What to Expect During a TMS Treatment Session

TMS sessions usually involve sitting or reclining while a clinician positions a coil against your scalp and delivers pulses. The first visit commonly includes mapping; timing and schedules vary by protocol.

During a transcranial magnetic stimulation (TMS) session, you usually sit or recline while a clinician positions a magnetic coil against your scalp and delivers pulses. You stay awake; treatment generally does not require anesthesia or sedation. The first appointment commonly includes mapping to determine where and how strongly to stimulate, so it may take longer than routine visits. The exact experience and schedule depend on your treatment protocol, device, condition, and clinic.

If you are wondering, “What does TMS treatment look like?” the short answer is that it is an outpatient appointment: the operator positions the coil, gives pulses according to your treatment plan, and monitors the session. There is no single visit length or schedule that applies to every patient. Ask your clinic for separate estimates for the mapping appointment and later routine visits.

Before your first appointment

A clinician should first assess whether TMS is appropriate for you and review your medical history. Tell the treating team about implanted devices, metal or implants in your head, a history of seizures or epilepsy, and any relevant changes in your health or medications. The American Psychiatric Association notes that some people with head implants, aneurysm clips or coils, cochlear implants, cardiac defibrillators, or epilepsy history may not be candidates. FDA guidance also warns that repetitive TMS can interact with pacemakers and other active implanted devices. These are reasons to have an individualized screening conversation, not a way to determine your eligibility on your own.

Be specific when discussing devices and implants rather than assuming that something is irrelevant because it is not near the scalp. The clinic needs enough information to assess your circumstances and explain any risks or precautions for the device and protocol it uses. FDA guidance discusses seizure precautions and common adverse events such as headache and pain at the application site. It also describes patient labeling that should explain warnings and risks. Ask your clinician to explain the screening and the information in the patient materials; a general article cannot establish whether TMS is suitable for you.

Follow the clinic’s preparation instructions. Cleveland Clinic advises removing metal items, and the treating team can tell you what to remove and whether anything else needs to be done before you arrive. Ask ahead whether the first appointment includes mapping, how long to reserve, what hearing protection the clinic supplies, and which personal medical details it needs.

It can help to make a short list of your implants or active devices, seizure history, medications, and recent health changes before the screening conversation. This is not a substitute for the clinic’s questions or instructions; it can help you remember what to disclose. If anything changes between screening and your appointment, tell your treating team rather than assuming the original plan still applies unchanged.

What happens at the first session

You will generally be seated or reclined. The clinician positions the coil against your scalp and delivers test pulses while establishing the treatment location and intensity. This mapping process commonly includes finding your motor threshold—the stimulation level used to help set treatment intensity—and identifying the treatment site. The operator may adjust or reposition the coil during this assessment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mapping is the part of the first visit that helps the team identify where to place the coil and determine the intensity used for your treatment. Test pulses are part of that assessment; they are not the same as a promise that every later session will repeat the entire mapping process. A brief hand or finger twitch can occur with test pulses, so do not be surprised if you notice movement during this stage. If you are uncertain what the clinician is checking, you can ask them to explain the step before proceeding.

Mapping helps establish settings for treatment; it is not necessarily repeated at every visit. FDA guidance says the motor threshold and treatment location are not typically reassessed at each session, though clinicians may reassess when appropriate, including after medication or health changes. Ask your clinician how mapping and any later checks fit your particular plan.

Mayo Clinic gives about an hour as a general estimate for a first appointment that includes mapping. That is a clinic-specific estimate, not a guaranteed duration. Confirm your appointment length with your own provider, especially if the visit includes additional assessments. When you ask, clarify whether the estimate is for the full appointment or only part of it, and whether any other assessment is scheduled at the same visit.

What a treatment session feels and sounds like

The coil produces clicking sounds as it delivers pulses. You may feel tapping on your scalp, and test pulses during mapping can briefly make a hand or finger twitch. Scalp discomfort and a mild headache are among the commonly described effects. People’s experiences vary, so it is not accurate to promise that treatment will be painless.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The tapping and clicking are part of the pulse delivery, and the experience may vary from person to person. Scalp discomfort or a mild headache can occur; FDA guidance also identifies headache and application-site pain among common adverse events. These descriptions do not predict what you personally will feel or how intense it will be. If discomfort, sound, or a headache concerns you, tell the operator or treating team so they can respond in the context of your treatment plan.

Hearing protection is an important part of session practice. FDA device guidance states: “Subjects should be required to wear earplugs during the treatment sessions.” Clinics may provide earplugs or other hearing protection; ask what is available and use it as directed. Tell the operator if the sound or sensation is uncomfortable so the team can respond within your treatment plan.

You generally do not need to purchase hearing protection before the visit: clinics commonly supply it, and Mayo Clinic describes patients receiving earplugs. Still, it is reasonable to ask what your clinic provides and whether it has instructions for wearing it. Reusable earplugs may be an optional personal preference, not a treatment requirement established by this guidance. Do not skip the hearing protection your clinic directs you to use.

What happens during routine visits

For later sessions, the operator generally returns the coil to the recorded treatment position and uses the settings prescribed for you. The session still involves pulses and the clicking and tapping sensations described above. The time required can vary by protocol and device; the available guidance does not establish one visit length that applies to every clinic or treatment plan.

