is nothing wrong with talking about your day, a movie you’ve seen, or even the apples that are on sale at the corner store.” Its communication guidance also recommends noticing cues that someone is tired or ready to stop.

Hallmark Sympathy Cards with Envelopes, 12-Pack (Watercolor Greenery)
  • When friends or family members are going through a difficult time, a card is more appreciated than ever. These are sure to bring a little light to anyone's day.
  • Cards feature four watercolor greenery designs (3 of each design). Each one contains unique sentiments inside.
  • The condolences in this boxed set are there for you when those you love need support, thoughts, and healing the most during times of loss and grief.
  • Sympathy cards measure 5" x 7.2" and come with coordinating envelopes. Assortment contains 3 cards of each design.
  • Printed on high quality paper stock, Hallmark's greeting cards are made with paper from responsibly managed forests.

If they do not want to talk, do not press for disclosure. If they seem tired, uncomfortable, or ready to end the conversation, make space for that. You can leave the door open without demanding an answer: “We don’t have to talk about it now. I’m here if you want to later.”

How to offer practical help

Offer a specific, manageable task that the person can accept or decline. “Could I bring dinner on Thursday?” or “Would it help if I sat with you while you make that call?” is easier to answer than “Let me know if you need anything,” which leaves the ill person to identify a task and ask for it. Adapt the offer to their preferences and energy, and do not take a yes for granted if circumstances change.

For sensitive questions about planning or arrangements, explain why you are asking and seek permission rather than raising the subject abruptly. You might say, “I want to support you in the way that matters to you. Would you be willing to tell me what’s important?” Carilion Clinic’s guidance on hospice conversations likewise recommends care and context around difficult practical questions.

What to avoid—and what to say instead

Try not to say Why it can hurt A more supportive response
“Don’t worry, you’ll be just fine,” or “You’re not going to die.” You cannot know the outcome, and reassurance can make fear feel unwelcome. “That sounds frightening. I’m here with you.”
“Everything happens for a reason.” Unless invited, an explanation may feel dismissive or suggest the person’s illness is deserved. “I don’t know why this is happening, but I’m here with you.”
“You must fight this,” or “Stay positive.” It can burden someone with expectations and make it harder to speak honestly. Ask what is on their mind and respond to what they actually say.
“I don’t want to talk about your illness.” They may want to talk, and closing the subject can deepen isolation. “I’m willing to listen. Would you like to talk about it, or do something else?”
“Have you made any arrangements?” without context The abrupt question may overwhelm someone or make the conversation feel like an interrogation. Explain your caring reason, ask permission, and let them decide whether to answer.
“There is nothing more that can be done.” It can sound as though support and care have ended, even when symptom relief and other help may still be available. Do not guess about treatment or prognosis. Ask whether they want help discussing the situation with their care team.

The International Association for Hospice and Palliative Care gives clinician-oriented communication guidance, including the warning never to say “there is nothing more that can be done.” For a friend or relative, the practical point is to avoid declaring what care remains; leave specifics to the clinicians.

If they ask what will happen

Be honest about what you know and what you do not know. Do not guess at a prognosis, promise a particular outcome, or present your impression as medical advice. A useful response is: “I don’t want to guess. Would you like me to help ask your doctor or nurse?”

Clinical communication guidance recommends finding out what the person understands, asking how much information they want, explaining uncertainty clearly, and discussing realistic options. You can support the conversation, but their care team is the right source for questions about diagnosis, treatment, and prognosis. The IAHPC communication guidance sets out these principles for health-care professionals.

If they seem confused or describe something you cannot verify

If the person is not distressed, do not automatically argue or abruptly correct them. Stay calm, offer support, and tell their health-care team about new confusion; some causes may be treatable. Do not dismiss a sudden or concerning change as merely conversational—contact clinicians so they can assess it.

Talking with children about serious illness

The US National Cancer Institute’s guidance addresses families facing advanced cancer, rather than every serious illness. In that context, it advises adults to be honest, listen instead of trying to solve every problem, and respect the ill person’s choice about whether and when to discuss the illness.

For children in that advanced-cancer context, use simple, truthful language; reassure them that they did not cause the illness and that they will be cared for; and invite their questions. Adapt the conversation to the child’s age and circumstances, and seek professional support when the family needs it.
