An AI safety audit log should let a reviewer reconstruct what happened, when it happened, which system version was involved, what relevant conditions shaped the result, and what people or systems did next. There is no universal field list: choose events and context for the system’s intended purpose, risks, deployment setting, and applicable rules, then check that the records can be protected, reviewed, and retained lawfully.
What events should an AI audit log record?
Start with the questions an investigation, monitoring process, or audit must be able to answer. Define event types around the system’s use and risks rather than logging everything indiscriminately. Depending on the system, useful event categories may include:

- Requests, inputs, or actions that trigger the AI system, with a protected reference to the input when keeping the full content is unnecessary.
- Outputs, decisions, status, or confidence fields that matter to the use case, including unavailable, rejected, or failed results.
- Calls to tools or external data sources, and safety filters or policy checks that materially shape behavior.
- Errors, interruptions, and other operational events that could affect the result or leave a gap in the record.
- Human review, approval, override, escalation, and incident-response actions.
- Changes to the model, prompts, configuration, access, or logging rules that could affect system behavior or how an event is interpreted.
These are design options, not a regulatory checklist that applies to every AI system. NIST assessment procedures treat defined event types and periodic review of event selection as relevant audit-control concerns; the selection should change when the system or its operating context changes. See NIST SP 800-171A Rev. 3.
What information makes an audit trail useful?
For each chosen event, capture enough information to link it to the system and conditions that produced it. A practical schema may include:
#1 Best Overall
- Hard Bound Book with Reinforced Imitation Leather Cover, and Placeholder Ribbon
- Section Sewn - Books lies flat when open; Archival Quality, Acid-Free Paper
- Page Dimensions: 8.5" X 11" (21.6cm X 25.4cm )
- Event identity: a stable event ID, event type, timestamp with an explicit UTC or timezone convention, and a session or correlation ID.
- System context: the AI system, model, configuration, policy, and deployment version needed to interpret the event later.
- Trigger and relevant input: the initiating action and either the relevant input or a protected reference. Minimize or redact personal and sensitive data when full payload retention is not needed, and follow applicable privacy and security requirements.
- Result: the output, decision, status, or confidence information that is material to the use case, with clear values for unavailable or failed results.
- Influencing components: relevant tool calls, external data sources, safety filters, policy checks, or system errors.
- People and follow-up: human review and response actions, with actor identity or role only where lawful and necessary.
The goal is not maximal data collection. It is sufficient, interpretable evidence for the purpose of the log, without retaining unnecessary sensitive content. NIST describes its AI Risk Management Framework as voluntary guidance for incorporating trustworthiness into AI design, development, use, and evaluation; it is not itself a logging mandate. See the NIST AI RMF overview.
When a specific legal minimum applies
For high-risk AI systems within the EU AI Act’s scope, Article 12 requires technical capability for automatic event logging over the system’s lifetime, with traceability appropriate to intended purpose. The article identifies objectives including detecting risk situations, enabling post-market monitoring, and monitoring operation. It also sets a more specific minimum for certain remote biometric identification systems: use start and end times, the reference database checked, input data that produced a match, and identification of people involved in verification. That special list should not be generalized to other AI systems. See Regulation (EU) 2024/1689, Article 12.

How long should AI logs be kept?
Set a documented retention schedule by record category and purpose. The applicable period depends on jurisdiction, sector rules, privacy limits, the organization’s role, and whether a legal hold applies; there is no general retention period for every AI log.

For covered high-risk AI systems under the EU AI Act, Article 19 requires providers to retain automatically generated logs under their control for an appropriate period and at least six months, unless applicable Union or national law, particularly data-protection law, provides otherwise. This rule is scoped to those provider-held logs and does not automatically govern systems outside the Act’s scope. The EUR-Lex consolidated text cited here is dated 2026-07-27; check current scope, implementation dates, amendments, and national rules before making compliance decisions. See Article 19.

Do not confuse log retention with documentation retention. Article 18’s ten-year requirement concerns specified provider technical and compliance documentation; it is not a general ten-year requirement for AI logs. See Article 18.
How should logs be protected and operated?
A log is only useful if it is dependable and can be reviewed when needed. Define who may write, read, export, and administer records; protect them against unauthorized changes or deletion; and monitor the logging pipeline itself.

- Specify how events are selected, records generated, timestamps produced, and retention applied.
- Restrict privileged access and make authorized review or export possible.
- Monitor for missing events, pipeline outages, timestamp problems, dropped records, and storage limits; alert and define a response when logging fails.
- Preserve enough version and correlation context to connect records during an investigation.
- Test the policy and process periodically, including whether the chosen event types still fit the system and its context.
NIST SP 800-171A Rev. 3 includes assessment procedures addressing audit-record generation and content, timestamps, retention, review and analysis, and response to logging failures. It is a security assessment resource, not an AI-specific legal mandate.

How to evaluate an AI logging design
When choosing a logging design or platform, assess it against the work the records must support:

- Coverage: Does it capture the event types required by the system’s risks and use?
- Reconstruction: Can a reviewer connect events to versions, sessions, relevant inputs and outputs, and follow-up actions?
- Integrity and access: Can records be protected while privileged access is restricted?
- Retention and deletion: Can it enforce policy-based retention, lawful deletion, and legal holds?
- Review and response: Can staff search records, investigate incidents, receive alerts, and export evidence?
- Resilience: Does it expose outages, dropped events, clock issues, and storage limits?
These are evaluation dimensions, not a ranking of products. Keep legal requirements separate from implementation choices: the EU AI Act is binding within its scope, while the NIST AI RMF is voluntary and NIST SP 800-171A Rev. 3 provides assessment procedures. The NIST AI RMF-to-ISO/IEC 42001 crosswalk also references event logging and production-operation monitoring, but does not establish a certification claim or reproduce the full ISO standard.
