Stop or pause the movement that brings on knee or joint pain. “Low-impact” means an activity generally places less stress on joints than higher-impact movements; it does not mean the activity is painless or suitable for everyone. If discomfort is mild and settles, try less time, lower intensity, fewer sessions, or a different activity that causes no or little pain. Do not push through sharp, constant, worsening pain, a limp, substantial swelling, or a hot or red joint.
First, decide whether to stop, modify, or seek help
Notice what the pain is doing, not just which exercise triggered it. A brief, mild ache that improves when you stop is different from pain that is severe, escalating, changes the way you walk, or comes with swelling, heat, redness, or illness. Pain during exercise does not identify its cause, so use symptoms and how they change to guide your next step rather than trying to diagnose yourself.

- Stop the provoking movement: pause the exercise if it hurts. Do not try to finish a set or session by forcing painful repetitions.
- For mild pain that settles: allow symptoms to improve, then reduce the session length, frequency, or intensity before trying again. Choose a movement that causes no or little pain.
- For pain that is sharp, stabbing, constant, worsening, or affecting your walking: stop the activity and seek advice from a clinician rather than continuing to test it.
- For a hot or red joint, significant swelling, or other urgent warning signs: follow the urgent-care guidance below.
The CDC’s archived exercise guidance, last reviewed November 15, 2021, advises contacting a doctor for sharp, stabbing, or constant pain; limping; pain that lasts more than two hours after exercise or worsens at night; symptoms that do not settle with rest or hot or cold packs; or a large increase in swelling, heat, or redness. Those are published warning signs, not a universal rule that everyone should wait two hours before acting. Severe symptoms or other urgent signs need attention sooner.

What to do in the next day or two
Give the painful movement a break
Temporarily reduce or pause the specific activity that provokes symptoms. That does not necessarily mean stopping all movement. NHS advice for joint pain is to rest the affected joint if possible but not to stop moving it completely. Keep any movement gentle and within comfortable limits; if ordinary movement also causes significant pain, or symptoms worsen, get clinical advice.

Use a cold pack for comfort if appropriate
For knee pain, NHS guidance reviewed in 2023 suggests applying an ice pack or a bag of frozen peas wrapped in a tea towel for up to 20 minutes every two to three hours. Never put ice directly on skin. This is an optional comfort measure, not a treatment that identifies or cures the reason for the pain. NHS joint-pain guidance also describes a wrapped cold pack for other painful joints.
Ask before choosing pain medicine
Whether a pain reliever is suitable depends on your health history and any other medicines you take. Ask a pharmacist or clinician instead of assuming an over-the-counter medicine is safe for you. The American Academy of Orthopaedic Surgeons notes that nonsteroidal anti-inflammatory drugs (NSAIDs) may require caution or avoidance for some people, including those with coronary artery disease, heart failure, or chronic kidney disease.

How to return to activity without repeating the same flare
- Wait for the immediate response to settle. Do not restart simply because a short break has passed if pain is still worsening, substantial, or interfering with normal movement. Arrange clinical review if symptoms meet the criteria in the follow-up section.
- Change one demand at a time. When you try activity again, make it shorter, easier, or less frequent than the session that caused pain. This makes it easier to tell whether the adjustment is helping.
- Warm up, use a comfortable pace, and cool down. The CDC recommends starting slowly, paying attention to how your body feels, and increasing difficulty in small amounts. A short, comfortable session is preferable to jumping back into a previous duration or intensity.
- Check the response during and after the session. Stop if pain becomes sharp, constant, or worse, or if you begin limping. Note whether symptoms settle with rest or continue later; do not treat the ability to finish an exercise as proof that it is appropriate.
- Progress gradually only if the activity remains comfortable. Increase the challenge in small increments rather than changing duration, intensity, and frequency all at once. The CDC’s archived 2021 guidance gives six to eight weeks as a general estimate for joints to adapt to a new activity level. That estimate is not a reason to wait through red flags or persistent or worsening pain.
The CDC says “Just 5 minutes” is enough to begin activity and that activity adds up. Its arthritis guidance, dated February 14, 2024, recommends choosing activities that are fun, easy to get to, cause no or little pain, and are ones a person will stick with. Start with what is manageable for you; the five-minute suggestion is an entry point, not a requirement or a target everyone must reach.

