There is no single “best” first food or universally safe schedule for ending a prolonged fast. If you have had little or no food for more than five days, or you are malnourished or have other risk factors, restarting nutrition can require clinical guidance and monitoring—not a self-directed meal plan.
First, work out whether general meal advice is appropriate
“Prolonged fast” can mean anything from a day without food to several days of little or no intake. The risk changes with duration, nutritional status, health conditions and medicines. NICE adult nutrition-support guidance treats more than five days of little or no intake as a reason to introduce nutrition cautiously; it does not prescribe a universal first meal for everyone.

Get medical advice before restarting food if any of these apply:
- You have eaten little or nothing for more than five days.
- You have a low body weight, significant recent unintentional weight loss, or a history of malnutrition.
- You have been told that your potassium, phosphate or magnesium is low.
- You have a relevant illness or take medicines that may affect nutrition or fluid and electrolyte balance.
These are prompts to seek an individual assessment, not a self-diagnosis checklist. NICE says people who meet its high-risk criteria should be cared for by healthcare professionals with appropriate skills.

Why restarting food can need medical supervision
After a period of very low or no intake, reintroducing nutrition can cause refeeding problems. NICE recommends that people who have eaten little or nothing for more than five days receive no more than 50% of their requirements for the first two days, with increases only if clinical and biochemical monitoring reveals no problems. This is clinical guidance—not a calorie target to calculate or follow at home.
NICE identifies a person as high risk if any one of these applies:
Rank #2
- 30g PROTEIN & 1g SUGAR: Each Ensure Max Protein shake has 30g protein to support muscle health and manage hunger
- 25 VITAMINS & MINERALS: Formulated with nutrients to support immune, bone, and brain health, as well as energy metabolism
- SUPPORTS DIGESTIVE HEALTH: Contains 4g fiber with a prebiotic to support digestive health
- IMMUNE SYSTEM SUPPORT: An excellent source of zinc, vitamins A & D, and antioxidants (vitamins C & E and selenium) for immune system support
- GREAT-TASTING VARIETIES: Including caffeinated flavors that have as much caffeine as a cup of coffee
- Body mass index (BMI) below 16 kg/m².
- Unintentional weight loss greater than 15% over the previous 3–6 months.
- Little or no nutritional intake for more than 10 days.
- Low potassium, phosphate or magnesium before feeding.
High risk can also be indicated by two or more lesser criteria. NICE’s management guidance calls for cautious introduction and gradual increases, with close attention to fluid balance and clinical status. A clinician must assess what monitoring and support are appropriate for the individual.
What should you eat first?
The cited NICE guidance does not name one food that everyone should eat first, or give a universal progression from a first bite back to normal meals. It says nutrition support should be planned around more than calories: energy, protein, fluid, electrolytes, minerals, micronutrients and fibre all matter, alongside a person’s condition and tolerance.

That is why a generic starter-food list cannot establish what is safe for someone who has fasted for several days, lost substantial weight or has a medical risk factor. If you are in one of those groups, ask a qualified healthcare professional how to restart eating rather than choosing a food or schedule based on general advice.
How to return to normal meals
There is no single timetable that applies to everyone. For an adult who has had only a short fast and has no relevant health or nutrition concerns, ordinary food choices may be guided by appetite and comfort. But that general observation is not a refeeding plan for someone with prolonged low intake or malnutrition risk.

If you have had little or no intake for more than five days, NICE’s approach depends on clinical and biochemical monitoring before increasing nutrition. People who meet its high-risk criteria need appropriately skilled professional care. In those circumstances, do not use a generic meal progression, calorie goal or supplement routine in place of individualized advice.
What NICE’s guidance does—and does not—establish
- It establishes: cautious introduction after more than five days of little or no intake, defined high-risk criteria, and the need for clinical and biochemical monitoring when increasing nutrition.
- It does not establish: one best first food, a universal schedule for returning to normal meals, or a home protocol suitable for every person ending a fast.
For individualized advice, contact a qualified healthcare professional, particularly if you have had several days of very low intake, significant weight loss or low body weight, known electrolyte abnormalities, or a relevant illness or medication.
