First, identify where the incorrect detail appears
Central e-Registration (Polish: centralna e-rejestracja) is a Polish digital health appointment service. How to report the problem
- Contact the Ministry of Health through its patient contact page. The Ministry’s Central e-Registration webinar directs patients to the patient contact page on pacjent.gov.pl to report problems.
- For a Central e-Registration question, email [email protected]. The Ministry webinar lists this address for questions about the service. Explain where the incorrect detail appears and ask which team can correct it.
- If the issue is with the mojeIKP app, contact its published app support. The app listing provides +48 22 19 239 and [email protected]. These are app-support contacts; the listing does not establish that this team can edit the underlying patient record.
- Ask what correction steps and identity checks are required. Do not include your PESEL, password, or other sensitive information in an unsolicited message. The published information does not specify what personal data to provide in an initial report.
