A useful handover note lets someone continue a task without guessing. State what the work is, what has happened, what remains, the next action and its owner, and where to find the current materials. Add only the context, risks, or decisions that could change what the recipient should do.
What a handover note needs to do
The aim is to transfer task-relevant information so work can continue across people or shifts. The UK Health and Safety Executive describes the goal of shift handover as “the accurate reliable communication of task-relevant information across shift changes or between teams thereby ensuring continuity of safe and effective working.” That principle also helps with ordinary work: the recipient should be able to understand the task, locate its materials, and act on the current status.

There is no single format that suits every handover. A short note may be enough for routine, low-risk work; tasks with dependencies, unresolved risks, or operational consequences need more context and may call for a direct conversation as well.

A practical handover note template
Use these fields as a starting point, not as a mandatory form. Include details that help the recipient continue; mark unknowns clearly rather than filling gaps with guesses.

- Task and outcome: Name the work and explain what completion means.
- Current status: Say what is complete, what is in progress, and what is waiting or blocked.
- Next action: State the first concrete step. Add a due date or trigger only when it is known and relevant.
- Owner and contacts: Identify who is responsible for the next step and who can answer questions.
- Links and location: Link to the current working file, record, decision, or instructions so the recipient does not have to search for them.
- Blockers, risks, and dependencies: Explain what could prevent progress, what it affects, and any known response or escalation route.
- Last updated: Include when the information was checked.
How to describe status and unfinished work
Separate completed work from open work. Avoid vague labels such as “in progress” on their own: say what has been done, what remains, and what is preventing the next step, if anything. If work is blocked, name the dependency or decision it is waiting on and the next known follow-up action.
For a formal project or service transition, UK government Gate 4 guidance calls for incomplete work and defects to be identified and recorded with a remediation plan. That is a useful model for consequential handovers: record the issue, what must happen next, and who is expected to address it. Do not invent an owner or deadline if neither has been agreed.

Include context that changes the next decision
A recipient usually needs the background that explains what to do next, not a full history of the task. Include relevant decisions already made, dependencies, deadlines or triggers, key contacts, and risks when they affect the next action. Link to fuller records where useful rather than copying large amounts of material into the note.

It is not a universal rule for every individual task note, but it reinforces the value of linking to material the recipient can rely on.

When a note is not enough
For safety-sensitive work, unresolved risk, or a transition with significant operational consequences, use a direct exchange or an agreed handover process alongside the written record. HSE guidance includes cross-checking as part of effective handover: the incoming person should be able to confirm important details, ask questions, and resolve uncertainty before acting.
For routine work, a concise written note may be sufficient. The NHS knowledge-retention toolkit likewise presents handover activities as options to choose from, rather than one checklist that fits every situation. Follow your employer’s procedures for regulated, clinical, safety-critical, or contractual work.

Check the note before you send it
- Can the recipient tell what the task is and what counts as completion?
- Is it clear what is done, what is open, and what is blocked or at risk?
- Can they identify the next action and the person responsible?
- Do the links lead to the current files or records?
- Are unknown details marked as unknown instead of guessed?
- Does the consequence of the work call for a conversation or cross-check too?
No general effectiveness percentage is established for ordinary task handover notes. The HSE discusses the 1983 Sellafield Beach Incident as an example of failed shift communication, but that historical example does not provide a rate that can be applied to everyday notes.