Because mapping and treatment setup are not typically repeated at every appointment, a routine visit may differ from the first mapping visit. However, that does not mean every routine visit is identical or has a guaranteed duration. Your clinic may include check-ins or reassessment as part of your care, and the treating clinician determines whether to revisit settings. FDA guidance notes that reassessment may be appropriate, including after medication or health changes.

Ask your provider to distinguish the time spent receiving stimulation from the time you should allow for the whole appointment. Also confirm whether your plan includes any check-ins or reassessment, rather than assuming every visit will follow an identical routine.

When comparing treatment options or clinic schedules, look at more than a headline session-time estimate. Useful points to compare include time per visit, how many visits are scheduled per day or week, the expected total course length, the treatment condition and device, and whether the first appointment includes mapping. Protocols can differ, so a schedule described as standard elsewhere may not match your prescription. Your treating clinic can explain which details apply to you.

How often treatment is scheduled

The American Psychiatric Association describes a standard course as five treatments per week for four to six weeks. This is a general description, not a universal schedule. FDA guidance characterizes a course as involving multiple treatments, and protocols can differ, including shorter sessions and accelerated regimens. Your clinic’s prescribed schedule is the one to follow.

The number of appointments is one practical difference between protocols, but it is not the only one. Ask how many visits are planned per week, whether the protocol calls for shorter sessions or an accelerated regimen, and how many weeks the clinic expects the course to take. A course described as five treatments weekly for four to six weeks is a reference point from the APA, not a prediction for every patient. The plan depends on the treatment condition, device, and protocol.

When discussing your schedule, ask how many visits are planned per week, how many treatments occur on a treatment day, how long routine appointments usually take, and how long the overall course is expected to last. The first mapping appointment may differ from routine visits, so ask for separate time estimates.

Protocols may also differ in whether mapping is completed at the first appointment and in whether settings are later reassessed. Ask the clinic how it handles those steps, especially if your medications or health circumstances change during the course. Rather than using another clinic’s timetable to plan your days, use the schedule your own provider gives you and check in if it changes.

After the appointment

TMS generally does not require anesthesia or sedation. The American Psychiatric Association says patients can drive to and from appointments and continue most usual activities. Follow your own clinic’s instructions, particularly if the first visit includes other assessments or your clinician gives you guidance specific to your circumstances. Do not treat general information as confirmation that driving or any particular activity is appropriate for you.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before leaving, ask what to do if you experience discomfort or a headache later and how to contact the clinic with questions. The research supports that headache and application-site pain are among commonly described adverse events, but it does not establish a single response that is right for every patient. Follow the instructions your team provides for your treatment and personal circumstances.

Questions worth asking your clinic

  • Will my first appointment include mapping, and how much time should I allow?
  • How long do routine visits take under my prescribed protocol?
  • What hearing protection will be provided, and when should I wear it?
  • Which implants, devices, medical conditions, medications, or health changes should I disclose?
  • How often will treatment be scheduled, and what changes could lead you to reassess my settings?
  • What should I do if the tapping, sound, scalp discomfort, or headache is difficult to tolerate?
  • Is the visit-time estimate for the full appointment or only the time receiving stimulation?
  • Does my course use a standard schedule, shorter sessions, or an accelerated regimen?
  • Will there be any assessments or check-ins in addition to mapping and treatment?
  • What should I tell the team if my health or medication changes during the course?
  • What should I expect after a visit, and whom should I contact if I have a concern?

Your treating team can answer these questions for your device and protocol. Session details differ, so the clinic’s instructions—not a general schedule or estimate—should guide your preparation.

Frequently asked questions

Will I be awake during a TMS session?

Yes. TMS is described as an outpatient procedure in which you remain awake while an operator positions the coil and delivers pulses. It generally does not require anesthesia or sedation.

How long is the first TMS appointment?

Mayo Clinic gives about an hour as a general estimate for a first appointment that includes mapping. This is not a universal promise: ask your clinic how long to reserve and whether the visit includes any additional assessments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does every visit include mapping?

Not usually. FDA guidance says the motor threshold and treatment location are not typically repeated at every session. A clinician may reassess when appropriate, including after medication or health changes, so ask how your plan handles later checks.

Will TMS hurt?

Experiences vary. People may feel tapping on the scalp, and scalp discomfort or a mild headache are commonly described. It would not be accurate to promise that treatment is painless. Tell the operator if the sensation is uncomfortable.

Do I need to bring earplugs?

Ask your clinic what hearing protection it supplies. FDA device guidance says earplugs should be required during treatment sessions, and clinics commonly provide protection. Follow your clinic’s instructions; buying your own is not generally necessary if protection is supplied.

Can I drive after TMS?

The APA says patients can drive to and from appointments because TMS generally does not require anesthesia or sedation. Follow your treating team’s advice for your circumstances, especially if your first appointment includes other assessments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How often will I have treatment?

The APA describes a standard course as five treatments per week for four to six weeks, but protocols differ and may include shorter sessions or accelerated regimens. Confirm your schedule and expected course length with your clinic.
