Preserve the original alert and media, check the scene and worker exposure, and have the responsible site safety reviewer document a decision. If the condition might be dangerous, control the risk while it is being checked; do not dismiss a possible hazard just because the alert may be wrong.
First, separate the software alert from the safety decision
A camera-based system can identify a visual pattern and label it as a possible violation. That label is an internal alert unless other facts establish what happened. The OSHA materials discussed here describe agency inspection and enforcement practices; they do not say that a private AI alert is an OSHA citation, and they do not establish a product-specific appeal process. The applicable rule, site procedure, and facts at the location—not the model’s wording alone—determine how the alert should be handled.

Keep two tracks moving at once. The safety track asks whether anyone was exposed to a real hazard and what needs to happen to protect them. The software-review track asks whether the alert correctly identified the time, place, activity, and condition, and how the product lets an authorized user report a mistaken alert. A false positive on the software track must not prematurely close a real safety issue on the first track.

What to do, in order
1. Preserve the alert and the original media
Save the alert text, its identifier, timestamp, and any available camera or image context. Preserve the original image or video in the form available to the reviewer, and do not edit or overwrite the only copy. If you need to make a marked-up copy for discussion, keep it separate from the original and identify it as an annotated copy. This creates a record of what the system actually flagged rather than a later reconstruction.

OSHA’s Field Operations Manual, Chapter 4 describes photographs, video, measurements, statements, and relevant documents as evidence used to document exposure during agency inspections. That is useful context for organizing a site review, not a rule that every private alert must be handled in a particular way.
2. Check the scene, time, and work activity
Establish where and when the event occurred, what task was underway, what the camera could see, and whether employees were present or could have been exposed. Compare the alert with the physical condition and relevant site materials, such as a safety plan, work rules, permits, equipment information, or the applicable standard. An image can capture one viewpoint at one moment; it may not show what happened immediately before or after, a control outside the frame, or whether a worker was actually exposed.
Do not infer the full work context from a model-generated label. OSHA field guidance calls for documenting observed and potential exposure and reviewing pertinent documents. A careful review therefore asks not only whether the pictured object or posture resembles the alert category, but also whether the suspected condition existed at that location and time and created actual or potential employee exposure. See OSHA’s documentation guidance.

3. Ask the responsible safety reviewer to assess it
Bring the alert to the designated competent person or the person responsible for the relevant safety program. The right reviewer depends on the hazard and site arrangements; an automated label is not a substitute for an informed assessment of the work. A walk-around can test whether the depicted condition matches the current scene, while discussion with the worker, supervisor, or witnesses can clarify what the frame does not show.
OSHA’s construction guidance describes the competent-person role and verification of a safety plan through walk-arounds and interviews. It is agency guidance, not a vendor workflow. The reviewer should check the relevant plan and work context and decide whether the image shows a hazard, a temporary safe condition, or a visual misread. The 1994 OSHA construction focused-inspection guidance discusses these inspection practices.
4. Record the evidence and the decision
Make a contemporaneous record of what was flagged, what the reviewer observed, who took part, when and where the review happened, what evidence and site requirements were considered, and whether corrective action or follow-up was needed. If the review relies on a worker or witness account, record enough identifying and contextual information to understand the statement later. OSHA’s inspection procedure says compliance officers “shall document interview statements in a thorough and accurate manner; including names, dates, times, locations, types of materials, positions of pertinent articles and witnesses.” This is agency procedural language, not a quotation from a named individual. The relevant procedure is in OSHA’s Field Operations Manual, Chapter 3.

5. Address a real or uncertain hazard, then report the software error
If the condition is real—or remains uncertain and could expose someone to harm—take appropriate steps under the site’s safety procedures while the review continues. Do not wait for a model dispute to be resolved before responding to a plausible risk. If the evidence supports that the alert was false, use the product’s documented route to challenge or report it, if one is available. Keep that feedback separate from the safety record: the safety decision should explain the observed condition and response, while the product report should explain why the model’s alert appears inaccurate.
No AI product is identified here, so its appeal button, retention period, escalation path, or model-feedback mechanism cannot be specified. Check the product’s own documentation or ask the system administrator how to submit a correction and retain the relevant record. Do not assume that reporting an alert to the vendor changes an employer’s safety records or satisfies any site, contractual, or legal procedure.
What evidence helps establish whether the alert is false?
Build the record around the question the alert raises: what condition was present, where and when was it present, and could a worker have been exposed? The following evidence categories are practical review aids drawn from the types of evidence OSHA describes documenting; they are not a claim that every case requires every item.