A lower-impact substitute can still irritate a particular joint. Judge an option by whether it causes little or no pain for you, how much pressure it puts on the sore area, whether you can access and enjoy it, and whether you can begin gently. There is no universal best alternative.

| Option | What the guidance establishes | How to decide whether to try it |
|---|---|---|
| Water exercise or swimming | The CDC lists water exercise among activities that may suit people with arthritis. For diagnosed knee osteoarthritis, AAOS identifies swimming as a lower-impact alternative to jogging or tennis. | Consider it if you have access and can move comfortably in the water. Stop or modify if it brings on pain. |
| Cycling | The CDC lists cycling among examples of joint-friendly activity. AAOS identifies it as a lower-impact alternative to jogging or tennis for diagnosed knee osteoarthritis. | Try only at a comfortable level and assess your symptoms rather than assuming that cycling will suit every knee. |
| Brisk walking | The CDC lists brisk walking as an example of activity to consider for arthritis. | Use your symptom response and walking pattern as the guide. If pain makes you limp, stop and seek clinical advice. |
These are examples, not a ranking or a promise of pain relief. The 2019 American College of Rheumatology/Arthritis Foundation guideline, published in 2020, recommends exercise for people with hand, hip, or knee osteoarthritis but says no single exercise form has been established as best. It emphasizes preference and access rather than a universal hierarchy. That guidance applies to diagnosed osteoarthritis; it does not establish what is causing new pain in someone without that diagnosis.
When knee or joint pain needs medical attention
Get urgent help for serious knee symptoms
NHS knee-pain guidance advises urgent assessment for a very painful or badly swollen knee; inability to move it or bear weight; a changed knee shape; locking or giving way; or feverish illness alongside a hot or red knee. NHS joint-pain guidance also treats a hot, swollen joint with fever or feeling generally unwell as urgent. After an injury, severe pain, inability to walk, a displaced joint, or numbness or tingling calls for emergency care under NHS guidance.

- NHS knee guidance advises seeking help if knee pain is not improving within a few weeks.
- NHS joint-pain guidance advises seeing a GP if pain stops normal activities or sleep, is worsening or keeps returning, has not improved after two weeks of home care, or is accompanied by morning stiffness lasting more than 30 minutes. The NHS page was last reviewed in 2026.
- The CDC’s archived 2021 exercise guidance advises a doctor’s review for the warning signs described above, including pain lasting more than two hours after exercise or worsening at night. Use this as dated published guidance, not as a threshold that overrides more severe symptoms.
If you have arthritis or another diagnosed condition, a clinician or physical therapist can help tailor an activity plan. The CDC notes that people with arthritis may benefit from working with a physical therapist when starting activity. Ask for individualized advice if you are unsure how to modify movement or symptoms keep returning.

What exercise-triggered pain can—and cannot—tell you
Pain that starts during a low-impact activity does not, by itself, prove that you have arthritis, a strain, or a particular structural injury. NHS guidance lists possible causes of joint pain including strain, osteoarthritis, bursitis, rheumatoid arthritis, gout, and joint infection, and advises against self-diagnosis. A single symptom description is not enough to distinguish among these possibilities.
If a clinician has diagnosed osteoarthritis, suitable exercise can be part of care: AAOS says exercise can improve pain and function in knee osteoarthritis. The ACR/Arthritis Foundation guideline also recommends exercise for hand, hip, and knee osteoarthritis. But the guideline says there is “no uniformly accepted level of pain at which a patient should or should not exercise” and advises common-sense shared decision-making between patient and treating clinician. That statement is not permission to push through new, severe, or unexplained pain; use it to discuss an appropriate plan with your clinician.
Getting help to make activity workable
If pain repeatedly interrupts activity, normal tasks, or sleep, ask a clinician whether a physical therapist or local musculoskeletal service could help assess your movement and adapt your plan. This is particularly relevant when you have arthritis, are returning after a long period of inactivity, or cannot identify a comfortable way to stay active. A professional can account for your diagnosis and health history—details that general exercise examples cannot address.
