A little tightness or mild pressure when you start a new Invisalign aligner can be normal; Invisalign says it usually lasts a day or two. Sharp or severe pain, a tray that stays loose or will not seat, or an aligner that is damaged or badly distorted needs a different response: do not force it or change trays on your own—contact the orthodontist supervising your treatment. Sudden severe pain with facial swelling, fever, or signs of infection requires urgent attention.
What to do right now
- Check the aligner. Look for cracks, sharp edges, or obvious distortion. Do not keep wearing an aligner that is sharp, painful, or badly distorted; the American Association of Orthodontists (AAO) advises against it. AAO guidance on orthodontic emergencies.
- Notice whether the problem is easing or continuing. Mild tightness after starting a new tray may ease within a day or two. Persistent pain, a loose feeling, or a tray that will not seat warrants advice from the orthodontist overseeing your case. A short description cannot establish why the fit is off.
- Contact your orthodontist for instructions. Tell the office which tray you are using, when the problem began, whether the aligner is damaged, and what you feel. Follow their direction about whether to wear the current tray or use a previous one. Do not force the aligner, file it, or independently advance or backtrack in the sequence.
- Get urgent help for warning signs. Sudden severe pain together with facial swelling, fever, or signs of infection is not routine tray tightness and needs urgent attention. The AAO identifies these as urgent concerns.

A cracked, sharp, or badly distorted tray
The AAO says not to continue wearing an aligner or retainer that is sharp, painful, or badly distorted. Contact your orthodontist for next steps. The AAO notes that the clinician may advise temporarily using the previous aligner if it still fits comfortably while arranging a replacement; do this only if your orthodontist advises it. Should you use chewies or go back to your previous tray?
Only use chewies if your own orthodontist has recommended them and explained how to use them. One orthodontic practice lists chewies as a seating aid and advises patients to contact the office if trays are not tracking, but that is practice-level guidance—not a universal Invisalign or AAO instruction. Likewise, do not decide on your own to return to an older tray. The AAO says your orthodontist may advise a previous aligner temporarily if it fits comfortably. Ask the clinician supervising your treatment what to wear while the issue is addressed.
How to avoid making the problem harder to assess
- Follow the wear and change schedule set for your individual treatment; do not add wear time or change trays to troubleshoot without the provider’s direction.
- Attend the check-ins arranged by your orthodontist. Invisalign describes provider oversight and periodic progress checks as part of treatment. Invisalign information about finding a provider and check-ins.
- When you contact the office, describe the tray number, when you began wearing it, whether it is damaged, and whether the discomfort is mild, sharp, or severe. This helps the clinician decide what guidance you need.
