What Next-Generation Sequencing Can Reveal—and What It Cannot

Next-generation sequencing can examine large amounts of genetic material, but a detected variant is only a starting point. Learn what short- and long-read approaches can reveal and what makes results meaningful.

Next-generation sequencing (NGS) lets researchers read large amounts of genetic material at once, making it possible to look for many kinds of variation across much larger stretches of DNA than a single-gene test examines. That broader view can help researchers and clinicians investigate disease, but finding a variant does not by itself show whether it caused a condition or what care a person should receive.

What is next-generation sequencing?

Sequencing determines the order of the chemical bases in DNA or RNA. NGS is a set of technologies that can sequence many fragments in parallel, then use software to assemble or compare those fragments. Depending on the test, the target might be a selected set of genes, a wider portion of the genome, or other genetic material.

The result is genetic data, not an interpretation on its own. Analysts compare the sequence with a reference and look for differences, often called variants. A variant may be common and harmless, may contribute to disease, or may remain uncertain because the evidence is incomplete. Its significance depends on the variant, the person’s clinical context, and the strength of available evidence.

How do short-read and long-read sequencing differ?

Sequencing instruments read DNA in fragments called reads. Read length affects how much surrounding sequence is visible at once, which in turn affects what kinds of genetic differences can be resolved. The National Human Genome Research Institute (NHGRI) describes typical short reads as 50–200 base pairs and long reads as 10–100 kilobases and longer.

Approach Typical read length What the read length can help with
Short-read sequencing 50–200 base pairs, as described by NHGRI Examining many regions efficiently; short fragments can be harder to place or interpret when a region is repetitive or a change involves a large or complex stretch of DNA.
Long-read sequencing 10–100 kilobases and longer, as described by NHGRI Seeing longer stretches at once can help resolve structural variation and characterize full-length RNA transcripts.

These are general read-length descriptions, not a platform ranking or a guarantee that every test using one approach can detect every variant type. Performance depends on the assay, the genomic region, the analysis pipeline, and validation for the intended use.

What can long-read sequencing reveal?

Some genetic changes affect more than a few adjacent bases. Structural variants can involve large sections of DNA that are deleted, duplicated, rearranged, or otherwise altered. A short fragment may not show enough surrounding sequence to determine how such a change is organized. Longer reads can span more of the region, helping researchers identify and characterize structural variation.

Long reads can also support sequencing of full-length transcriptomes. A transcript is an RNA copy produced from a gene; reading a longer transcript can help show how its parts are joined together. This gives researchers another way to study how genes are expressed, beyond examining DNA sequence alone. It does not, by itself, establish that a transcript difference causes disease.

As an example of the scale of long-read research, NHGRI reported on its page updated July 27, 2026, that the Human Pangenome Reference Consortium released 47 nearly complete, haplotype-resolved human genomes from diverse backgrounds using multiple long-read technologies. This is a research resource, not a count of people diagnosed or a measure of clinical benefit.

How can sequencing information help researchers and clinicians?

The FDA describes NGS tests as capable of rapidly sequencing large sections of a person’s genome and identifying variants that may be useful in diagnosing, treating, or understanding disease. In precision medicine, genetic information can contribute to decisions about a person’s care, including investigation of rare genetic diseases and pharmacogenetics—the study of how genetic differences may relate to responses to medicines.

For example, when clinicians suspect a rare inherited condition, broad sequencing may identify a candidate variant in a gene relevant to the person’s symptoms. That finding can help focus further interpretation or testing. It is not an automatic diagnosis: clinicians must consider whether the evidence supports a link between the variant and the condition and whether the result fits the person’s clinical picture.

In research, sequence data can be studied alongside clinical outcomes, electronic health records, imaging, and proteomic data—the study of proteins—to investigate disease and improve interpretation. These combinations can help researchers ask questions that sequence data alone cannot answer, but they also require appropriate data quality, analysis, and governance.

What makes an NGS result reliable and meaningful?

A useful result depends on more than the ability to produce sequence data. The FDA distinguishes analytical validity—whether a test accurately and reliably detects what it claims to detect—from clinical validity—whether the result is meaningfully associated with a health condition or other clinical characteristic. A finding can be technically real without being clinically interpretable.

  • Analytical performance: The assay must be evaluated for its intended use, including the variant types and genomic regions it is meant to assess.
  • Reproducibility: Results should be comparable across methods and testing sites. Reference materials and concordance checks help assess whether different approaches produce consistent results.
  • Interpretation: Evidence about a variant may be strong, limited, conflicting, or absent. A test report must be read in context rather than treated as a stand-alone explanation.
  • Care implementation: Integrating comprehensive sequence information into healthcare can require interpretive tools, institutional policies, and changes to clinical workflows.

In a 2021 notice, NIH and FDA identified needs including sufficiently characterized and diverse reference samples and datasets, sustainable resources, analysis tools, and standards for validation and interpretation. The agencies also described gaps in diverse, well-characterized reference materials and consensus standards at that time; this is a dated assessment, not a newly measured 2026 statistic.

How should researchers choose an approach?

The right sequencing strategy depends on the question, not on a general claim that one read type is best. Researchers and clinicians consider the read length needed, whether the question involves structural or complex variants or full-length transcripts, and whether the assay has been validated for its intended use.

  • If the question concerns many targeted regions or smaller sequence differences, evaluate whether the selected assay can detect the relevant variant types in those regions.
  • If a suspected change may span a large or complex region, consider whether longer reads could help resolve its structure.
  • If the goal is to study full-length transcripts, assess whether the method and sample type are suitable for that RNA analysis.
  • For clinical testing, weigh analytical validation, clinical evidence, interpretation resources, and how results will be handled in the care setting.

There is no universal platform winner, general cost ranking, or single turnaround time established across NGS applications. The useful comparison is between validated assays for a specific research or clinical question.

What are the limits and ethical questions?

Broader sequencing can produce findings beyond the question that prompted testing. Some results may be uncertain, incidental, or unexpected. Deciding whether and how to return such findings raises ethical, legal, and psychosocial questions, including what information a patient wants to receive and how it could affect family members. NHGRI’s Clinical Sequencing Evidence-Generating Research program identifies these issues, alongside the challenge of integrating results into routine care.

Clinical utility—the extent to which using a test improves decisions or outcomes—is an evidence question as well as a technology question. NHGRI notes that evidence for promising clinical uses is still accumulating. The FDA’s discussions of NGS also emphasize validation and clinical evidence. Accordingly, no single diagnostic yield, cost, turnaround time, or patient-outcome improvement should be assumed to apply to all NGS tests.