| Record | What it can clarify |
|---|---|
| Original image or video, with source and timestamp if available | What the system saw, the camera viewpoint, and the moment represented. |
| Exact location and work activity | Whether the alert is attached to the right place and task. |
| Worker-exposure information | Whether employees were present or could reasonably have been exposed to the condition. |
| Worker, supervisor, or witness statements | What happened outside the frame, including task sequence and conditions not visible to the camera. Record relevant names and date, time, and location details. |
| Plans, permits, work rules, safety-program records, equipment information, and measurements | Whether the apparent condition conflicts with a relevant requirement or is consistent with an approved or controlled work condition. |
| Reviewer, rationale, and follow-up record | Who made the decision, what evidence and rule or procedure were considered, and what action was taken. |
These categories reflect OSHA’s inspection documentation approach, which includes exposure facts, witness accounts, images or video, measurements, and relevant site records. The Field Operations Manual, Chapter 4 is guidance for OSHA offices. OSHA states that the manual creates no enforceable duties, rights, or benefits for other parties; it is not a substitute for the regulation, jurisdictional requirement, or project procedure that applies to a particular site. See the Field Operations Manual for the manual’s scope and limitations.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to evaluate the alert without over-trusting the model
When several alerts need review, use the same questions each time rather than treating one convincing image or a confident label as conclusive. The first five checks below are practical criteria inferred from the kinds of facts OSHA says inspectors document. Whether a product supplies a complete audit trail or a correction route depends on that specific product.
- Image quality and coverage: Is the relevant area visible, or does blur, obstruction, angle, or missing context make the apparent condition uncertain?
- Timestamp and location: Do the alert’s time and camera location match the work being reviewed?
- Rule and work context: Is the relevant safety requirement clear, and does the alert account for the task and site conditions?
- Exposure: Was a worker exposed, or could a worker have been exposed, rather than merely appearing near a visual pattern?
- Site-plan consistency: Does the observed condition align with or conflict with the site plan, applicable standard, permit, or work rule?
- Audit trail and correction route: Can a reviewer trace the alert, decision, evidence, and any correction, and does the product document how to report a false alert?
A result should be recorded as a decision based on the available evidence, not as a claim that the AI is generally accurate or inaccurate. The available official sources do not establish a current empirical accuracy rate or false-positive rate for AI construction-inspection systems. Do not use a result from one alert to make an unsupported performance claim about an entire product or class of systems.

Construction hazards and the historical 90% figure
OSHA’s focused-inspection directive dated August 22, 1994, names falls, struck-by, caught-in/between, and electrical hazards as four leading construction hazard categories and states that they account for 90% of deaths and injuries in construction. That is a historical statement in a 1994 directive, not a current-year estimate. It is context for why a plausible hazard deserves prompt attention, not a statistic about AI alerts or a shortcut for deciding whether a particular alert is valid. The figure and the agency’s construction inspection guidance appear in the 1994 directive.
The same guidance discusses competent-person responsibilities and safety-plan practices, including documentation concerning training, permits, hazard reports, inspections, uncorrected hazards, incidents, and near misses. Those records may help establish site context where relevant; the particular documents to consult depend on the task and suspected hazard. OSHA’s stated goal in that guidance is “to make a difference in the safety and health of employees at the worksite.”
What changes by location, hazard, or system
The sources here cover U.S. federal OSHA guidance. State-plan requirements, local rules, contract terms, and site procedures may add requirements, so do not assume federal guidance is the only applicable authority. The exact rule also depends on the condition the system flagged: a suspected fall hazard, electrical issue, struck-by risk, or other condition may call for different standards and evidence.

Quick Recap
Common mistakes to avoid
- Deleting the alert after deciding it looks wrong: preserve the original alert and available media so the decision has a traceable basis.
- Closing a possible hazard because the model has made other mistakes: assess the current condition and exposure on its own evidence.
- Judging only from a still image: verify location, time, task, scene, and worker accounts where relevant.
- Recording only “false positive”: document what was reviewed and why the conclusion was reached, including the applicable site rule or standard considered.
- Assuming a private alert is an OSHA citation: distinguish the system’s internal finding from an agency inspection or enforcement action.
- Assuming all products handle disputes alike: the appeal and retention process is product-specific and is not established by the OSHA sources.
