What SMART Health Cards Can Verify—and What They Cannot

Verifiable clinical information is FHIR-based clinical data issued as a credential tied to a person. Verification can check the credential, but not guarantee the source record is clinically accurate.

Verifiable clinical information is clinical data represented in the FHIR format and issued as a verifiable credential tied to a person’s identity. In the HL7 SMART Health Cards and Links standard, that can make it possible to check who issued the credential and whether it has been changed—but it does not prove that the underlying medical record was entered correctly.

What the term means

The Verifiable Clinical Information (VCI) coalition uses the term for signed clinical data bound to an individual identity. In the SMART Health Cards and Links implementation guide, that information is represented using FHIR and presented in a form based on verifiable credentials. The data included depends on the purpose: there is no single fixed bundle that every credential must contain. VCI describes its work as harmonizing standards and developing implementation guides for health credentials.

Identity binding is essential: a credential must be associated with the person whose information it contains. The specific method for making that association depends on the implementation.

What verification does—and does not—establish

Credential verification can help establish that a credential came from the stated issuer and has not been altered after issuance. It is different from validating the clinical truth of every item in the source record. The organization that performed a test or vaccination is responsible for the accuracy of that record and for correcting errors. The SMART Health Cards FAQ directs people with incorrect information to the organization that provided the test or vaccination, or to the relevant registry.

A credential also does not guarantee acceptance. Receiving organizations may set different rules, so a person should check what a particular organization accepts and keep other ways to provide the required information.

How SMART Health Cards and Links present information

The HL7 SMART Health Cards and Links FHIR Implementation Guide describes two approaches. A card is a compact credential that can be carried as a QR code on a phone or printed on paper. A SMART Health Link can provide access to a larger amount of information stored in the cloud, with limited-time or ongoing sharing; access may also be protected by a PIN sent separately. See the HL7 SMART Health Cards and Links Implementation Guide.

Information format A smaller amount of information is carried in a QR credential. Can provide access to more information stored in the cloud.
Access period The guide describes a carryable credential; it does not specify link-style time-bounded or ongoing access for the card. Can support limited-time or ongoing access.
Without a smartphone Can be printed on paper; an issuing organization may provide a paper copy. The cited guide describes cloud-based access; it does not establish a paper equivalent.

The guide gives examples such as proof of immunization, insurance-member information at check-in, a child’s immunization history, lab results, and time-limited or ongoing provider access to health data. These are implementation examples, not a promise that every issuer or receiving organization supports or accepts each use.

Standard status and scope

The published HL7 guide is version 1.0.0, is based on FHIR R4, and is marked trial-use with maturity level 4. It covers both formally attested facts for which verification matters and health information shared mainly for convenience or communication. This is an implementation guide, not a universal mandate; acceptance, registry access, and issuer directories vary by organization and jurisdiction.

What to do if a credential is wrong or unavailable

  1. If the record is inaccurate: contact the organization that performed the test or vaccination, or the relevant registry, to ask about correcting the source record.
  2. If you need an updated credential: request it from the issuing entity. A SMART Health Card does not update itself when the underlying record changes.
  3. If you do not have a smartphone: ask the issuer whether it can provide a paper copy.
  4. Before sharing: check what the receiving organization accepts, and share the credential only with organizations you trust with that information.
