What to Know About Dengue in the U.S. in 2026

Dengue transmission varies by location: most cases reported in the states are travel-associated, while local transmission is sporadic. Here’s how to interpret CDC’s latest detailed data and protect yourself.

Dengue is not spreading uniformly across the United States. Most cases reported in the 50 states and Washington, DC are linked to travel, while local transmission has occurred sporadically; frequent or continuous transmission is concentrated in several U.S. territories and freely associated states. The latest detailed national analysis covers 2024, not 2026: CDC counted 3,798 cases among state and DC residents that year, and those historical figures should not be read as this year’s total or current distribution.

What is the current U.S. dengue picture?

CDC’s 2026 surveillance page, updated September 30, 2026, presents current-year reporting, but its figures are provisional, can be delayed, and may change. CDC’s detailed national report published May 14, 2026 analyzes cases reported in 2024 among residents of the 50 states and Washington, DC. It is the most useful detailed national snapshot here, but it does not establish the 2026 total. Check CDC’s current dengue data or your state or territorial health department for the latest counts.

Surveillance does not capture every mild illness. CDC assigns reported cases by the patient’s county of residence, which may differ from where exposure occurred, and reporting schedules can vary between CDC and state or territorial agencies. These limitations matter when interpreting maps and totals.

What the 2024 state and DC report found

In 2024, CDC recorded 3,798 reported cases among residents of the 50 states and Washington, DC—359% above the 2010–2023 annual average of 828. Of the 2024 cases, 3,693 (97.2%) were travel-associated and 105 (2.8%) were locally acquired. These are historical surveillance counts, not a 2026 estimate. CDC reported that 41.6% occurred from July through September.

The states with the most reported cases in that 2024 analysis were Florida (1,044), California (720), New York (338), and Texas (241). Locally acquired cases were reported in Florida (85), California (18), and Texas (2). Those figures describe where cases were reported by residence, not a current ranking or a forecast.

Where does dengue transmission occur?

Dengue spreads mainly through bites from infected Aedes mosquitoes. A case associated with travel means the person was exposed outside the place where the case was reported; local transmission means infection occurred in the reporting area. Most cases reported in the continental states are travel-associated, although local transmission has occurred sporadically. CDC describes frequent or continuous dengue transmission in Puerto Rico, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau. It has also reported previous local transmission in Florida, Texas, Hawaii, Arizona, and California. See CDC guidance on dengue public health considerations for jurisdictional context.

That distinction is important when interpreting “outbreak in the U.S.” It does not mean dengue is spreading at the same rate across the continental states. The destination, whether transmission is ongoing there, and the time of year all affect exposure risk; the 2024 seasonal pattern is not a forecast for another year.

What are dengue symptoms, and when is it urgent?

Dengue has four related virus types, and a person can be infected more than once. Common symptoms include fever, aches and pains—often behind the eyes or in the muscles, joints, or bones—nausea or vomiting, and rash. Symptoms usually begin within two weeks of an infected mosquito bite, typically last two to seven days, and most people recover after about a week. CDC’s dengue overview explains symptoms and care.

Contact a healthcare professional if you develop symptoms, especially after travel to an area where dengue occurs, and tell the clinician where and when you traveled. Seek urgent medical assessment for warning signs such as abdominal pain or tenderness, persistent vomiting, fluid accumulation, or mucosal bleeding. Severe dengue can involve shock or fluid leakage that causes breathing difficulty, severe bleeding, or serious organ involvement; it requires urgent medical care.

The seriousness of the illness is reflected in the 2024 state/DC surveillance report: 36.1% of reported patients were hospitalized, 2.8% of cases were classified as severe dengue, and six patients (0.2%) died. The deaths were among travel-associated cases that developed severe dengue. These are outcomes among reported 2024 cases, not an estimate of an individual’s current risk.

Is a dengue vaccine recommended for U.S. travelers?

CDC’s U.S. vaccine recommendation is limited: dengue vaccine is recommended for children ages 9–16 who have laboratory-confirmed evidence of a previous dengue infection and live in areas where dengue is common. CDC says the vaccine is not approved for U.S. travelers who visit those areas but do not live there. Recommendations and eligibility can change, so consult current CDC guidance and a healthcare professional about an individual situation.
