If an AI agent exposes sensitive information online, first stop it from exposing more: restrict or suspend the affected agent, tools, endpoint, or connected service, and revoke or rotate credentials that may be compromised. Then preserve evidence, establish what was exposed and who could access it, reduce the exposure, and promptly assess notification duties with privacy and legal counsel. An exposure is not necessarily a cyberattack; it can also result from permissions, configuration, workflow, or misuse.
1. Contain the exposure without losing evidence
Put a human incident lead in control of consequential actions. Do not rely on the potentially affected agent to investigate or remediate itself using the same permissions; validate decisions and authorization independently.

- Limit the route the information took. Suspend the agent or restrict its capabilities, publishing route, endpoint/API, or connected service while responders assess safe containment. Disable or narrow the implicated tools, integrations, permissions, and credentials. OWASP’s GenAI Incident Response Guide 1.0 (July 28, 2025) advises revoking or rotating keys and tokens associated with a compromised model endpoint, considering limits on provider API interactions, and monitoring for suspicious use.
- Review credentials and access. If a key, token, password, or other credential may have been exposed, rotate it and review its access. The FTC warns that stolen credentials can leave systems vulnerable until they are changed.
- Coordinate system isolation. Preserve evidence while containing access. The FTC advises against turning affected machines off before forensic experts arrive; coordinate isolation or shutdown with responders where possible. Do not let evidence preservation become a reason to leave an active leak running—get incident-response help to manage the trade-off.
OWASP’s AI Agent Security Cheat Sheet recommends least-privilege access, permissions scoped per tool, and explicit authorization for sensitive actions. CISA and partner agencies’ agentic-AI guidance, announced May 1, 2026, also cautions against broad or unrestricted agent access, especially to sensitive data or critical systems.
2. Preserve records and determine what happened
Start a time-stamped incident record. Capture the discovery time, who reported it, the affected agent and version, relevant prompts and tool calls where retained, endpoint and integration details, publication URLs, screenshots, relevant logs, and actions already taken. Keep the record in an appropriately restricted location; avoid copying the sensitive information into new tickets, chats, or reports unless necessary.

Preserve logs and other relevant records as containment and remediation proceed. The FTC’s Data Breach Response: A Guide for Business cautions: “Do not destroy any forensic evidence in the course of your investigation and remediation.” If your team cannot confidently preserve evidence or establish scope, engage forensic incident responders.
Build a scope, not just a list of URLs
Work out what information was involved, where it came from, when it was exposed, which people or organizations may be affected, and what paths could provide access. Determine whether the information was published, merely accessible, actually viewed, acquired, or copied where that can be established. Review logs and access controls to identify who could reach it and whether access was necessary. Separate confirmed facts from unknowns in the incident record.
For a compromised GenAI endpoint, OWASP’s incident-response guide recommends reassessing outputs produced during the compromise period and considering an investigation by the provider and a detailed post-incident report. A forensic specialist can help capture affected-system evidence, trace access, and outline remediation.

Apply data-specific assessment where relevant
If unsecured protected health information (PHI) may be involved, the U.S. Department of Health and Human Services’ HIPAA breach guidance describes a risk assessment that considers the nature and extent of the information, the unauthorized recipient, whether the information was actually acquired or viewed, and how much risk was mitigated. This framework applies to HIPAA-regulated entities and unsecured PHI; it is not a general test for every data incident.
3. Remove the exposure and reduce further harm
Take improperly posted information off websites and systems your organization controls. Search for other copies, contact the operators of sites hosting them, and request removal. The FTC notes that search engines may cache information and advises organizations to contact search engines about content posted in error. Removing the original does not establish that every copy has disappeared, so describe removal status only to the extent you have verified it.
Communicate carefully. Give affected people useful protective information without repeating the sensitive data or creating additional risk. The FTC cautions against misleading statements, withholding key protective details, or publicly sharing information that could put consumers at further risk.

4. Assess who must be notified—and when
Bring together security or incident response, privacy, legal, IT, communications, and the relevant business owners. Consult counsel experienced in privacy and data security. If information was held on behalf of a business customer, notify that customer as required by the applicable agreement and law. Consider contacting law enforcement where appropriate.
Notification is not determined by the fact that an AI agent was involved. Map the information types, affected people, locations, the organization’s role, and applicable contracts to the relevant laws and regulator rules. U.S. state breach-notification laws and federal or sector-specific requirements may apply. OWASP also recommends reviewing provider terms, breach-notification obligations, and regulatory requirements. The following are conditional examples, not a complete legal checklist:

EU: GDPR Article 33, where GDPR applies
A controller generally must notify the competent supervisory authority without undue delay and, where feasible, within 72 hours of becoming aware of a personal-data breach, unless the breach is unlikely to pose a risk to individuals’ rights and freedoms. Article 33 also addresses documenting breaches and the content of a notification. Determine whether the GDPR applies to the organization and incident before relying on this rule.
United States: HIPAA, where the breach-notification rule applies
Following a breach of unsecured PHI, covered entities generally must notify affected individuals without unreasonable delay and no later than 60 days after discovery. HHS notification and, in certain circumstances, media notification also apply; business associates have duties to notify covered entities. The rule includes exceptions and detailed conditions. Confirm the facts and current requirements with counsel rather than applying the deadline automatically.

5. Find the cause and prevent a repeat
Once the immediate exposure is controlled, determine how it happened. Possible causes include excessive permissions, prompt injection, compromised credentials, a misconfigured connector, sensitive information in agent context, outputs, or logs, an unsafe publishing workflow, or a provider or tool issue. OWASP’s agent-security guidance describes risks including exfiltration through tool calls, API requests, and outputs.
- Reduce the agent’s permissions to the minimum needed; scope access per tool and separate tools by trust level.
- Require explicit human approval for sensitive operations, and validate outputs before displaying or executing them.
- Filter sensitive information and keep memory and context isolated across users.
- Monitor for abnormal behavior and reassess third-party access. Verify that providers have fixed any relevant vulnerability, and review whether network segmentation limited the spread.
Use the incident to improve the organization’s wider security process. NIST SP 800-61 Rev. 3, published in April 2025, places incident response within the risk-management activities of the NIST Cybersecurity Framework 2.0. NIST SP 1800-29, published February 23, 2024, is a practical guide to detecting, responding to, and recovering from data-confidentiality attacks.
